Passage Planning Risk Assessment

Risk Assessment number ______________________ For Passage from: ________________________________ to: ________________________________ On Voyage from : _______________________________to: ________________________________ Date: _________________________________ Assessed by: ________________________________

Hazard: Failure of GPS when entering the Delta Bay Risk: Unable to use ECDIS due to unavailability of GPS position vessel running aground Risk Assessment: Severity of Hazard
5 4 3 2 1 Very High High Moderate Slight Nil

Likelihood of Harm
5 4 3 2 1 Very likely Likely Quite Possible Possible Unlikely

Risk Level
Formula to assess risk level Severity x Likelihood

5 x 3 = 15

Control Measures
Do not use GPS as the primary source of position fixing. Ships autopilot not on track control. (must be in manual steering) Both radars switch on one solely for position fixing. Parallel indexing in use with reference to Balfe Point. Calculate set and rate and apply as required. Position fixing interval to reduce to 10 minutes. Engine on Standby for immediate maneuvering.

Monitoring required to ensure that the control measures are maintain Efforts to reduce risk, required with attention to allocation of resources and amount of time required for reducing risk Ship cannot proceed on passage without reducing the risk. Allocation of resources and time can increase to very high amount but ship may proceed on voyage once risk has been reduced. Passage cannot continue even with unlimited resources
