Short term goal: Independent: Psychomotor: After10 minutes of nursing care, client will be able to demonstrate techniques in resuming normal activities of daily living. Affective: After 10 minutes of nursing care, client can cope up in limiting activities during post surgical procedure Cognitive: After 10 min. of nursing care client will be able to understand the importance of increase activity tolerance. Determine baseline vital signs Note responses to activity Provide adequate rest for the patient Assist patient in self-care activities Encourage to change positions slowly Adjust activities or reduce intensity level or discontinue activities that cause undesired physiological changes. Increase exercise levels gradually Promote comfort measures and provide relief of pain Collaborative: To track changes in the patients condition Assess degree of debilitation For maximum comfort To provide assistance on the clients health needs To reduce risk of decreased in resuming activities To prevent overexertion
After 30 minutes of nursing care, patient has able to understand the proper techniques in coping to activity intolerance such as changing of positions, relaxation techniques and responses to activity.
