Please confirm if placing of concrete barrier is to proceed if there is alteration in design to stitch the centre median. Please elaborate in detail on the end treatment of concrete barrier as safety measures for the main carriageway and the slip road (ie. provision of impact attenuator) Please state that an excavator with rubber tracks will be used to protect the road surface. Please include safety officer/safety supervisor in the organization chart and ensure that they involve in this operation.

Please specify how the updated certificates would be submitted before commencement of work No certificates attached with this method statement for personnel involved in this operation. Please provide relevant certificates for excavator operator, signaler and rigger.

6L. Appendix 2, Temporary Traffic Control Plan

Please include and indicate position of VMS in the TCP Please provide drawings and make provisions for cones to be placed along chevron along side the slip road (ie. line of cones placed along slip road chevron that merges with cones from the main carriageway). Please be more specific regarding Risk Assessment and Hazard Analysis ie. Excavator to lift concrete barriers, there are such hazards as lift gear broken, worker finger crushed, worker hit etc. Please take in to consideration inclement weather conditions when assessing the possible risks that may occur during the works ie. What is the procedure to take when the laying of concrete barriers is interrupted by heavy rainfall and how it affects visibility and increases the risk of vehicle collision. Please clarify the difference between Risk Assessment and Hazard Analysis and investigate the possibility of combining the two documents in to one tabular format, where common sections could be matched up or share columns. The attachment is not filled out by the Contractor. It must be duly filled and submitted together with the method statement.
