    This document contains assessment forms for short and long medical cases. The short case form evaluates candidates on clinical examination skills, discussion of findings and diagnosis, and justification of investigations and management. Examiners rate candidates in each area as excellent, good, adequate, inadequate or poor. The long case form similarly evaluates candidates on interviewing skills, clinical examination, case presentation involving findings, interpretation and diagnosis, and discussion of investigations, management, prognosis and recent advances. Examiners provide scores in each area and can include additional remarks. Both forms require the examiner's signature and are used to assess medical candidates during practical examinations.

    Please enter your award on the performance against each item according to the rating scale. Total time allocated for all short cases is 40 minutes (including both examination and discussion).

    Each examiner to complete his/her own form without conferring with fellow examiner. All awards must be made in ink. Each examiner must rate all items of the form. Use separate form for each short case.
