PDF with color codes attached. 8-Mar Thanks. 26-Feb Cell algorithm colors are included on the attached figures.
2/6/2013 and reply to query on Feb 7 2/6/2013 and 11th Feb updated changes received 30-Jan My marked up algorithms are attached 7th feb 21-Feb
I have attached Figure 29.6 with the colors indicated as requested. Figure 29.1 and 29.3 do not include recommendations so all boxes would be blue. / Yes, the box you refer to should also be green. Sorry for omitting that on the document I sent you. 31-Jan It should be green. am in receipt and will respond back in next couple of 31-Jan days...tx 6-Mar On algorithm 47-02 the following color coding should occur per your guidelines: Top TWO Boxes - BLUE Left Sided FOUR Boxes starting with NON-Pharmacologic Treatment and the three below - RED Right Sided FIVE Boxes and the SINGLE box at the very 26-Feb bottom - GREEN. Attached are my suggested colors. They are in the sticky notes. Boxes without a sticky note should be blue. Top TWO Boxes - BLUE Left Sided FOUR Boxes starting with NON-Pharmacologic Treatment and the three below RED Right Sided FIVE Boxes and the SINGLE box at the very 26-Feb bottom - GREEN.
Here you go so sorry again for the delay! Our institution has greatly limited our Adobe PDF capabilities, so I had to 20-Feb use the Sticky Note feature. Hope that works. 31-Jan You can see the comments below I have looked over the algorithm & the examples. Our figure is actually one of the example pdfs that was sent (figure 116-01) and the colors for those cells will be the 30-Jan same for this figure.
Please find the attached figures with the desired colors marked. Again, sorry it took so long to get these back to you. We also found a couple of changes that needed to be made to the figures themselves, beyond the colors. I wasnt sure if this is the appropriate time to be making those sorts of changes, but they have been marked on the 27-Feb figure as well (i.e. one change on figure 88-02).
26-Feb Here are the color coded charts for the UTI chapter. 30-Jan All the cells should be blue We also found a couple of changes that needed to be made to the figures themselves, beyond the colors. I wasnt sure if this is the appropriate time to be making those sorts of changes, but they have been marked on the 27-Feb figures as well (i.e. two changes on figure 100-02).
Attached is the algorithm with colors indicated by sticky 12-Apr notes. I hope thats ok. Here you go. We will have to add dimethyl fumerate to 11-Apr-13 the text. Thank you, Attached is the color coded file./ Both boxes should be 4/16/2013 and 18- Apr-13 GREEN. 11-Apr-13 Please see attached mark ups.
Sorry these are now late. Not sure what happened (I sent them April 13th). Here are the recommended colors. They are indicated by sticky notes. 23-Apr-13 And yes, we hope spring gets here before fall begins! Attached are the identified figures. Please also note that there was one error on 45-3, likely as a result of unclear instructions on my part within the text description. Thank you for correcting this also. 16-Apr-13 23-Apr-13 Attached you will find the color assignments for Fig 48-1. Attached you will find the figures along with the color assignments. Please feel free to contact me if you have 17-Apr-13 any questions. Attached as requested. Please let me know if you have 10-Apr-13 questions.
SO sorry I misunderstood what you were asking!!! I add the color recommendations. The only question I had (and perhaps Christian can weigh in) is for the two boxes that being with the word maintain. I marked these as red because they are a recommendation for how long therapy should be administered but I didnt know if you meant that red should be used for more specific nonmedication 24-Apr-13 treatment recommendations like psychotherapy.
Please find attached the algorithms with the colors marked for the cells for Figures 053-2 and 053-3. For Figure 053-1, the algorithm needs to have the same colors as the original IPAP GAD alorighm (same as 8th edition). The colors are keyed by level of evidence and IPAP wants us to keep the original algorithm intact. So, on that attachment, I instructed to color the cells as in the previous edition and as they originial published 12-Apr-13 algorithm. Is this acceptable? Thank you! Attached is the one figure for Chapter 54 in the DiPiro book. Thanks and please let me know if you have any 17-Apr-13 questions. Please see attached. Let me know if this is not sufficient 18-Apr-13 and you need more information.
Here are the algorithms. I have colored Figure 83-7 7-May The other two I have outlined the boxes in color.
Attached is a pdf indicating which colors should be assigned to the specific boxes. Please let me know if you 16-Apr-13 have difficulty interpreting any of the writing. I copied Dr. Jordan who is a co-author for this new edition. The figure is not complex, so I will just provide the information by e-mail. Please note a few other minor corrections. The boxed information in the figure is structured in six levels starting from the top (Level 1): Level 1, middle column: blue Level 2, left: red Level 2, right: green (change SRL to SA) Level 3, left: green (change SRL to SA) Level 3, middle column: red Level 3, right: green (change increase dose, frequency or combination to increase dose and/or frequency Level 4, middle column: red Level 5, left: red Level 5, right: green 11-Apr-13 Level 6, middle column: red
Here are the 3 figures. I used the highlight function in tools to identify the boxes as red, green or blue per the desired color assignment noted below. 11-Apr-13 13-Apr-13 Please find the information you requested regarding coloration of 3 diagrams for my chapters on Benign Prostatic Hyperplasia and Erectile Dysfunction. Please holler if there are any other questions.
12-Apr-13
I have attached the color notations on the requested 17-Apr-13 algorithms. Thanks. Cindy
Ive attached color suggestions for the algorithms. I have formatting suggestions for the algorithms as well should I provide those comments now or will there be 17-Apr-13 additional processing done before the final versions? am sorry this took so long. I have been traveling and having some problems with my email. 23-Apr-13

That time frame does not fit in with our schedule. Please suggest an earlier or later time frame/ The following week (Mar 9-16th) looks good.
Can you please use my correct email? I have not had the @uchsc.edu email in 6 years. I have provided my correct email many times prior and this was a problem for the last editon. My email is Joseph.saseen@ucdenver.edu Also, I will be on vacation the first 2 weeks of July so I anticipate not being able to comply with your 7 day turn around. Please send eproofs./ Please also send them to Robbie Parker rparker@uthsc.edu

We prefer eProofs. Dr. de Denus is on vacation at that time. Would it be possible for us to receive the proofs either /before July 31 or after August 26th? Sarah/ OK thanks
You can send the page proofs to my home address: 2404 Doral Dr. Blackwood, NJ 08012 E proofing is fine. I will be working in July (no vacation)
25-Apr-13
25-Apr-13
Saloni, Edith Nutescu and Stuart Haines are no longer authors on this chapter and should be removed from all future correspondence. I prefer E-proofing. I have no vacation conflicts currently. Will let you know if that changes. Thanks, 25-Apr-13 23-Jan-13 23-Jan-13 Dan eProof will be fine with me. Timing is fine
Thanks for the advance notice. Either mode is fine with me so please choose the mode that is easiest for you. Please note that I will be out of town the end of June through about July 10th so if for some reason the proof is delayed I will not get to it until I am back.
E-proofing will work for me. I would prefer E-proofing Plan confirmed. I prefer to receive the proofs as hard copy, but defer to Bill as to his preference. Chris I would prefer to receive the proofs electronically. I dont have plans for vacation during this time in August.
We would like it electronically. No July vacation scheduled yet, so should be able to meet the deadline. 25-Apr-13 25-Apr-13 25-Apr-13 29-Jan-13 29-Jan-13 31-May-13 31-May-13 31-May-13 23-Jan-13 I will review the proofs using the E-proofing method. You can send the link to me; only one of us needs to review the proofs. Prefer electronic. I would prefer E-Proofing. I have no vacation plans during that time period.

I would prefer the online method. I prefer online. Hi Saloni. E-proofs will be fine for me. I dont foresee any problems with my being able to comply with the dates you have given me. Thanks so much! Julie I look forward to working with you and should have no problem meeting the deadlines set forth. I would prefer to receive the galleys electronically. Please let me know if there is anything else you need from me.

31-May-13 2-Feb-13 2-Feb-13 23-Jan-13 We are most happy to utilize the E-Proofing process. We have no vacation plans during the period of April 2-9.
23-Jan-13
We would prefer e-proofing. Also, no vacation plans between now and March for me. Dr Wazny and I would prefer to use the e-proofing method. I would also like to request that since she and I will be coordinating our proofreading efforts that we have until early the following week to complete the review (March 26 if possible). We do not anticipate being out during the time the proofs are to be made available. Let us know if you need any additional information. E Proofing would be preferred. Eproofing is fine Thank you for your email. I prefer to receive proofs via email (EProofing). The schedule outlined below fits well into my vacation plans. I will out of the office from June 30-July 7, and again from July 26-August 5.
25-Apr-13 25-Apr-13
25-Apr-13
I would like the online method (e-proofing). As for time scheduled off, I plan on being off from June 22nd to the 30th. Thanks for the email. As it turns out, I am on vacation at a family wedding in San Francisco beginning Wed, July 10 and ending Monday, July 15, which is exactly the time frame mentioned for my proof review. However, I wouldn't mind getting the chapter before I leave because I will likely have some time on the plane flights to review it; however, I can't guarantee that I would get it back by the 17th. If I could get it on the 10th electronically but have until the 22nd to return it, that would work best with my vacation plans. Thanks.
29-Jan-13
In regards to vacation plans, I don't currently have anything scheduled for March, so a quick turn-around to Dr. Matzke should be no problem.
29-Jan-13 23-Jan-13 NA 28-Mar-13 28-Mar-13 28-Mar-13 That schedule should work for me. I will be out of the office from tomorrow through April 8, June 24-28, and July 15-19. 28-Mar-13 30-Mar-13 30-Mar-13 30-Mar-13 My vacation plans during this time are the following: I will be in Key West, FL from 5/10 thru 5/14/2013 On April 3: Thank you. I will let you know if I need 1-2 extra days once I see the comments/changes. Susan Rogers I would prefer to receive the proofs electronically. Hi Saloni: I will opt for the e proofing option and I will be around during this March period NA
30-Mar-13
29-Jan-13 Thanks for letting me know about the chapter review process and timetable. March 15 should work out fine for me to review the chapter.
I will be available,thanks

30-Mar-13 Nice to meet you electronically. I will be out of the country on vacation from May 2 to May 16. On May 17, I fly out for a relatively intense meeting that lasts until the 21. I would ask that if possible the time line be pushed back a week dependent on Christian's schedule.
30-Mar-13 30-Mar-13
30-Mar-13 Thanks for the information. I will not be on vacation during the week of the 14-21. I will likely be covering our consult service myself on the 16th and 17th during our resident retreat. If there is any chance the document is ready by the 13th, I may have some time in the afternoon to start reviewing.
30-Mar-13
30-Mar-13 30-Mar-13 Dear Saloni: Thank you for letting me know the schedule. I have had a change in plans and will be out of the office during this period. Would it be possible for the galley link to be available the first of the month (May)? Please let me know an alternate date.
30-Mar-13
As far as I can determine, I will be available at the times outlined below. Thanks for moving this along. I do not have any plans for leave from May 14th through 21st. Thanks
I think it would be easier for all authors for Diabetes Mellitus to give us access to E-Proofing. Curtis Triplitt 28-Mar-13 E proofs would be good. Please change the email contact you are using. It is not one I usually use. The correct one is below.Thank you.Jacqueline Jonklaas Associate Professor jonklaaj@georgetown.edu PLEASE NOTE NEW EMAIL ADDRESS
29-Mar-13
We would prefer the e-proofing option. However, we all have travel plans during the month of June. If possible, we would request to receive the proof in early or mid-May to allow time for review and re-submission; we are not sure how much time we will have to devote during our time away in June.
Please send the electronic version of the galleys to ALL study coauthors (four individuals). Two hard-copy proofs should be sentone to me at the Silver Spring address and one to co-author, Dr. Joe Jordan (see below).

Hi there. Please note my email has changed it is sshrader2@kumc.edu. We also are at a meeting during the time frame you proposed. Can we move the due date back by at least a week? Thank you. Sarah Shrader

There have been a couple major changes so we will try to minimize the changes but might need to add an addendum. Can we get the chapter a week earlier or later? I am on vacation from 8/7 to 8/18. Since I will be traveling with frail parents and kids, won't be much time to proof a chapter. Mary Beth

31-May-13 31-May-13 I would just like to request they be sent as soon as possible. We are in the middle of an international adoption and are anticipating travel some time in August or September and will be out of the country for 2 weeks. I just want to make sure it doesn't end up the week you send the proofs. Thanks!

I would prefer to get chapter proofs (both chapters 88 and 100) via Eproofing so that my co-authors would also have access to the proofs. I also wanted to let you know that I will be out of town from March 30th through April 7th, so it will be very difficult to get proofs returned within one week if they are sent during that time. Thanks, I would lean towards e-proofing, but Angie I leave the decision to you as lead author. AOK with me. E-proofing is the preferred method for this chapter. I have no vacation plans that would interfere with the schedule.
29-Jan-13
e proofs are preferred There will be 3 of us working from different locations. At the moment, I do not have vacation plans in April.
23-Jan-13
Thank you for your note announcing the review of Pharmacotherapy and requesting our preference for review. I will happy to go with the electronic version (E-Proofing) for the review. In terms of vacation, I not not plan to take any vacation between now and April, 2013. I will complete the review as requested and meet the deadline set. Thank you. Kind Regards. I am currently out of the office with limited access to email and will return Wednesday January 23, 2013. If you need immediate assistance, please contact Gerida Brown at 713-743-3260.
We would prefer to receive our proofs electronically. You have the correct email addresses for Dr. Shehab and myself.

For some reason, I have not received your previous emails about this request until Dr Dager emailed me. Im not sure why that happened because I can see that you sent them to me but they did not make it to my Inbox. If we can have time until Friday to send you the feedback, that would be great. Im very sorry about the delay if you could next time in addition cc the emails to my personal e-mail address (halilovic@gmail.com) , that will hopefully avoid this from happening agagin.
