1 Purpose This procedure has been written to ensure, all Tilt-up and Precast Construction by EMCO complies with Australian Standard AS 3850 Tilt-up concrete construction and the Code of Practice Tilt-up and Precast Concrete Construction and as far as is practicable an environmentally sound and safe system of work for tilt-up concrete construction activities. The following legislation is applicable to all construction activities: Occupational Safety and Health Act 1984 Occupational Safety and Health Regulations 1996 Environmental Protection Act 1986 All other relevant Commonwealth and Western Australian Environment, Health and Safety Legislation and regulations.

2 Scope This procedure covers the manufacture, handling, storage and erection of tilt-up and precast concrete elements on projects under the control of EMCO Building.

3 Procedure

3.1 Training EMCO Building will ensure that all personnel undertaking work on projects were TiltUp panels are being manufactured or erected have the necessary training to ensure that work is conducted in a manner which minimises the risks to the persons carrying out the work, fellow workers, visitors to site and members of the public.

3.1.1 General Training and Safety Awareness EMCO Building will ensure that all personnel undertaking work on projects where Tilt-Up panels are being manufactured or erected, receive, at a minimum, training pertaining to the hazards and risks associated with the manufacture and erection of Tilt Up panels. The EMCO Site Manager/Supervisor will ensure that the hazards and risks associated with the manufacturer and erection of Tilt-Up Panels are communicated through: The induction process Toolbox Meetings Pre Starts Emergency Preparedness and Response Drills

3.1.2 Carry out Tilt-Up Work Safely EMCO Building will ensure that persons undertaking any of the following tasks involving Tilt-Up panels have completed a Worksafe approved Tilt Panel Safety Awareness Course (CPCCCM1007A) in accordance with legislative requirements (OSH Reg 3.88J): Casting of tilt panels Transporting of tilt panels Cranage of tilt panels Erection of tilt panels Stabilisation of tilt panels Conducting other work such as drilling, grouting, painting, to tilt panels while they are propped and until they are incorporated into the final structure.

EMCO Building will ensure that all EMCO employees including Site Managers, Site Supervisors, Project Managers, labourers, and apprentices have completed a Worksafe approved Tilt Panel Safety Awareness Course prior to commencing work on EMCO Building projects where Tilt-Up panels are being manufactured, or erected.

3.1.3 Supervise Tilt-Up Work EMCO Building will ensure that persons supervising work involving the manufacture, cranage, erection and stabilisation of Tilt-Up panels have completed a Worksafe approved Supervise Tilt Up Course (CPCCBC4022A) in accordance with legislative requirements (OSH Reg 3.88J [1a]) EMCO Building will ensure that all Site Managers and Site Supervisors have completed a Worksafe approved Supervise Tilt-Up Course where they are managing or supervising work on projects where Til-Up panels are being manufactured, or erected.

3.1.4 Recording and Maintenance of Training Documentation EMCO Building will ensure that the recording and maintenance of training documentation is carried out in accordance with HSEPC007 EMCO Procedure Induction & Training.

decides whether there is extra reinforcing required on the panel to enable it to be lifted, also responsible for all temporary inserts. Site Manager: to ensure a competent person is appointed on site to supervise the Tilt-Up and Precast Concrete Construction. Notify the WorkSafe Western Australia Commissioner of the intention to carry out tilt-up construction work. This notification is to be made available 5 working days prior to work intended to begin. to ensure sufficient lead-time for the planning and documentation phase of the project and to factor this into letting of the contract. to ensure environmental legislative requirements are met. Rigging Contractor: delivery of workers who possess certificates of competency in rigging. The rigging contractor has a responsibility for performing prestart meetings with his staff and advising EMCO of any deviations from the work plan or JSA submitted. Crane Driver: Person holding a competency certificate relevant to the crane required to undertake the panel lift.

3.2 Planning 3.2.1 Pre planning and Co-ordination Pre-planning and close coordination is essential for the safe, efficient and environmentally sound construction and erection of tilt-up construction. EMCO will, in association with the design engineer, the manufacturer and erector ensure the complete construction and erection sequences have been planned and signed off prior to work commencing. Factors to be considered in pre planning include: site limitations; local street access; element sizes, shapes and masses; crane size, mobility and access; casting sequence; overhead obstructions, especially overhead power lines and environmental considerations of the location (near waterways, neighbours etc)

3.2.2 Documentation The following documentation will be available at any EMCO site involved in Tilt-up and Precast Concrete construction as per the requirements of the Occupational Safety and Health Regulations 1996 (r 3.88):

AS 3850 Tilt-up concrete construction; WorkSafe Commission Code of Practice Tilt-up and Precast Concrete Construction; copy of the approval by the WorkSafe Western Australia Commissioner in relation to the work; copy of EMCO procedure for Tilt-up and Precast Concrete Construction; copy of the Method Statements and JSA for casting and erection; a copy of design engineer approved shop drawings; and a statement by the manufacturer that each precast or tilt-up concrete element has been manufactured in accordance with design engineer approved shop drawings.

The Project Manager is responsible for compiling the Site Folder that contains all documentation and plans for the precast and tilt-up work. A copy is to be maintained for the rigger, and a condensed version for the fabricator. The fabricators condensed version includes the certified drawings, installation methods and requirements for all casting and items, engineers certification of panel compliance, concrete strengths and design mixes, and any other material relating to the fabrication that the engineer deems necessary.

3.2.3 Notification to WorkSafe Commissioner Regulation 3.88 requires that the WorkSafe Western Australia Commissioner be notified of the intention to carry out tilt-up construction work. This notification is to be made available 5 working days prior to work intended to begin. The Site Manager will perform the notification on behalf of EMCO.

3.2.4 Work Method Statements All contractors involved in the tilt-up construction will be required to submit Method Statements (Code of Practice Tilt-up and Precast Concrete Construction for Buildings section 8.3) for approval to the Site Manager and Erection Design Engineer prior to the work commencing. The Method Statements will comply with the requirements in the Appendices and will contain: Persons involved in the work, site induction details and specifying their competencies (to be sighted by the Site Manager) and responsibilities on site for each stage of the erection process (Appendix A); a risk analysis to identify critical activities involved in the erection and description of how the risks will be eliminated or minimised including a traffic management plan (Appendix A) a panel construction master checklist with stage sign-offs (Appendix B) a panel lifting schedule (Appendix C); a panel casting schedule - derived from the erection sequence (Appendix D); a panel fabrication checklist with stage sign-offs (Appendix E); a concrete pour record (Appendix F); description of the erection procedure (Appendix G);

methods of stabilisation (temporary or permanent) of the structure including brace torque checks and visual inspection of panels (Appendix H); a statement by the erection design engineer that the erection design and detailing is in accordance with AS 3850 and AS 3600; design calculations prepared by the erection design engineer.

3.2.5 Shop Drawings The design engineer will ensure that the shop drawings comply with the structural design. The project manager must check the shop drawings for compliance with dimensions. Shop drawings should include all the following as a minimum standard for panel drawings: Overall dimensions of the panel All reinforcement information All necessary lifting and propping points Any other castin items The concrete pour strength The concrete lift strength The panel thickness The panel area The volume (amount of concrete) The mass of each panel The location, orientation and depth of embedment of inserts, and where the insert type is known, the configuration and cover of any component reinforcement required. The diagonals The connection loadings for all temporary propping The brace type, size, safe working load limit, extended and closed limits, details (both temporary and permanent) The torque and limitation The fixing lengths and angle The sling or rigging configuration The engineers approval (signature) and date The name of the panel designer The panel identification number

3.2.6 Stacking and Erection Sequence The following factors shall be taken into consideration when determining the stacking and erection sequence: The shape, size and mass of precast members. Specific design requirements, including the concrete strength required for handling and the stability of long or unusually shaped members during handling, transportation and erection. The size and mobility of the crane needed to erect at the precast members. The suitability of the site crane, if available, should be determined at the project design stage if possible.

The manufactured size of the precast members will depend on the availability of suitable cranes with particular attention given to the lift capacity at maximum radius. Where a mobile crane is to be used, the adequacy and strength of the erection platform. Where the erection platform is a suspended slab, the suspended slab may either be designed to carry the additional loads, or temporary supports may be used. All suspended slabs and temporary support systems intended for use as erection platforms should be confirmed as being adequate to withstand the anticipated loading. Access for the crane and the precast members through local streets and all-other access onto and across the site. Overhead obstructions, particularly power lines Erection equipment and capacity and location of lifting inserts Temporary storage, where required

3.2.7 Rigging Activities The following factors shall be taken into consideration when planning the rigging activities: Size of panels Specific design requirements eg the required concrete strength at time of lift. Size and mobility of crane needed to lift the panels. Adequacy and strength of the erection platform. Access for the crane through local streets, onto and across the site and on or alongside the casting bed for erection including the provision of a lifting plan, which considers underground services. Casting sequence, positioning of panel stacks, erection sequence and mobility of crane for erection. Overhead obstructions, particularly power lines Intended location of inserts and rigging system to be used. The possible need for on-site transportation of panels. Traffic management plan interface plan Site interface -Traffic management plan, fall zones, exclusion zones, ground conditions and services Surrounding interface traffic management, fall zones, and exclusion zones pertaining to public, plant, equipment, vehicles, existing structures, power lines, ground conditions, underground services. Job Safety Analysis - as identified in the Method Statement, high-risk activities to be written as separate JSAs. Risk assessment as per the JSA. Toolbox meeting/communication Contractor specific and site toolbox meeting to be planned for each lift.

3.3 Production and Erection 3.3.1 Panel production Refer to the pre-planning phase, to ensure a plan is developed for the appropriate location of the pour, given consideration for lifting sequence, crane access. Ensure the erection platform is capable of withstanding the downward forces of the proposed panels. All steel dockets to be kept as per document. Formwork is prepared (Appendix E). Project Manager to ask Electrician to locate panels with any conduits required prior. When pouring is to be performed conduits can be placed. The Box Numbers for each panel are to be recorded, as per the Building Casting Bed Schedule (Appendix D). Panels then released for pouring after the Site Manager signs off on the Panel Fabrication Checklist (Appendix E). Record the Date, Time and Ambient Temperature for each Concrete pour. Sign off on the Panel Construction Master Checklist. A Concrete Pour Record is then developed (Appendix F). Testing/Sampling: The Site Manager needs to determine how the sampling regime is to be managed. The Panel Fabrication Checklist (Appendix E) covers the fabrication of the panel. The concrete pour record (Appendix F) provides information on date, time and type of concrete that was included, weather conditions etc. The Site Manager is to discuss with the engineer to find a suitable sampling schedule. The Engineer determines the increase and the safety factor, so that the lifting can occur at a nominated period of days after the pouring. Cylinders from various panels are taken and then crush tested. It is important that during the curing of the sample it is left next to the panel to enable the same rate of curing. The progressive crush is to be verified by NATA laboratory, to achieve the required lift strength as specified by the temporary engineer. 3.3.2 Panel Lifting Ensure the site is briefed in the Site Toolbox meeting of the activity and location of the lifting of the panels. Site Communication is essential for ensuring the workers are kept informed of the lifting to occur. These are all to be documented, both site specific (everyone) and for rigging contractor performing the lifting. Everyone is to be signed on the toolbox meeting form. If practicable, exclude the non-essential trades from the site. Rigging Contractor to discuss the Job Safety Analysis for the lift. Rigging Contractor to check again the crane selected is suitable for the task, and the boom is within radius.

Rigging Contractor to refer to the Rigging Method Statement for Panel Installation (Appendix G). Ensure the copy of the Master file to be at the location for the rigging contractor is accurate. Cordon off the area to exclude all personnel not involved in the lift. If practicable, close entry to the gate and where practical the cordoned off area is to be three times the height of the panel. Use and compile the Panel Lifting Schedule (Appendix C). The panels are sequenced for the lift, as required by the engineer. It is the responsibility of the Site Manager to ensure each panel is lifted in the correct sequence. Rigging contractor to place temporary props. Stabilisation of both temporary and permanent structures is critical to the safe operation of the job. Rigger to torque each bolt and carryout checks as specified by the Bracing Engineer. Panels inspected after erection and Brace Bolt Torque Checks & Panel Visual Inspection Register completed (Appendix D). Certifying Engineer to sign for the structure being in place. Site Manager then signs off Panel Construction Master Checklist (Appendix B).

3.3.3 Post Erection Site Control All incidents will be investigated as per EMCO Safety and Health Management Plan.

Complete Panel Lift Schedule as required in the Panel Construction Master Checklist. Set-up selected crane in the correct location at a suitable predetermined radius for the panel being lifted. Select correct rigging equipment using chains and slings etc with suitable W.L.L. Place required temporary prop on panel and fix using specified fixings. Connect rigging equipment to panels. Hoist panel from panel bed under guidance of 2 Riggers (ensure JSA is being followed). Hold firm and place bars or brackets on panel as required. Place panel into design location and fix temporary brackets. Remove rigging equipment.

Appendix J Tilt up & Precast Panel Awareness Training for Site Personnel The contents of this information sheet must be communicated to all site personnel on EMCO Building projects where Tilt up & Precast Panels have been erected and braced prior to incorporation into the final structure. 1. Background Tilt up & Precast Panels are a typical method of construction within the commercial building sector. Tilt up & Precast Panels may come in various sizes, shapes, thicknesses and finishes. While the use of Tilt up & Precast Panels is common, the hazards associated with their construction must never be There have been and continue to be numerous cases of tilt panel collapse within the construction industry in Western Australia. In September of 2002 at a construction site in Myaree, a worker was killed as the result of a tilt panel collapse. All site personnel are asked to take the utmost care when undertaking work on construction sites where Tilt up & Precast panels exist. 2. Training It is a legislative requirement to have completed a Worksafe approved Carry Out Tilt Up Work Safely Course if your work involves the casting, transport, carnage, storage, erection, or stabilisation of tilt up or precast panels. It is a legislative requirement to have completed a Worksafe approved Supervise Tilt Up Work Course if your work involves the supervision of any of the aforementioned activities. It is a requirement of EMCO Building that persons involved in the drilling, coring, grouting, painting, cutting, patching of, or fixing to tilt panels while they are braced to have completed a Worksafe approved Carry out Tilt up Work Safely Course. Site personnel must speak to their EMCO Site Manager of Supervisor if they are required to undertake work with Tilt up or Precast Panels and believe they do not have the necessary training. 3. Working Safely Around Tilt & Precast Panels 1. At no time are non essential site personnel permitted to enter the exclusion zone where tilt panels are being lifted or stabilised. 2. At no time are site personnel to touch, tamper with, or move props, braces or any other component which is used to lift, brace or support tilt panels. 3. At no time are site personnel permitted to climb on or over props or braces which are supporting tilt panels

4. Site personnel are asked to maintain a safe distance from propped or braced tilt panels at all times. 5. At no time are site personnel permitted to operate mobile plant (bobcats, EWPS, cranes, telehandlers) in the vicinity of propped or braced tilt panels. Where it is necessary to operate plant in close proximity to propped or braced panels, the EMCO Site Manager/ Supervisor must be informed so that adequate controls can be put in place to prevent panels collapsing.
