Shipping Agreement Form for International Orders

For us to ship products to you in a country outside the United States, you must fill out this form, sign it, and fax or email it to us. If you are the cardholder, we must have your signature. If not, we must have the signature of the cardholder. You should place your ordered through PayPay. You can reach PayPal by clicking on the image of the product(s) you want to order. We are grateful for your cooperation

Agreement
If products Franconia Ltd (owner of ThyroidScience.US) ships to me in my country, which is outside the borders of the United States, do not arrive for any reason whatsoever, I agree that I, __________________________________________ (please print your name), will take full responsibility for the loss of the products and the shipping cost. I agree, then, that I accept the risk of the loss of the products and the shipping cost.
