1. PURPOSE :

1.1 To ensure all patients are fully informed and prepared for their surgery and post - operative
recovery.

2. DEFINITION : 2.1 Nursing Assessment is the gathering of information about patients Physiological, Psycholo gical, Sociological, and Spiritual status of selected patients for admission to hospital for a planned surgical procedure, returning home safely on the same day.

3.

POLICY:

3.1 Patient must be fully informed, so will be less stressed and recover more quickly.

3.2 A nurse will perform an admission assessment and provide pre - operative instructions.
4. PROCEDURES: 4.1 Take nursing history to establish a rapport with the patient and family. Elements of the History include : 4.1.1 Overall Health Status. 4.1.2 Course of present illness including symptoms, current management of illness, medical history. 4.2 Psychological and social examination, assessment of cognitive function. 4.3 Physical Examination - the observation or measurement of signs which can be observed or measured; symptoms such as nausea or vertigo.The techniques may include palpation, aus cultation and percussion. In addition to vital signs further examination of the body systems such as cardiovascular or musculo - skeletal system. 4.4 Documentation of the assessment.
