Generic Name: Hydrocortisone

Indication: Severe inflammation Dosage: 100 mg

Brand Name: Cortef, Cortenema, Hydrocortone

Route: IV Frequency: Q8

Chemical Effect: Not clearly define; may stabilize leukocyte lysosomal membranes, suppress immune response, stimulate bone marrow, and influence nutrient metabolism. Therapeutic Effect: Reduces inflammation, suppresses immune function, and raises adrenocorticoid hormonal levels.

ADVERSE REACTIONS: CNS: euphoria, insomnia, pseudotumor cerebri, phsycotic behavior, seizures. CV: arrhythmias, edema, heart failure, hypertension, thromboembolism. EENT: cataracts, glaucoma. GI: GI irritation, increased appetite, pancreatitis, peptic ulcer. Metabolic: carbohydrate intolerance, hyperglycemia, hypokalemia.

Monitor vital signs and electrolyte levels. Monitor patient for stress. Fever, trauma, surgery and emotional problems may increase adrenal insufficiency. Be alert for adverse reactions and drug interaction

Musculoskeletal: Growth suppression in children, muscle sickness, osteoporosis. Skin: acne, delayed wound healing, easy bruishing, hirsutism, various skin eruptions. Other: acute adrenal insufficiency with increased stress (infection, surgery, or trauma) or abrupt withdrawal after long-term therapy, susceptibility to infections.

Indication: Generic Name: Pantoprazole sodium Brand Name: Pantoloc, Protonix IV, Protonix Short-term therapy for erosive esophagitis related to gastroesophageal reflux disease (GERD). Dosage: 40 mg Route: PO Frequency: OD

Chemical Effect: Inhibits the activity of the proton pump by binding to hydrogen-potassium adenosine triphosphatase, located at secretory surface of the gastric parietal cells. Therapeutic Effect: Suppresses gastric acid secretion.

CNS: anxiety, asthenia, dizziness, headache, insomnia, migraine, pain. CV: chest pain. Resp: bronchitis, dyspnea, increased cough, upper respiratory tract infection. GI: abdominal [ain, constipation, diarrhea, dyspepsia, eructation, flatulence, gastrointestinal disorder, nausea and vomiting GU: rectal disorder, urinary frequency, UTI. Skin: rash. Other: flulike syndrome, infection.

Assess underlying condition before starting therapy and regularly thereafter to monitor drugs effectiveness. Assess patient for complaints of epigastric or abdominal pain and for bleeding (such as blood in stool or emesis). Be alert for adverse reactions and interactions.

Generic Name: Azithromycin Brand Name: Zithromax, Zmax

Indication: Acute bacterial exacerbations of chronic obstructive pulmonary disease Dosage: 500 mg Route: PO

Chemical Effect: Exertsh1-receptor antagonist activity. Therapeutic Effect: Relieves seasonal allergic rhinitis and conjunctivitis.

Frequency: OD

ADVERSE REACTIONS CNS: dizziness, dysesthesia, fatigue, headaches, somnolence. EENT: conjunctivitis, epistaxis, eye pain, paroxysmal sneezing, pharyngitis, rhinitis, sinusitis, stinging, transient eye burning, temporary eye blurring. GI: bitter taste, dry mouth, nausea. Metabolic: weight increase. Resp: asthma, dyspnea. Skin: pruritus. Other: flulike symptoms.

Obtain history of patients allergy condition before therapy begins, and reassess regularly thereafter. Be alert for adverse reactions and drug interactions.

Generic Name: Montelukast sodium Brand Name: Singulair

Indication Asthma Dosage: 10 mg Route: PO Frequency: OD HS

Chemical Effect: Inhibit action of cysteinyl leukotriene1 (CysLT) receptor by binding with CysLT1. This reduces early-and late-phase bronchoconstriction caused by antigen challenge. Therapeutic Effect: Improves breathing.

ADVERSE REACTIONS:

Supervise ambulation.

Promethazine sometimes produces marked sedation and dizziness. Be aware that antiemetic action may mask symptoms of unrecognized disease and signs of drug overdosage as well as dizziness, vertigo, or tinnitus associated with toxic doses of aspirin or other ototoxic drugs. Patients in pain may develop involuntary (athetoid) movements of upper extremities following parenteral administration. These symptoms usually disappear after pain is controlled. Monitor respiratory function in patients with respiratory problems, particularly children. Drug may suppress cough reflex and cause thickening of bronchial secretions. For motion sickness: Take initial dose 3060 min before anticipated travel and repeat at 812 h intervals if necessary. For duration of journey, repeat dose on arising and again at evening meal.

Do not drive or engage in other potentially hazardous activities requiring mental alertness and normal reaction time until response to drug is known. Avoid sunlamps or prolonged exposure to sunlight. Use sunscreen lotion during initial drug therapy. Do not take OTC medications without physician's approval. Avoid alcohol and other CNS

depressants. Do not breast feed while taking this drug.
