KATYS DOG PLACE

ADOPTION AGREEMENT As per our signature(s) below, we understand and agree to the following conditions and restrictions in relation to our adoption of _________________________________ through Katys Dog Place: 1.) We will provide a loving, safe, healthy environment for this dog and will accept this dog as a member of our family. We will provide proper food, fresh water, and medical attention, including all vaccinations and tests to promote and maintain good health. We will provide this dog with adequate exercise and proper shelter at all times, and will not tie or chain this dog for long periods of time and will not allow it to run loose unsupervised. 2.) We agree to comply with all state and local laws and ordinances relating to the keeping of this dog including licensing laws. 3.) We recognize that although Katys Dog Place has made every possible effort to provide a dog which is compatible with the home environment, Katys Dog Place does not warrant/guarantee the temperament or behavior of the dog. We release Katys Dog Place and its members from any liability that may result in acquisition of the dog. 4.) This dog is presumed to be in healthy condition. Katys Dog Place will not be responsible for any conditions for which clinical symptoms are not in evidence at the time of adoption. 5.) We will immediately notify Katys Dog Place if we are unable, for any reason, to keep this dog. We will not relinquish this dog to another party without the knowledge of and agreement of Katys Dog Place and Katys may require the execution of a new adoption agreement with the prospective new owner. 6.) We will at no time allow this dog to ride unsecured in the open bed of a pickup truck. 7.) We will ensure that the dog wear a collar with identification tags at all times, or be tattooed with a registered tattoo or have a registered microchip implanted. 8.) Katys Dog Place is authorized to remove this dog from my home if it is judged not to be receiving proper care or if there has been a violation to this agreement. We agree to permit Katys Dog Place to visit the premises where the dog is kept given reasonable notice and agree that such an entry will not constitute a trespass on the premises occupied by me. 9.) We will immediately notify Katys Dog Place if this dog is lost, stolen or has died. 10) We agree to contribute the amount of $ 40.00 as a donation to Katys Dog Place for the adoption of this dog, thereby enabling the fund to continue to support the program for the rescue of other dogs. _____________________________ Adopter _____________________________ Katys Dog Place _________________________ Date _________________________ Date
