I further authorize any physician or hospital to release any information which may be necessary to determine my ability to perform the job for which I am being considered or any future job in the event that I am hired.

Not make false statements in any documentation. Be knowledgeable of his/her administrative responsibilities and rights. Respect the provisions of the job contract. As per the rules and regulations of the Management, you cannot leave during the Semester. If you would do so, you would not be entitled for the dues of the entire semester. If you cancel/skip any lecture, you would be bound to hold additional classes to compensate for the loss of time. You can start the classes on the date of appointment if you would agree to the terms and conditions hereby provided.
