008 - PRECALCULUS MATHEMATICS (CRN: 70913) The above named individual is a student with a disability who has requested accommodations and assistance from the Office of Accessibility. He/She has provided this office with individualized, confidential documentation from a qualified professional that substantiates the disability. Federal law and University policy require that the University create an environment where students with various disabilities can partake of the programs and services we have to offer in a nondiscriminatory manner. The recommended classroom accommodations for this student are as follows: 1. Alternative Formats Use of textbooks in an alternative format 2. Alternative Testing Time extensions for completing tests and in-class work equal to 75% additional time. Students must notify instructor and make arrangements prior to test date. If the instructor is unable to provide the testing accommodations, the student may be required to contact the Office fo Accessibility a minimum of four days prior to the test date for scheduling. Use of a computer for essay tests and in-class writing assignments, with spell check (provided by the instructor whenever possible or by the Office of Accessibility) Assistance in reading tests. Student must request a reader through the Office of Accessibility at least 4 days prior to test Testing in a distraction reduced space (provided by the instructor whenever possible, or by the Office of Accessibility) 3. Equipment For courses beyond Basic Math I, use of calculator in courses with math content is permitted when the essential elements of the course are not altered. Developmental Math Courses: Calculator is not permitted for Basic Math I due to essential elements of the course. Use of a four-function calculator is permitted for Basic Math II after Test One. Use of tape recorder in the classroom 4. Notetaking Services Note takers - this may include notes taken by another student with the use of carbonless paper provided by the Office of Accessibility (provides copy for student and note taker). Student must attend class in order to utilize note-taking services.

The student has been encouraged to discuss with you the effects of the disability and how these recommended accommodations are helpful. Students may choose not to use all of these accommodations in your class. Each student is ultimately responsible for notifying you of these accommodations in a timely manner. Office of Accessibility policy suggests that students present this document to instructors by the end of the second week of classes in a 15-week session or its equivalent time frame for shorter sessions.
