Summary of Rules and Regulations

To enter to win a Describe Prize (ie. an Acme Computer System) (retail value Retail Value of
Prize (ie. $1,500.00)), no purchase is necessary. Simply visit our web site and fill out the
application form on-line. Contest closes Date Contest Closes (ie. August 1, 2000) at the end of
the business day. There is a maximum of 1 entry per household, and only completed entries will
be considered eligible. A random draw from all eligible entries will take place on Date of
Random Draw (ie. August 31, 2000). A time limited skill-testing question must be answered to
win, and will be administered over the telephone. The winner must also sign a standard release
form confirming compliance with contest rules. Chance of winning depends on the number of
eligible entries received. This contest is open to all residents of Enter Jurisdiction (ie. New York)
who have reached the age of majority, except employees of Enter Name of Business(es)
Sponsoring Promotion, their affiliated companies, the independent judging organizations and all
persons with whom they are domiciled.

Complete Rules and Regulations

1. This contest is open to all residents of Enter Jurisdiction (ie. New York) who have
reached the age of majority, except employees of Enter Name of Business(es) Sponsoring
Promotion, their affiliated companies, the independent judging organizations and all
persons with whom they are domiciled. When you complete the contest entry form you
are automatically entered in the contest.
2. Limit of one entry per household. Eligible entries must be received no later than end of
business day on Date Contest Closes (ie. August 1, 2000), the contest closing date.
3. No purchase necessary.
4. The contest prize is one (1) Describe Prize (ie. an Acme Computer System). Approximate
value Retail Value of Prize (ie. $1,500.00). Taxes and preparation expenses included.
Enter Name of Business(es) Sponsoring Promotion reserves the right to substitute a
product of equivalent or higher value in the event that the prize described herein is
unavailable. The prize will be delivered to the winner's residence in the United States of
America.
5. A random draw will be conducted at Time of Draw (ie. 2:00 p.m.) on Date of Random
Draw (ie. August 31, 2000) in Place of Draw (ie. New York, New York) from all eligible
entries received on or before the contest closing date.
6. The chance of being selected will depend on the total number of eligible entries received.
If the selected entrant cannot be contacted by phone at the phone number listed on the
entry within 30 days of being selected, he/she will not be eligible to win the prize and an
alternate entrant will be randomly selected.
7. The prize will be awarded to the first person whose entry is drawn, who correctly
answers, unaided, a time-limited arithmetical, skill-testing question to be administered by
telephone at a pre-arranged, mutually convenient time, and who complies with the contest
 rules. In the event that the first selected entrant does not correctly answer the skill-testing
question and comply with the contest rules, he/she will not receive the prize and another
entrant will be selected until the prize is awarded.
8. To be confirmed as the winner, the selected entrant will also be required to sign a
declaration and release form that confirms compliance with the contest rules and
acceptance of the prize awarded, releases the contest sponsors and their agents from any
liability and consents to the use of his/her name, voice and/or photograph or likeness
without compensation, in any publicity carried out by Enter Name of Business(es)
Sponsoring Promotion in connection with this contest.
9. The prize is not transferable or convertible into cash or otherwise and must be accepted
as awarded.
10. The right is reserved to terminate or withdraw this contest at anytime without prior
notice.
11. This contest is subject to all and Federal, State and Municipal laws and regulations. Void
where prohibited.
12. Enter Name of Business(es) Sponsoring Promotion and its affiliates accept no
responsibility for stolen, lost or misdirected entries. Enter Name of Business(es)
Sponsoring Promotion and its affiliates accept no liability or responsibility in connection
with injuries, loss or damage of any kind arising out of this contest or prize. Entry forms
obtained from unauthorized sources or that are illegible, mutilated, altered, mechanically
reproduced, forged, counterfeited, or irregular in any way will be considered void.
