The 3D Rica-nator

Description: The 3D Ricanator is built off of bands on the maxillary first permanent molars. There is an anterior bite plane of acrylic that engages the lingual of the anterior teeth and that the lower teeth bite into in a forward bite relationship. The anterior acrylic is connected to the molar bands by arms that engage the lingual of the bicuspids and that insert into special vertical tubes (3D tubes) on the lingual of the molar bands. There are rests on the first bicuspids for stability. The anterior inclined plane, which guides the mandible forward into a Class I molar relationship as the patient closes and is effective in mild Class II Division I cases. The bite can be set to exactly where the doctor wants to finish the case. The position of the lower jaw may be brought a little beyond a Class I molar relationship in cases where the appliance is used in lieu of a removable functional appliance for Class II correction. In cases where vertical development is needed, the acrylic bite plane may be built further open and posterior elastics may be worn from the lower to the upper posterior teeth to aid in posterior vertical development. The 3D Ricanator was developed as an effective alternative to removable functional appliances that bring the mandible forward and encourage vertical development in Class II Division I cases. The appliance can be used as a retainer after functional appliance treatment or in conjunction with straight wire case finishing. With this type of appliance design the doctor has total control of the case since the appliance is fixed to the patient, but, being constructed with 3D lingual tubes, is removable by the doctor for adjustment and cleaning of the appliance. This appliance achieves rapid results because of it's full time wear and treatment time can be greatly reduced when the 3D Ricanator is combined with straight wire appliance therapy. One-week prior to taking impressions, separate the Maxillary 6-year molars. Separators need not always be placed before impressions, but makes it easier for the laboratory to fabricate an accurate appliance. 2. Complete maxillary and mandibular alginate impressions. Obtain a wax bite at normal biting relationship. 3. Take a construction bite at the desired mandibular position. The thickness of the bite material should also determine the anterior and posterior vertical dimension. 4. Pour models with lab stone. 5. Complete an Orthodontic Technologies laboratory prescription form. See Orthopedic Appliance, Type: Ric-a-nator. Note, if buccal tubes are desired indicate that on the lab slip with your arch wire slot preference .018 or .022. 3D tubes will be added to the lingual of the molar bands unless otherwise requested. When the appliance is received: 6. Trial fit the appliance, adjust as needed. 7. Cement the bands in place. 8. Insert the Ric-a-nator and adjust if needed. 9. If retention is an issue, the arms can be ligated to the bands (tie wire under prongs and over occlusal aspect of the tube) or, prongs can be bent to diverge slightly so that when inserted there is some resistance to insertion. The rests may be bonded to the first bicuspids. 10. Schedule a 1-week follow up visit to verify compliance and check the appliance. Afterwards, progress can be monitored in 4-6 week intervals. 11. Note, the Ric-a-nator is ideally suited to be used in conjunction with fixed mechanics (brackets and straight wire). Length of Treatment Time: Depends on the use of the appliance; whether it is used to correct a Class II relationship, as a retainer or while finishing a case with straight wire and brackets.
