Title: PROTECTING VULNERABLE CHILDREN AND ELDERLY PATIENT Applies to: All Nursing Staff Effective Date: 01-01-1432 H

1. PURPOSE:

1.1 To provide nurses guidelines for protecting vulnerable children and elderly patients.
1.2 To provide safe and secured environment for those who are unable to protect themselves.

2. DEFINITION:
2.1 Refers to the safeguarding of elderly and children , who are extremely susceptible to physical and psychological harm .

3. POLICY: 3.1 Upon admission the client should be assisted, complete medical history to be taken, any history of fall, psychiatric illness either the client needs a watcher or a guardian.

4. PROCEDURES: 4 - A General : 4.1 Participation of children / familes elderly / family in decision affecting their own medical treatment. 4.2 Environmental Safety - all staff will implement interventions to create a safe environment ( Supervisors Environmental Rounds ). 4.3 Assess and document patients fall risk upon admission, change in status or transfer to another unit. 4.4 Assign the patient to a bed that enables the patient to exit towards his / her stronger side Approval whenever possible.

4.5 For elderly patients. Assess the patients coordination and balance before assisting with transfer and mobility activities. Bowel and bladder programs to decrease urgency and incontinence : 4.5.1 Use treaded socks and / or non - skid footwear.

4.5.2 Approach patient toward unaffected side to maximize participation in care. 4.5.3 Transfer patient towards stronger side. 4.5.4 Actively engage patient and family in all aspects of fall prevention. Teach patient use of grab bars. 4.5.5 Instruct patient with proper medications time, dose, side effects and management . Instruct the patient to call for help before getting out of bed. Demonstrate nurses call system. 4.6 Lock all movable equipment before transferring patients. 4.7 Orient the patient to the environment, especially the location of the bathroom. 4.8 Individualize equipment specific to patients needs. 4.9 Provide physically safe environment ( eliminate spills, clutter, electrical cords, and un necessary equipment.) 4.10 Minimization of the hospital stay duration by planning patients discharge needs. 4.11 Consistent, supportive and nurturing care which meets the emotional and psychosocial needs of patients, and fosters open communication. 4.12 Do comfort and toileting rounds as per the condition of the patient. 4 . B To prevent abduction of children NURSERY. 4.1 Nurses should observed strict adherence to a newborn identification system. Matched ID. band must be presented to Nursery Staff, when mother or significant other comes to Nursery for breastfeeding, if any doubt, staff should accompany the baby to the mothers room to confirm. 4.2 Mothers should be instructed to only release their baby to staff wearing appropriate identification. 4.3 Babies should only be transferred to other department per bassinet / crib. Anyone carrying a baby in the hallway should be inspected. 4.4 During visiting time nursery door should be monitored and locked, an authorized staff will always be present. 4.6 If the child cant be found in the mothers room or the Nursery, and the nurse has sus picion that infant is missing, inform your Head Nurse immediately, staff will check every room in the unit. 4.7 Simultaneously the following actions will follow : 4.7.1 Communication to the Head of Security and Head of the Department. 4.7.2 Security Staff should stop the flow of traffic going out of the hospital until authorities arrive. Security staff will search around the hospital. 4.7.3 Once the abduction has been confirmed, the attending physician should notify the parents. 4.7.4 All staff on duty when the abduction occurred will remain in the unit until authorities completed proper questions. 4.7.5 Document the incident from the discovery of the abduction until infant is located. Incident Report / OVR should be accomplished.
