PETITION
COMES NOW Petitioner, by counsel and unto this Honorable Court most respectfully states: 1. Petitioner is a minor, a Filipino citizen, duly represented by his father Jose A. Blanco Rios, a resident of 20 th Street, City Subd., San Pablo City, Laguna; while, the respondent Local Civil Registrar of San Pablo City and the National Statistics Office (NSO) are both government agencies and with address at San Pablo City and East Avenue Diliman, Quezon City, respectively where they may be served with the processes of this Honorable Court and said respondents are impleaded herein as necessary parties pursuant to the Rules of Court; 2. Petitioner was born on October 5, 2001, at San Pablo City and his parents are Jose A. Blanco Rios and Amada E. Burgos, however, his date of birth was registered as October 3, 2001 instead

2 of October 3, 2001, photocopy of his Certificate of Live Birth is hereto attached as Annex "A", and made part hereof; 3. Petitioner was actually using October 5, 2001, as his true and correct date of birth, photocopy of petitioner's school records is hereto attached as Annex "B"; 4. That petitioner desires to correct the erroneous entry in his Certificate of Live Birth, as to his date of birth from "October 3, 2001" to "October 5, 2001", to straighten his official records and to avoid future confusion;

PRAYER
WHEREFORE, its is respectfully prayed of this Honorable Court that after due publication and hearing, an order be issued for the correction of petitioner's Certificate of Live Birth as to his date of birth from "OCTOBER 3, 2001" TO "OCTOBER 5, 2001".
