Langley AFB

1. Establish the relationship among the Base, the Wing(s), and NORAD, prior to 9/11
concerning the Air Defense mission and the Air Superiority Aircraft (ASA).
- How many wings perform the mission
- Local relationship between military and FAA
- Who maintains the flight strips, for example

2. Establish the scramble procedures prior to 9/11 as executed on 9/11.

3. Establish the communications paths in place and available on 9/11.

4. Establish the chain-of-command on 9/11 and the reporting responsibilities to-from


FAA and to NORAD/NEADS

5. Obtain the AD/ASA scramble history prior to 9/11.

6. Understand the chain of events as they occurred on 9/11.


- The alert at 9:09
-The scramble order at 9:24
- The egress route followed
- The significance of the defined point "Langley 090"
- The controller "chain-of-custody" for Quit 25, 26, and 27 from 0909 through the
establishment of a combined CAP over Washington DC involving planes from
Andrews AFB and perhaps other locations
- SOP for afterburner flight or other governing procedures for ASA speed
- The rebirth of AA11

7. Determine what after action, "hot wash" or other reviews were conducted and where
are those records maintained?

8. Determine who other than the three pilots was interviewed and where are those
records maintained?

9. Determine what other assets might have been available that day—Richmond, Atlantic
City, other

10. Ask about exercises, training, simulations concerning hijackings.

11. Ask about exercises Vigilant Guardian and Global Guardian

12. Ask about lessons learned and recommendations


SAR

1. Establish the USAF SAR/RCC relationship with the FAA prior to 9/11

2. Establish the communications paths, procedures and chain-of-command in place on


9/11, including FAA, NORAD, NMCC and PEOC

3. Establish the universe of SAR/RCC actions on the morning of 9/11 in response to:
- The ELTs from the NYC area

~ second
-AA11
-UA175
-AA77
-UA93
- Gopher 06 (report of it loss)
-Delta 1989

4. Establish the ELT mission and the assets available on the eastern seaboard to respond
to that or any other SAR request/requirement on 9/11

5. What awareness did SAR have about the rebirth of AA11

6. Understand the SAR "intercept and escort" mission

7. Ask about the history of SAR response to aircraft in distress, e. g. Payne Stewart

8. Ask about any exercises or training concerning aircraft in distress, in particular any
such activity that included FAA.

9. Ask about after action reviews or hot washes and about the universe of persons
interviewed

10. Ask about recommendations, lessons learned


GIANT KILLER

1. Establish who GIANT KILLER is and its pre-9/11 role with the FAA and NEADS

2. Establish the communications paths and chain-of-command on 9/11 that linked


GIANT KILLER, NEADS and FAA

3. Establish the emergency procedures that were in place on 9/11 and whether those
procedures included hijacking

4. Establish GIANT KILLER involvement in the scrambles


- Otis, PANTA 45 and 46
- Langley, QUIT 25, 26, and 27
- Andrews, WILD and BULLY
- Richmond
- Atlantic City

5. Ask what other military aircraft were available that day that might have performed the
Gopher 06 role

6. Establish the GIANT KILLER knowledge of and hunt for the reborn AA11

7. Ask about the history of exercises and training concerning hijacked aircraft, to include
the circa 1996/97 live exercise involving a flight to Gander and involving the FAA

8. Ask about the military/civilian relationship concerning control of airspace,


communication concerning emergencies, and reporting procedures

9. Establish the universe of GIANT KILLER personnel interviewed, if any, and the
existence of any after action or other reviews of the events of 9/11

10. Ask about lessons learned and recommendations
