1. PURPOSE :
1.1 To improve patient outcomes by educating patient and families. 1.2 To promote recovery, adjustment and health behavior.

2. DEFINITION :
2.1 The process of informing a patient and his family about a health matter, to secure informed consent, patient cooperation and a high patient compliance .

3. POLICY :
3.1 All patients identified family members and / or significant others are provided with appropriate education and training pertinent to the diagnosis, problems or needs identified during initial and on-going assessment. 3.2 Asessment of needs, readiness, and ability to learn will take into account the individuals literacy and educational level , age specific / developmental needs, cultural considerations, emotional barriers motivators, physical or cognitive limitation and financial impact of care decisions. 3.3 The Nurse and Physician or designated staff are primary coordinators of patient / family education, and have responsibility for ensuring the patient receives appropriate educational activities, outpatient education will be rendered by competent staff as dele gated by individual departments / programs.

3.4 The patient and family are provided specific knowledge and skills to cope with health care needs : 3.4.1 Expanding the patients knowledge and understanding of their diagnosis and treat ment needs. 3.4.2 The safe and effective use of equipments ordered for their use. 3.4.3 Dietary requirements and any potential interaction with medications.

3.4.4 How and when to take medications in a safe and effective manner and any specific precautions related to their needs. 3.4.5 Instruction in rehabilitation techniques to facilitate adaptation to and / or functional independence in the enviroment. 3.4.6 Directions on how to contact and how to contact people i.e. Physicians outside the agencies, etc for further information regarding their care and treatment . 3.4.7 Providing an opportunity for the patient / or family to ask questions that allow them to make informed decision concerning their care and treatment . 3.4.8 Clearly informing patients and families as to what their responsibilities are in the pa tients care. 3.4.9 Providing information about rehabilitation potential and resources. 3.4.10 Promoting maximum achievable recovery and functioning. 3.5.The patient/family education section will serve as a resource to review hospital wide policies related to patient education, to coordinate patient / family education efforts inter-depart mentally, and to plan continuous quality improvement activities related to patient / family education. 3.6. Patient education requires documentation in the patients medical record, this will be done in the patient / family education section of the chart . 3.7. Education for patient / family must be based on assessment of their ability to learn, based on the following : 3.7.1 3.7.2 3.7.3 3.7.4 3.7.5 3.7.6 Cultural and religious practices. Emotional barrier. Desire and motivation to learn. Physical and / or cognitive limitations. Language barriers. Education topics.

3.8.Education include instruction in the specific knowledge and /or skills the patient/family needs to meet the patients ongoing health care needs, including but not limited to the following : 3.8.1 Current health status. 3.8.2. Safe and effective use of the medication. 3.8.3. Instruction on potential Drug - Food interactions and counselling on nutrition intervention and / or modified diet as appropriate. 3.8.4. Safe and effective use of medical equipments. 3.8.5. Instruction in rehabilitation techniques to facilitate adaptation to and /or functional independence in the enviroment. 3.8.6. Access to available community resources. I -THE IMPORTANCE OF EDUCATION FOR THE PATIENT AND FAMILY :

1- Facilitate the patient / family understanding of the patients health status and health care
options selected.

2- Increases patient / family potential to follow the therapeutic health care plan. 3- Maximizes care skills. 4- Increases patient / family ability to cope with the patients health status, prognosis, outcome.

5- Enhances the patient / family role in continuing care and promoting a healthy life style. 6- Teaches the patient / family skills and techniques to aid in improved out patient functioning. 7- Encourages participation in decision - making about health care options.
II - PROVIDE EDUCATIONAL RESOURCES : 1.- One-to-One Presentations. 2- Group Class. 3- Video Tape. 4- Pamphlets and Patient Handouts. III - GIVE A CLEAR DISCHARGE INSTRUCTIONS : Every patient should received discharged instructions in how to use her / his medications / appoint ment / dietary instructions.etc. IV - INDIVIDUALIZED EDUCATIONS : The treatment team adapts the education to the appropriate age, culture, language and individualizes it for the specific patient / family. The patient and family educational process will be interdisciplinary as appropriate to the plan of care. V - MEDICAL RECORD INCLUSION : The treatment team documents the education of the patient / family in the patients medical record. # Such documentation must include the following : A - Assessment and identification of educational needs. B -The intervention the staff member used to meet those needs. C -The patient / family understanding of the instruction or education. VI - DATA ENTRY : Documentation of patient education is initiated by nursing within eight (8) hours of the patients admission. Documentation of patient education is initiated by social services within seventy-two (72) hours of the patient admission. VII - NURSING DOCUMENTS :

The education should be related to the patients problems as identified on the treatment plan.
