FORM OF AGREEMENT FOR ENGAGEMENT OF CONSULTANT

This AGREEMENT is made the BETWEEN of (the Client) AND of (the Consultant) THE Client engages the Consultant to provide the professional services set out in Appendix A and agrees to pay the Consultant as described in Appendix B and to undertake its other obligations set out in this Agreement. THE Consultant agrees to perform the Services on the terms of this Agreement. The following documents shall form the Agreement in order of precedence:
