1. The field in Monitoring Division reviewed, processed, evaluated and endorsed the following documents to the Cooperative Development Authority (CDA). 2. Reviewed the documents of the following Cooperatives: - New San Isidro Consumers Coop LEYECO II Employees Credit Coop (LECCO) Tacloban BSRP Consumers Cooperative

3. Reviewed the coop amendments of the following cooperatives: Kibbal Consumers Cooperative Sto. Nio Sped Center Teachers MPC TWIN MPC Knights Fraternal MPC 4. Issued clearances of 16 cooperatives as supporting documents for the application of a Business Permit

F4 Fund and COOPFUND Program


1. Reviewed the loan applications of 5 clusters or 20 individual borrowers for the F4 Fund and one cooperative for the Coopfund. 2.Conducted on site inspection and credit investigation. 3.Facilitated the processing of loan applications and their supporting documents. 4. Prepared assessments for loan payments. 5. Recorded and evaluated loan payments. 6. Conducted monitoring of loan borrowers. 7. Indorsed to the City Legal Office loan delinquents.

1. To conduct more Coop Orientations and PreMembership Education Seminars to organizations interested in organizing coops 2. To conduct more required trainings of the Cooperative Development Authority for cooperative leaders 3. To conduct livelihood trainings to SPES in coordination with the City Agriculturist Office 4. To facilitate the conduct of the Coop Summer Sportsfest 2012 5. To finalize the Livelihood Agad Program that will provide livelihood to micro entrepreneurs.

5. To coordinate with the TCCDC in conducting Community Development Projects 6. To work on the LGU Tacloban- City Cooperatives Office Accreditation as Training Provider 7. To facilitate the conduct of the Convergence of Millionaire Cooperatives in Region 8
