Who May Avail of the Service? Students with special need specifically those who are: hearing impaired (Any degree of Hearing Loss) identified as Fast Learners/Mentally Gifted academically challenged students (non-numerates/non-readers) Fees: Free of Charge

Schedule of Availability of Service: Monday-Friday 8:00AM-5:00PM

HEARING IMPAIRMENT PROGRAM


Requirements for the Applicants with Hearing Impairment (Incoming Grade 7):
Photocopy of Birth Certificate Form 138-A Card/ Form-137 Good Moral Character Referral from a Physician/Elementary Principal/SPED Teacher Photocopy of Medical History diagnosed that the applicant has hearing disability PWD ID Card (if available)

Procedure for the Hearing Impairment Program:

Requirements for the Applicants under Fast Learner Program (Incoming Grade 7): a. A graduate of any government/private recognized elementary school; b. Not over 13 years of age at the opening of the School Year 2013 - 2014; c. Must have a general average of at least 85% in Math, Science and English; and a general average of 83% in other subjects. No grade lower than 80% in any grading periods. d. Should be included from the top 20% of the class of big elementary schools and top 10% of small elementary schools. e. Should undergo the screening procedure conducted by the school committees headed by the Principal/OIC, Guidance Counselor, SPED Teachers and Key teachers. Preliminary documents to submit: Photocopy of Birth Certificate Form 138-A Card Principals certification that the applicant belongs to the upper 20% of the class of big elementary schools and 10% of the class of small elementary schools. Good Moral Character Medical certificate showing that the applicant is in good health and fit

Procedure for the Selection of Fast Learners:


Applicant/Clientele PART 1: Initial Screening (Selection of Top 60 Pre-Qualifiers) 1. Submit the following requirements for the initial assessment: - Form 138-A Card - Photocopy of Birth Certificate - Good Moral Character -Certification/recommendation from the Principal/Guidance Counselor that the applicant belongs from top 20% of the class of big elementary schools or top 10% of the class of small elementary schools. 2. Take the Entrance/Classification Exam for Grade 7. Service Provider Duration of Activity Persons-In-Charge

Receive all the documents for the initial consolidation and assessment

(March 1-April 14) 10 minutes

Guidance Counselor/ SPED Teachers

The date of Entrance exam is scheduled. Consolidate, evaluate and process the test results

April 15-17, 2013 (2 hours) 2-3 working days (after taking the entrance exam) April 19, 2013

3. Get the results.

Announce Top 60 Qualifiers (To be posted on the Guidance Bulletin Board)

PART 2: Final Screening (Selection of Top 40 Qualifiers) 4. Secure, accomplish and submit the final requirements: a. Multiple Intelligence Index form (to be accomplished by the applicant) b. Multi-factored Rating Scale form (to be accomplished by the elementary adviser) c. Teacher Observation Checklist form (Pay P10 as fee for the photocopies of all needed forms.)

Provide all the needed forms for the applicants to accomplish. Receive P10 as fee for the photocopies of all forms

April 19-26, 2013 (Applicant is given one week to accomplish all the needed requirements)

5. Undertake the following tests: a. Standardized Tests in English , Science, and Math b. Mental Ability Test /Creativity Test c. Reading Comprehension 6. Undergo Interview

Provide two conducive rooms as the venue for examination (Schedule of Exam is posted on the Bulletin board)

7. Get the Results

Consolidating, Evaluating and Processing of test results and forms (Verification) Announcement of Top 40 Qualifiers (To be posted on the Guidance Bulletin Board)

Note: Those students who graduated with honors (Valedictorian, Salutatorian, Honorable mention) in elementary will only accomplish STEPS 4 and 5. Please coordinate with Mr. Edgar R.
