What this booklet is about


Most people will experience mental health problems at some point in their life. Hartlepools Beautiful Minds is a partnership project comprised of a range of organisations working across Hartlepool that deliver mental health services. The project is dedicated to promoting good mental health. This booklet is designed to give residents of Hartlepool knowledge on mental health services that can offer support and advice as well as ways to self help.

Healthwatch
Healthwatch is the new independent consumer champion for both health and social care. It has two distinct forms Local Healthwatch at local level, and Healthwatch England at national level. The aim of local Healthwatch will be to give citizens and communities a stronger voice to influence and challenge how health and social care services are provided within their locality. Local Healthwatch will also provide or signpost people to information to help them make choices about health and care services.

Local Healthwatch will also:


Have a seat on the local Health and Wellbeing Board ensuring that the views and experiences of patients, carers and other service users are taken in to account within key local strategies such as the Joint Strategic Needs Assessment (JSNA). Enable people to share their views and concerns about local health and social care services and help to build a picture of where services are doing well and where they can be improved. Be able alert Healthwatch England or Care Quality Commission (CQC) where appropriate, to concerns about specific care providers and other health and social care issues. Provide people with information about their choices and what to do when things go wrong Give authoritative evidence based feedback to organisations responsible for commissioning or delivering local health and social care services. Help and support Clinical Commissioning Groups (CCGs) to make sure that local services really are designed to meet citizens needs. Be inclusive and reflect the diversity of the community it serves.

Hartlepool Crisis Resolution and Intensive Home Treatment Team

Who the Service is for


The service is for adults aged 18 years + presenting with an acute mental health need that may require admission to a mental health unit without the potential involvement of an intensive home treatment team.

Being referred
The Crisis Team can be accessed by the following: Substance Misuse Services. Primary Care Health Professionals including GPs Accident and Emergency Departments/Acute Hospitals Social Services/Emergency Duty Team Police/Forensic Medical Examiner working in custody suites Improved Access to Psychological Therapies (IAPT) Non Statutory Agencies Self Referrals Those individuals who are currently open to secondary mental health services and their respective families and/or carers can contact the team during out of hours for additional support or advice.

Description of Service The service provides assessment and intensive home treatment in a range of settings and offers an alternative to inpatient care. The service also responds to service users in a mental health crisis with deteriorating mental health that requires a change in management and increased support. The Crisis Resolution and Intensive Home Treatment Team is multi disciplinary. It comprises medical, nursing, social care, occupational therapy, support and administration staff. These are available 24 hours a day every day.

Who the Service is for


DISC is a service appropriate for individuals over 18 years old whose drug and/or alcohol use is problematic, living in the Hartlepool area and who wish to make positive lifestyle changes.

Being referred
DISC has an open referral system and can accept referrals via; telephone, fax and in person

Description of Service
DISC provides what is known as wrap-around support to adults with substance misuse issues. The project supports those people who want to stop, reduce or control and recover from their drug or alcohol use. We help people with; psycho-social interventions, community reintegration, prescribed treatment and detoxification, signposting for further treatment around a range of social issues, access to residential rehabilitation, peer education and support programmes, counselling services and structured programmes with group work and one to one sessions. Service users are at the heart of our work within DISC, and we adopt a client centred attitude. Our workers are innovative and proactive in their approach and working styles. The project works in partnership with other key professionals under one roof within a multi-agency setting.

Who the Service is for


Adults 18+ with a diagnosed mental health need who are a resident of Hartlepool.

Being referred
We accept referrals from a variety of agencies. This includes statutory services, GPs and other agencies that work with adults who have diagnosed mental health needs. For more information or to request a referral form please contact Choices for Life.

Description of Service
Choices for Life is an innovative day service for adults with mental health needs that puts participants at the heart of their decisions. As part of Creative Support, a nationwide not for profit provider of personalised services we encourage and support project members to access a broad range of in house sessions, one-to-one support and community based activities and events. We work in partnership with a range of services across Hartlepool. At the centre of our service is our social enterprise, Creative Caf, a community hub open to the general public that sells delicious home cooked food using locally sourced ingredients. The Caf also has an exciting selling and exhibition space and offers voluntary opportunities allowing individuals to get work experience, learn new skills and gain qualifications.

Who the Service is for


Adults with mental health needs in receipt of direct payments or other forms of self-directed support funding.

Description of Service
Creative Support provides bespoke packages of support to people with Direct payments and other forms of self-directed support funding. We can support you to: Get out and about on a one-to-one basis, Budget effectively, Take part in group or one-to-one day or evening activities and trips, Access benefits, Pay utility bills, Go shopping, Attend hospital or other important appointments, Access appropriate housing, Develop coping strategies, Achieve any other identified in your support plan. We also provide traditional domiciliary care services to people aged 18 or over. This includes help in the home with tasks that you might find difficult, such as preparing meals, administering medication and other aspects of personal care. All of our services are fully compliant in all CQC essential standards. We offer access to advice 24 hours a day 365 days a year via our out of hours on call service and operate a safeguarding report centre with a senior practitioner who is on duty 24 hours a day, 365 days a year.

Who is the Service For


Hartlepool carers support and information to carers in their role. A generally accepted definition of the term Carer is as follows: A carer spends a significant proportion of their life providing unpaid support to family, friends or neighbours. This could be caring for a relative, partner or friend who is ill, frail, disabled or has mental health or substance misuse problems.

Being referred
Carers can contact Hartlepool carers to self refer. Professionals and carers can refer using the referral on Hartlepool carers website.

Description of Service
Hartlepool Carers can offer support and information on issues connected with your caring role:

Information and advice on local services, benefits, financial and housing problems, employment, education and training Counselling and emotional support Health and social care Complementary therapies, including massage, reiki, reflexology One to one support Volunteer help with dog walking, gardening, DIY, sitting services, shopping etc. Group social activities Advocacy and representation A dedicated young carers team also offering a 'Think Family' support service.

Who the Service is for


The service is for people who may require secondary mental health services such as treatment from the affective or psychosis teams or specialist services such as autism assessments. Following referral from your GP, the service will assess you and identify the most suitable service to meet your needs.

Being referred
To be referred for assessment by the service, a referral from your GP is required. Following referral, an assessment will usually be arranged to take place within 2 weeks.

Description of Service The service provides assessments and short-term treatment of up to six weeks for people with mental health needs that cannot be met within primary care. The aim of these assessments is to determine whether you would benefit from secondary mental health services and to signpost or refer you to an appropriate service.

Who the Service is for


The In Mind service is appropriate for anyone who is experiencing emotional distress such as anxiety, stress and depression.

Being referred
The In Mind service has a GP referral system established. We also accept self referrals. Organisations can request a referral from and refer their clients to the service.

Description of Service
In Mind: Emotional Wellbeing and Psychological Service. We offer one to one Counselling and Psychological Therapy such as: Cognitive Behavioural Therapy (CBT), Interpersonal Therapy (IPT), Solution Focused Brief Therapy (SFBT), Couples Counselling and Bereavement Counselling. We offer one to one private and confidential counselling to help people overcome difficulties such as anxiety, stress, depression, eating disorders, self harming, and anger management. We provide support and teach new techniques and strategies such as: relaxation skills, sleep management, communication skills, assertiveness skills. We work towards improving self confidence, self esteem and wellbeing. In accordance with NICE guidelines all staff work professionally and collaboratively with each client to develop individual personalised recovery plans. It also provides people with the skills and the practical and emotional support they may require to improve their capacity to overcome problems and difficulties to better manage their daily lives.

MHCO: Tees Talking Therapies

Who the Service is for


The therapies are free to access and available to anyone living in the Teesside aged 16 and above who are not in full time education.

Being referred
From GPs, other health and social care professionals and self referral.

Description of Service
MHCO is a not for profit organisation specialising in the delivery of Primary Care Psychological Therapy Services for nearly ten years. MHCO delivers IAPT and counselling services in the North-East in Darlington, Newcastle and Northumberland as well as in a number of other regions in the UK and has built up a reputation for providing innovative, flexible services achieving excellent clinical outcomes (regularly amongst the top 15% in the UK according to Department of Health figures for IAPT services). MHCOs service in Teesside is now accepting referrals and is listed on Choose and Book as MHCO Talking Therapies.

Who the Service is for


The service is for family members/carers or any person who is affected by the substance misuse (drugs or alcohol) of another person including young people.

Being referred
Service users can self-refer into the service or be referred from any organization they are currently working with e.g. social services

Description of Service
This service offers family support and counselling to those people affected by the substance misuse of another person. This includes family mediation, one to one counselling and couples counselling. Family support includes family activities, educational groups including mental health awareness and drug and alcohol awareness sessions, parent and child sessions as well as trips out. We also run the Community Reinforcement and Family Therapy group (CRAFT) supporting family members to encourage change in the substance user.

Who the Service is for


Younger people who have dementia and live in Stockton, Middlesbrough. Hartlepool or Redcar and Cleveland.

Being referred
Service users can self refer or you can refer a family member or friend if you have concerns.

Description of Service
Assess people with memory problems and provide a diagnosis and support if required. This includes signposting to relevant teams or agencies who can help you.

Description of Service IntraHealth are a dynamic team who are committed to providing specialist prescribing and psycho-social support with regular appointments offered to patients who are encountering issues around substance misuse.

Reablement Practitioner for People with Dementia

Who the Service is for


The service is for people with a dementia or suspected dementia.

Being referred
Referrals will generally come from other workers supporting you, though you can refer yourself if you wish.

Description of Service The service supports people with dementia or suspected dementia to maximise their independence and well-being. The practitioner will work with other professionals, e.g. nurses, therapists and social workers, and offer guidance about memory difficulties and useful strategies.
The practitioner will also support referrals to assess for dementia when needed.

Description of Service The purpose of the Forum is to enable people 50+ in Hartlepool to have a voice on matters which are important to them, whilst providing organisations and agencies with a mechanism to consult with people 50+ on a range of issues that relate to planning and developing services. Members of the 50+ Forum lobby for the rights of older people on a local, regional and national basis. The Forum has elected representatives on many groups and partnerships across Hartlepool and the North East who feed the views of older people into key decision making.

Who the Service is for


The service is commissioned to work with people aged 18+ who are suffering from mild-moderate mental health symptoms such as depression, anxiety, trauma and phobias. Intended as a preventative service, patients must not currently be in secondary mental health care.

Description of Service
Talking Matters is a free talking therapies service (IAPT) providing brief wellbeing interventions in Middlesbrough, Stockton, Hartlepool, Redcar and Cleveland. You will be assessed over the phone at a time convenient to you, and from the information you give us, we have a range of ways to help: Guided self-help Group psycho-education Telephone therapy Face to face therapy All are designed to give you new resources to cope with things and help you going forward. We can see you in your local area, and at a time convenient to you.

Who the Service is for


If you are over 16 and live in the Teesside area, i.e. Hartlepool, Stockton-onTees, Middlesbrough or Redcar and Cleveland this service is eligible for you. Sometimes feeling stressed, worried or struggling to cope can be a normal reaction to certain significant life events but when these feelings persist, as they can do for up to 1 in 4 people, you may need to seek help. There are many things that we can do to help you with the difficulties you are experiencing. Our free service is based on the best research evidence, Department of Health guidelines and recommendations from the National Institute for Clinical Excellence (NICE).

Being referred
The easiest and quickest way is by calling us on: 0800 8048155 or 01642 424030. When you contact the service an administrator will ask for some basic details and offer an appointment with a member of the team.

Description of Service
All of our high quality staff are trained in NHS approved therapies and offer a range of therapies to suit individual needs. These include: wellbeing courses where you will learn about the things you can do to overcome your problems self-help materials that you can access online from home supported by one of our practitioners online talking therapies that allow you to see and speak with a therapist, offering more flexibility to fit in with modern day lifestyles specific one-to-one help from a qualified therapist or practitioner who can help you work through a range of problems
