Commencement Rehearsal-- MANDATORY (Moran Theatre) 8:15 - 11:30 a.m.* Meet at DA bus-loading zone by 8:15 a.m., *seniors must all ride bus Commencement, Moran Theatre ..........................................7 p.m.* *seniors must arrive by 6:15 p.m. and enter at Terry Lobby *See Graduation Information sheet for instructions and arrival times for gradua tes and guests Parents/Seniors are responsible for transportation to and from Moran Theatre for commencement. Parents/Seniors are responsible for transportation to and from school on days of special activities. Any senior on campus must either be 1) in his/her regularly scheduled classes OR 2) in a senior activity. Attendance at Baccalaureate rehearsal is mandatory for graduates choosing to participate in Baccalaureate. Attendance at BOTH COMMENCEMENT REHEARSALS IS MANDATORY

participate in the senior activities listed on the 2013 End-Of-Year Schedule and to be dismissed from school after completion of these activities.
As the parent/legal guardian of the above-named student, my signature below serves as confirmation that I have read and understand the enclosed information about and instructions for all senior activities, including rehearsals and ceremonies. I do hereby release and discharge the Duval County School Board and its officers, employees, and agents from all liability and damages of any nature whatsoever arising from or in connection with said participation in senior activities associated with the graduation of the Douglas Anderson School of the Arts Class of 2013, including but not limited to transportation to and from the Moran Theatre, Times-Union Center for the Arts for Commencement. Further, my signature below serves as my agreement to release and discharge the Duval County School Board, its officers, agents, and employees, exercising reasonable care within their scope of employment, from liability growing out of personal injuries and/or property damage resulting from or occurring after my son/daughter/ward is released from school or in transit from school following the early dismissal. I further understand and agree that the provisions governing student behavior in the Code of Student Conduct apply to each student who is participating in any school activity on or off school property. Any offense committed while participating in the activities described above can result in suspension of the student, thereby prohibiting his/her participation in Commencement Exercises. School Dress Code will be in effect! As the parent/legal guardian of the above-named student, I give my permission for him/her to not attend school from May 22 through June 3, except for those elective and/or required activities for graduating seniors. I understand that if he/she needs to or chooses to attend classes on these days, he/she must attend all regularly scheduled classes, and traditional attendance and readmission rules will apply. I further understand that if he/she is NOT in classes, he/she must have THIS SIGNED PERMISSION FORM ON FILE IN THE ACTIVITIES OFFICE BY MAY 20. Failure to comply with this policy is an infraction of the Student Code of Conduct, and the student can be suspended, thereby forfeiting his/her participation in Commencement on June 3. When the senior class meets with Mrs. Cornelius April 26, your senior will receive Mrs. Cornelius Agreement Letter To Seniors that must be signed by the student and the parent. That agreement letter and this Parental Agreement and Authorization for Early Dismissal MUST BE turned in by May 20. The senior fee must have also have been paid in order to attend the senior activities.
