1. PURPOSE: 1.1 To make the patient welcome and aware of unfamiliar surroundings. 1.2 To take baseline assessment of objective and subjective symptoms. 1.3 To document statistical information.

2. DEFINITION: 2.1 Refers to the formal acceptance by a hospital or other inpatient health care facility of a patient who is to be provided with room and continuous nursing service or care.

3. POLICY :

3.1 A written doctor order is required to admit patient to the hospital. 3.2 All patients admitted must receive equitable treatment and care. 3.3 All admitted patients will be provided with standard information to enable them to be
safely oriented and feel secure in the environment. 3.4 A copy of patient rights and responsibilities will be provided for every admitted patients.

4. PROCEDURES: 4.1.Prepare the room and bed when informed of admission. 4.1.1 Admitting Department of ER or OPD will inform the unit concerning the admission. 4.1.2 Welcome the patient and family with courtesy, introduce yourself and other staff. 4.1.3 Verify patient identity by asking the name and checking the identification band. 4.3 Take care of patient belongings. Send valuables home with relatives or endorsed to
Social Worker ( see Policy and Procedures for Patients Valuables.). 4.4 Orient to the surroundings, facilities and equipment.

4.5 Take baseline vital signs, weight and height. 4.6 Perform the initial nursing assessment. It must be completed within the shift on which
they are admitted. 4.7 Document subjective and objective data in nurses notes

5..FORMS AND EQUIPMENTS : 5.1 A clean patient area, bed, locker, over-bed table, scale and gown.

5.2 Sphygmomanometer, Stethoscope, Thermometer. 5.3 Drinking water, unless NPO is ordered.
5.4 Emergency equipment, oxygen, suction, etc, if needed. 5.5 Patients Chart / File. 5.6 Census sheet. 5.7 Supervisors report. 6..REFERENCES: 6.1 Ministry of Health Policy and Procedures Manual.
