SECTION A - SAFETY AND HEALTH PROVISIONS Health Provisions Yes No 1 Every workroom is adequately ventilated. 2 Effective means have been provided to reduce workers exposure to vibration. 3 Effective measures are taken to prevent accumulation of toxic air contaminants and to protect workers from inhaling them.
4 Regular hygiene monitoring is carried out for toxic airborne substances. 5 Precautions are taken in accordance with the Safety Data Sheets (SDS) of chemicals used.
Noise Provisions Yes No 6 Measures are taken to reduce or control noise from any noisy machinery, equipment or process.
7 Where it is not practicable to reduce noise, the duration of workers exposure is limited.
8 If 50 or more workers are exposed to excessive noise, a Noise Control Officer is appointed.
9 If 10 or more workers are exposed to excessive noise. Noise monitoring is carried out at least once every 3 years.
10 Hearing protectors are provided to all workers exposed to excessive noise. 11 Warning signs indicating the use of hearing protectors are posted at all entrances to areas where persons are likely to be exposed to excessive noise.
12 A training program is implemented, and conducted every 3 years, for all workers exposed to excessive noise.
13 Yearly audiometric examinations are conducted for all workers exposed to excessive noise.
Safety Provisions Yes No 14 Every dangerous part of the machinery in the factory is securely fenced. 15 Efficient devices or appliances by which power of the transmission machinery can promptly be cut off is provided and maintained.
16 All fencing or other safeguards are of substantial construction, properly maintained, kept in position and properly adjusted while the machinery is in use.
17 All electrical installation and equipment is of good construction, sound material, free from defects and maintained.
18 Practical measures are taken to protect any persons against the risk of electric shock.
2 19 Lock-out procedures have been established and implemented to prevent inadvertent or accidental activation of machinery.
20 Every person carrying out any inspection, cleaning, repair or maintenance of any plant, machinery, equipment or electrical installation is fully instructed on the lock- out procedures before commencing work.
21 All reasonable steps are taken to ensure self-acting machine do not pose safety and health risk to any person.
22 Goods are stored, stacked or placed on firm foundation and do not obstruct the proper operation of machinery or other equipment.
SECTION B DOCUMENTATION Training Yes No 23 All machine operators are trained and competent to operate the machinery. 24 All workers have attended the Safety Orientation Course (Metalworking). Risk Assessment Yes No 25 Risk assessments have been conducted for the operation of the machinery. 26 All reasonably practicable steps have been taken to eliminate any foreseeable risk and where it is not practicable to eliminate the risks, reasonably practicable measures have been implemented to minimize the risk and safe work procedures implemented to control the risk.
27 Record of the risk assessment is maintained and kept for a period of not less than 3 years.
28 Any persons exposed to risks are informed of the nature and the safe work procedures implemented to eliminate and control these risks.
29 The risk assessment is reviewed and revised at least once every 3 years; and upon the occurrence of any bodily injury or significant change in work practices or procedures.
Safety Management System Yes No 30 Factories which employed 100 or more persons have implemented a safety and health management system.
