Enclose appropriate registration fee for each week of participation. We reserve the right to dismiss any camper whose conduct is detrimental to the overall good of the camp or campers.
MEDICAL RELEASE
In consideration of the youth named herein and the opportunity for that youth to voluntarily participate in the Cougar Volleyball Camp, I hereby release and discharge the University of Saint Francis, Fabiana Souza and the rest of the staff, of any injuries or illnesses which may result because of participation in this camp. I also hereby authorize any attending physician to render any and all medical care which said physician deems necessary.

Cougar Volleyball Camp is held on campus, in the Air Conditioned Hutzell Athletic Center at the corner of Leesburg Road and Spring Street in Fort Wayne.

Fabiana Shields served as assistant coach at USF during the 2008 volleyball season and returns to Fort Wayne after spending the last two years as an assistant coach at Ashland University in Ashland, Ohio, an NCAA Division II program. She also served two years as an assistant coach at IPFW, where she earned both a Bachelors and a Masters degree in Communications. A starter from her first match at IPFW, Shields earned her teams Most Valuable Player honors each year. She was also named IPFWs Female Athlete of the Year in 2003, 2004 and 2005. Shields currently holds the career kill record with 2,305 kills in her four-year career at IPFW. In 2004, she set a new, single-season record for kills with 621. The previous record stood for 20 years before Shields set a new record.
Position Camps
June 25- June 28 (6:00 9:00 pm) 9th 12th grade
A camp designed for high school players with the goal of providing in- depth skill training for specific positions (setter, libero, outside hitter, or middle-hitter). Campers will learn both offensive and defensive skills through stations and various contests. $30 per day/position
