Neumann University NUR 330 Mental Health Nursing Guidelines for Process Recording

The student uses this learning tool to increase self-assessment skills and gain insight into own personal style of communicating. Identifying patterns of interaction, whether therapeutic or non-therapeutic, can assist the student in developing better communication skills.

The Process Recording will be used by the student to: 1. improve and enhance communication skills. 2. observe and interpret non-verbal behavior. 3. develop increased awareness in the therapeutic use of self. 4. assist in the nursing process and begin formulation of a care plan. Note: SEE Sample Process Recordings: Table 10-4 and Table 19-7, Varcarolis, E. & Halter, M. (2010). COVER PAGE 1. Identify demographic data: see Process Recording template. 2. Describe the patients physical appearance and attitude. 3. Describe the physical environment. Record how and where the interaction
takes place: Note the use of personal space, noise level, number of people in the area, and other environmental/situational factors.

COLUMNS in the Process Recording: First 2 columns are due the 1st clinical week. Completed Verbal Communication and Non-Verbal Communication columns to be submitted at post-conference of 1st clinical day.
Verbal Communication next segment, Non-verbal Communication* QUOTE verbatim or word-for-word dialogue. [e.g. Student: (verbal statement) followed by the Patient: (verbal response) ] DESCRIBE body posture, eye contact, and gestures by the patient and the student.

These should be on the same line with Verbal Communication (See Table 10-4)

Thoughts and Feelings* IDENTIFY thoughts and feelings of the student during each segment of the interaction. Analysis of Communication* NAME the communication technique ** (verbal and nonverbal) used by the student for each segment. (** REFER to Tables 10-2 and 10-3) ANALYZE how technique used may be therapeutic or non-therapeutic at this point in the conversation. ** 1. ASSESS your use of active listening skills (see Handout) and

communication techniques.

2. NOTE your ability to establish rapport and to attend to the patients

verbal and nonverbal cues.


3. STATE how conversation drew to a close. NOTE if you used techniques

of a) summarizing points of discussion; and b) acknowledging contributions of patient.

The Process Recording with the Cover Page should be at least 4 typed pages.
Completed Process Recording is due the 2nd clinical week by 7 am.
