ANNEX A DOCUMENT CHECKLIST: (PLEASE ATTACH THE DOCUMENTS IN THE ORDER MENTIONED BELOW)

Admission letter to PhD (Also provide Copy of semester results if any) The university must be fulfilling HEC Indigenous program & Quality Assurance Criteria to run a PhD program, instruction available at HEC website. Exact date of joining of classes duly verified by the competent authority Willingness/Consent of HEC Approved Supervisor with contact details i.e. email, phone, fax, office address, university. (Supervisor must fulfill HEC prescribed limit for supervision of scholars) Department and Supervisor Profile as per HEC required format available at HEC website with details of students enrolled with supervisors. Supervisor must be from the parent university.

Deed of Agreement/Surety Bond/ Guarantee to HEC on Stamp Paper as per specimen at HEC website (Annex-II must be dully attested by 1st Class Magistrate and each leaf of the legal documents must be attested by Notary Public; if NOT, it will be considered invalid) Attested copies of CNIC must be accompanied of all witnesses guarantor/surety Person with requisite information available on specimen. Legal papers must be properly registered with correct date from backside. Total value of stamp papers must be in accordance as below;
