Members include 12 clinic organizations which provide primary care, women's care and behavioral health services, as well as health education, to medically underserved residents of the area. The Community Health Navigator (CHN) project being coordinated by CCHN is a collaboration of culturally competent social service agencies working with community health providers. Paid and volunteer CHNs will provide access to health education, identify families in need of health interventions, navigate those families through the health system and help them find permanent health homes.
