DECLARATION: The above Club / Society wishes to apply for affiliation to UCL Union. The final decision is taken by Activities Board. Affiliation is dependent on the following points being fulfilled: 1. The application must meet the criteria set out in the Notes for Applicants that accompanies this application form. 2. The application must be accompanied by a list of 30 full members of UCL Union who support the application (see sheet overleaf). 3. Applicants are required to attend the relevant meeting of Activities Board to discuss the application. 4. Successful applicants must complete an Annual Registration Pack and a Risk Assessment before their Club / Society can be formally established. 5. All Clubs / Societies must respect UCL Unions Constitution and Standing Orders and the Clubs and Societies Procedures and Regulations and Good Practice Guidelines. Signature :
