I am Juan Miguel S. Cruz, a current student of De La Salle University College of Law. Last June 30, I sustained a very serious injury. During a basketball game at Enrique Razon Sports Complex, I broke my left arm. This happened when I was hanging on the basketball ring. I was holding the ring with my two hands, I let go of my right hand, my body twisted, and then I heard a pop sound in my left arm. I knew I immediately that something wrong had happened with my arm. I asked my friend to bring me to the clinic at the ground floor of the sports complex. The nurse attended me and he made a sling for my arm. Minutes later, the ambulance came and brought me to the Manila Adventist Hospital. After preliminary diagnosis and x-ray, the attending physician told me that I, indeed, broke my left arm. I was admitted there and then. The orthopedist told me that due to the severity of my injury, there is only one way to fix my arm and that is through an operation. On July 1, I was brought to the operating room and the orthopedist implanted an aluminum plate in my arm. From then on up until today, I am at my recovery phase. The doctor scheduled a monthly check up to see any improvements to my arm. I was told that De La Salle University would be shouldering 40,000 pesos from my overall due of 180,000 pesos, which my family and I are very grateful about. However, my parents cannot pay the remaining 140,000 pesos in one bulk. That is why we would like to request and make a proposal with regards the payment scheme for the balance: Half down payment of 50,000 pesos on September 2, 2013 The remaining 90,000 pesos in 6-month installment at 15,000 pesos per month : o OCTOBER 7, 2013 o NOVEMBER 7,2013 o DECEMBER 7, 2013 o JANUARY 7, 2013 o FEBRUARY 7, 2013 o MARCH 7,2013

We made this proposed payment scheme not to prolong the term of payment without any valid reason. I am a full time student and my parents still supports me. Had it been I am working, I could have at least make contribution to the remaining balance. I also have three younger siblings, all in secondary school. This payment scheme is the most convenient manner in which my parents could still manage the household budget and at the same time pay whats left on my hospital fees. As an assurance, I would like to state that I am only in my 2nd year as a full time student at De La Salle University College of Law, which means that I will be staying for 3 more years in this university. I am hoping for your greatest consideration.
