TITLE: INTERVIEW WITH
SUMMARY:
On October 3, 2013, Special Agent Matthew Collins conducted an interview with
at the Woodmere High School Administration Building in Elmore, Ohio. Also present during
the interview were his parents, on the Woodmere
High School football team. .
During the interview, was asked if he had been present and participated in practice on
Tuesday, September 10, 2013. He was asked about normal practice drills and drills conducted
on September 10. was asked ifhe had witnessed anyone become injured or sick that day.
He was also asked if there were any hits that day which stuck out in his mind.
DETAILS:
On October 3, 2013, Special Agent Matthew Collins conducted an interview with
at the Woodmere High School Administration Building in Elmore, Ohio. Also present during
the interview were his parents, is a senior on the Woodmere
High School football team. .
was asked if he was present during practice on Tuesday, September 10 and he stated yes.
was asked if he recalled how long practice was that day. said it started around 3: 15 p.m. and
lasted until 6:30p.m. stated they did not have that many water breaks that day. He said
there was one water break the first hour of practice and believes the others were one per hour.
was asked to describe a typical practice. said after stretching they go to a drill called
Noose where they catch balls and do releases. Then they do Pat & Go, where they run their
routes and work on their releases again. After that they would go to quick game or routes on air.
Then they would go to a 7 on 7 drill. From there they would go to team offense (first team
offense vs. second team defense). Then they would go to individual defense, where they work
on back peddling and tip drill. After individual defense, they would do team tackle or team
takeaway.

was asked to describe the drills they would do during team tackle. said there is one
drill where a black pad is used and the players stand five yards away from each other. One
person puts their arms up so the other person can "throw their bowling balls," and the player can
tackle them. Another drill he described involved one person, down on one knee "throwing his
bowling balls," and another player would pick them up. The next drill used a two person dummy
and they would back-pedal, then run up and tackle the dummy. The last drill is called the mirror
drill. There is a ball carrier, a blocker and a defender working on shedding the block.
Then they have team takeaway drills (tip drill, fumble recovery and strip drill). After that they
go to team defense vs. scout team offense, and then finally team conditioning.
was asked to describe what occurred on Tuesday, September 10. said they started out
with Noose and Pat & Go and went to passing practice or quick game. said the coaches
were mad at them and they did not get a drink like normal. said the coaches who were
present at this time were, and said they were in "thud"
(helmet, shoulder pads and shorts). said they never got to the 7 on 7 drill because the
coaches sent them in to get changed. said told them they had eight minutes to
change into full gear. said they did not take him seriously because they did not think he was
actually keeping time. was asked if was there during this time and he
stated no. said did not show up until after he got out of the locker room.
was asked what he did once he got out to the field after changing. said he went over to
get a drink of water. said after that, told the players who were out on time
to go stand in a line. said called them "New Woodmore" and said they were
waiting on "Old Woodmore." said there were about 30 seconds left remaining for the others
to get out to the field. said when the "old Woodmore" came out, lined
them up about 15 yards across from "new Woodmore." said all the other coaches were
present, but they were standing by the water station, which was not far away.
said had told the "old Woodmore" players they were to just stand there and put
their hands behind their backs and they could not defend themselves. said a couple of the
players did defend themselves and they had to go run as a result. said had pushed
them toward the "old Woodmore." was asked what "new Woodmore's" instructions were.
They had to "go after them" and hit them for being late. said they were "old
Woodmore" and we do not want to be like them. said Eric Greulich, John Morris and
himself were talking to each other between hits. They decided amongst themselves to not hit
anyone hard. was asked if he recalled who he had hit. said he hit four people; Brad
Bringman, Todd Williams, Parker Lowe and Jake Matwiejczyk. Jake was the last person to be
hit. was asked what they did after he hit them. said they waited for them to get back
from running to the com.
said once they got back from running, they went into team offense versus scout defense.
said again the coaches were not happy with their performance, so they went into another
defensive drill, which he thought was called "bull in the ring." described the drill as the butt
and press drill, however he claimed they were to do whatever they wanted for one minute.
said and call it "the pit." said after that they went back to
team offense.

said after a water break, they went to team defense. He said it was about 5:30p.m. and the
athletic director (A.D.) carne out and told them that they had to go in. said
was talking to the A.D. said he heard him say it was hot out and they should end practice
pretty soon. said the A.D. also asked everyone if they were getting enough water. said
no one said anything because they did not want to get in trouble. said he heard
say to the A.D. there is no law that says we cannot practice. He also told him they had
more drills to practice. said later they ended practice without doing team conditioning.
was asked if there were any hits that stuck out in his mind. said no. was asked if
he had seen anyone who was sick or injured that day. stated there were five to six people
who he thought were injured; Jake Matwiejczyk, Chase Kuchenriter, Dustin Haar and his brother
said he heard there were people at the volleyball game asking what was going on
in practice because there were so many players going to the locker room. said the starting
quarterback had two broken fingers on his non-throwing hand. said Chase Kuchenriter was
hurt, but he did not know when, he just saw him walking to the locker room. said Haar was
hurt during the "pit" drill when his fingers were smashed in a facernask.
was asked when he noticed was hurt. said he did not know when was
hurt. said he saw and he looked sick. said he saw him leave the field during
practice. said left the practice field and headed toward the locker room by himself.
said he did not see him for about ten minutes and then he carne back. said they were
doing team defense. saw say something to did not know
what was said, but did not put back in. said later he saw
lying on the ground with a group of people around him. said he saw Tyler Mulligan and
Zach Sandwisch help take his helmet and shoulder pads off.
said about 10-15 minutes later, went over and talked to him. At that point,
saw Zach and Tyler help up and walk with him to the locker room. said he
could tell something was wrong because he was not walking right. was asked when he saw
next. said after practice carne up to him and told him he should go
check on his brother because something might be wrong with him.
said he saw his brother lying down with an ice pack on his neck with no shirt on in the
trainer's room. said Tyler Mulligan was on one side and Colin Sandrock was on the other
side. said they were trying to keep awake because the trainer was outside talking to
morn. said he walked up to and asked what was wrong, but he never
answered him. said he noticed pupils were really big and that is when he knew
something was wrong. said he went outside to see where the trainer was and found him
talking to morn on the telephone. said the trainer carne in long enough to see how
bad was and then walked back outside. said that is when the trainer called his morn
again and told her they need to take to the hospital.
said he saw the trainer walking out with and opened the door for
said did not even know how to sit down so they had to help him into the car. said
the trainer told him to take horne and his parents would know what to do. said he
did not think was that bad, so he took Tyler horne first. said Tyler was trying to
keep awake by slapping him. said they talked to but he did not respond.

After dropping Tyler off, went to his house. said there was never a time that
said anything to him.
said once he got home, pulled in behind him. said he heard tell
his parents that took a couple ofhard hits at practice. asked was okay
and if he needed to stay with while his parents went to the ER.
SA Collins asked if he and his brother played the same positions on both offense and
defense and he said yes. was asked if they were ever separated during practice. said
only during team offense or team defense, would line up on the other team. was
asked if anyone had told him not to talk about what happened at practice that day. said he
only heard Toni Rozzi tell someone they should not say what happened at practice because they
could get the coaches in trouble. said Toni does not know he heard this because was
using the restroom and Toni probably did not know anyone else was in there. said J.T.
Atkins had also come to him and told him someone (Unk) had told J.T. not to talk about what
happened at practice.

On October 3, 2013, Special Agent Matthew Collins conducted an interview with
at W oodmore High School in Elmore, Ohio. is for the
Woodmore High School football team. was asked ifhe was present for the practice on
Tuesday, September 10, and he stated yes. He was asked if he recalled how often water breaks
were given that day and he stated they were about every ten minutes or after every drill.
was asked to describe the typical drills run in practice. said he is the offense
line coach. He usually has his players run through ladders using a couple different variations of
footwork through the ladder rungs. From there they go to a water break and then to what he calls
the "little things." said it refers to the footwork, or first step on a zone play, first step
on a tag play or a first step on a pass play. Then they go to the sleds and do different blocks
using the sled. Then they go to what they call PUP, or pass under pressure. PUP is where they
will use the upcoming opposing team's style ofblitzes and give the offense different looks.
From there they will come into team offense and run screen plays, followed by down field
passing plays. He described the drills to be run in 15 minute intervals with water breaks between
each drill. Next, they do team defense. They start out with a drill called pursuit, teaching the
players how to use angles to catch someone down field. Then they go to team tackle.

each other. said the drill he is in charge of has a player down on one knee and another
standing across fiom him, facing him. He described how they throw "bowling balls" and wrap
up. He said it will usually last about three to four minutes and then they rotate.
said team takeaway is next. This drill uses stations lasting three to four minutes each.
The drills are used for stripping the ball, intercepting the ball or anything to cause a turnover.
Then they go to individual defense, where he teaches pass rushing. Then they go to team
defense versus scout offense. At the end of practice, they do team conditioning.
was asked to describe what he recalled from Tuesday, September 10.
stated it was a little warmer than usual that day, so they did not have the players in full gear at
the beginning of practice. stated they had the players come out in helmets and
shoulder pads. He said they started off like any other practice, stating they followed pretty much
the same procedure as described above. said at some point, was not happy
with receivers so he sent the whole team in to put their full gear on. He said they had eight
minutes to do this. said about a quarter of the team made it back to the field on time
and the remaining 75 percent did not. said this was done in order to get a change out
of the players, as far as their intensity. said also thought it would be a good
idea to start off with defense. He thought they could get some hitting done and some "rah rah"
going, so they decided to go with team tackle. stated as they were lining up, he walked
over to where he is normally stationed and prepared for the players to rotate to his drill. As the
players started to come out, and started grabbing the players and
lining them up across from each other, based on who came out on time or late. said
they were lined up about three to five yards apart. said it looked similar to the drill that
he does, but has always done that drill standing up. stated it was no
different than what they had done in the past, aside from the use of stations. said once
the tackle was made, the player who came out late had to run to the cornfield or what everyone
calls "running to China."
SA Collins asked to provide a more detailed reference of the instructions for the
tackling drill. said he heard the players who were on time were told to stand in one
line and the late players were told to stand in another line. The lines were about three to five
yards apart. said the late players were told to stand there and take the tackle.
stated was giving the instructions. said several times, he wanted to
be clear with SA Collins that this drill was done for weeks. SA Collins asked him what other
specific instructions were given for this drill. stated he did not hear all of the
instructions given, because he was 20-30 or 30-50 yards away. Normally they split the players
up into groups and do stations, which is why they were separated. was adamant that
this was a normal team tackle drill, done week after week by SA Collins
asked if it was true that the drill was usually done in smaller groups. stated
yes and described how defensive backs would be set up in one group, linemen in another group
and linebackers in another group. SA Collins asked if they were separated based on their skill
set, and stated no. said, "Position, not necessarily skill set, just because
you're a lineman doesn't mean you're that good and a freshman lineman versus a senior lineman
is a lot of times not comparable." SA Collins asked if during this drill, it was possible for a
senior to be matched with a freshman. stated it appeared to be more at random.
added that he did not think the skill set of a player mattered in this drill because it was

not a "full go" drill and was more about fundamentals. was asked if it appeared
players were throwing their bowling pins correctly and he said, "Absolutely."
was asked if this drill had a name and he said it was called team tackle. He was asked
about the term "New Woodmore" or "Old Woodmore." said the only time he has ever
heard those phrases was during camp when a speaker came. said he did not hear it on
that day. said he has heard the rumors, but did not hear it on that day. He again added
that he was 30-50 yards away.
was asked if any hits stuck out in his mind from that drill. said no, and he
did not know when the appropriate time would be, but he wanted to reiterate what he had told the
principle and superintendent already. He said he did not know anyone got hurt until the next day
when he had come to practice. Nothing stood out that was different from any other practice. He
said one of his complaints was that the student who was allegedly injured during that drill had
gone "full go" through scout team offense and scout team defense. He went on to say it was an
hour and a half later that this player sat down. clarified that he did not even know this
player sat down because he did not know anyone was hurt until the next day. went on
to describe how he is and knows what to do in those types of situations.
said there was nothing that day that indicated a severe injury of any sort. was
asked if he was told had a concussion and he stated no. said the next day
told them a serious injury occurred at practice. said if he would have
seen someone on the ground, he would have gone over to them.
was asked what they had done immediately after the team came out with full pads and
completed the hitting drill. said they went to "bull in the ring," which is another drill
they have done on numerous occasions. said this is a drill they usually do the very first
day of full contact each year to get the players into it. He said they also see some good hits
during this drill. described the drill as having a ball carrier and a defender. The
defender had to shed a block and make a tackle on the ball carrier.
was asked if he had seen any other kids getting dinged up or slightly injured during
practice and he said no. was asked if he had seen prior to practice that day and
he said no. He clarified that he could have seen him in the locker room, but did not talk to him.

On October 3, 2013, Special Agent Matthew Collins conducted an interview with
at the Woodmore High School Administration Building in Elmore, Ohio. Also present
during the interview were his parents, and

Prior to the interview, SA Collins had been informed that was having
difficulty recalling any events of Tuesday, September 10. was first asked ifhe had
participated in practice on September 10 and he stated yes. said practice started at 3: 15
p.m. and said he got home around 6:30 p.m. was asked how many water breaks the team
had that day and he believed they had four.
was asked to explain what a typical practice consisted of. said they start off with
stretching. He described how the backs and receivers would split off from the lineman and work
on catching and running routes, while the lineman work on ladders and footwork. He described
this as the NOOSE drill. Then they would go to Pat & Go. Pat & Go is described as the
receivers working on getting the ball off with a defender in front of them. Once they get around
the defender, the defender does not follow them. described how the contact is initiated
by the receiver and they just slap the defenders arms away in order to get by. Then they would
go to a 7 on 7 scrimmage. After 7 on 7, the lineman would join up and they would run through

Interview with
first team offense vs. scout team defense. described how he was a cornerback on the
second or scout team defense. said they would usually run team offense for a long time.
Then they would do defensive positions, including team tackle. described there being
three stations during team tackle. He said one station, which was in charge of
had one person standing in front of a black pad with their arms up. The other person would be
five to six yards away and come up and tackle the other person (working on bowling balls).
Another drill is called "butt and press" where there is a ball carrier, a blocker and a tackler. The
ball carrier picks a side to run to and the defensive player has to find the ball carrier and shed the
block. said on Tuesdays, they actually take the ball carrier down. Another drill is where
they wrap up on a two man sled.
said they would also do team takeaway. He described there were stations, each having a
different drill (tip drill, strip drill and a fumble recovery drill). While was explaining the
team takeaway drill, he began describing the "bowling ball" drill, where one player is on a knee.
SA Collins asked to clarify whether he was talking about a team takeaway or team tackle
drill. first said he did not remember, but then realized that he was talking about a team
tackle drill. went on to describe working on a fumbling drill. said next they
would go to team defense versus scout team offense. said he would play running back on
the scout team offense. He also said there was team conditioning at the end of practice, which
was in charge of.
was then asked what he could recall from Tuesday, September 10, as it pertained to
practice. said he remembers going to practice in "thud" (shoulder pads, helmets and
shorts) and running through plays. He said that was about it. He does not remember how he got
undressed out of his full gear. was asked ifhe recalled how he was feeling before
practice. said he thought he must have felt good, because he had gone to school that day
and usually if he was not feeling well, he would have sat out. was asked what drills he
recalled that day and he stated he could not remember any of them. He also stated he could not
even remember stretching. stated his last recollection for that day was being in the
locker room changing prior to practice. said the next thing he recalls was being at the
hospital and having x-rays completed. He remembered an LV. in his arm. He said he does not
remember getting the x-ray, but remembers after it when he had the LV.
was asked ifthere was anyone specifically who had told him what happened while he
was at practice that day. said Tyler Mulligan came to the hospital and he could hear
Tyler talking to said he heard Tyler say they were standing 10-15 yards
apart. was asked if Tyler had told him who hit him. said no one knows who hit
him. was asked if anyone had told him that he had made a big play while playing scout
defense and he said no. was asked if he had been told ifhe was hit by someone else in
any of the other drills that day. said he was told he was the ball carrier during the "bull
in the ring" drill and he was hit two times. was asked who had told him this and he said
he could not recall. He went on to state that they told him it was not a hard hit.
stated he does not recall being in school that day.

Greulich at Woodmore High School in Elmore, Ohio. Eric's stepfather, Jason Scavio, was also
present during the interview. Greulich is a senior on the Woodmore High School football team.
Greulich plays the positions of offensive and defensive tackle. During the interview, Greulich
was asked ifhe had been present and participated in practice on Tuesday, September 10,2013.
He was asked about normal practice drills and drills conducted on September 10. Greulich was
asked ifhe had witnessed anyone become injured or sick that day. He was also asked if there
were any hits that day which stuck out in his mind. Greulich said he heard some good hits
during the "New Woodmore/Old Woodmore" drill, but could not recall specifics. Greulich
recalled seeing lying on his back with his arms out trying to breathe (later in practice).

practice but sat out the second part due to a previous injury which occurred in the game a week
before. He recalls practice starting at 3:15 p.m. and ending around 6: 15/6:30p.m. Greulich said
he could not recall how many water breaks they got that day, but said they usually get them
every 15 minutes.
Greulich was asked to describe the different drills conducted at a typical practice. Greulich said
after stretching they split up into positions groups. Lineman would do ladders, push the sled and
flip a tractor tire. After that they would go to team offense where they run through plays. Then
they would go to individual defense where they would work on defensive lineman moves. Then
they would go to team tackle where they work on tackling drills. Greulich was asked to describe
the team tackling drills. He said in one drill they would have a player on ony knee and they

Interview with Eric Greulich
worked on getting their hips through. Another drill required them to tackle someone onto a pad.
He described another drill where they would shed a blocker and make a tackle (Butt and Press).
He also described a fourth drill where they would just tackle a dummy. Greulich described the
pad drill as being five yards away and work on tackling through the person, instead of stopping
when you hit them.
Greulich was asked to describe the practice on Tuesday, September 10. Greulich said they
started off with stretching and then went to individual offense. He said the coaches got mad at
the receivers (Greulich did not know what they did to upset the coaches) and sent them in to the
locker room to change into full pads. Greulich said it was who had sent them in to
get changed. told them they had eight minutes to go in and put their pads on and come
back out. did not say what the result was going to be. Greulich said he was one of the
first ones out (on-time). Greulich was asked ifhe was told anything once he got out. He said he
was just told to wait by the water until everyone else came out. When the other people (late
ones) started coming out, lined them up. Greulich stated said
the people who came out on time were the "New Woodmore" and the people who did not make
it out on time were the "Old Woodmore." then told them to, "Just go hit 'em."
Greulich said he believed it was about five to ten yards. Greulich said Old W oodmore kids were
to just stand there and get hit. He said he did not know if they were told to put their hands
behind their backs or not (at least he did not hear that). Greulich was asked who he had hit
during this time and he said he hit Spencer Linki and Ben Wilt. Greulich further stated he had
taken his time getting back up after the second hit because he did not want to hit anyone else. He
said he did not know who hit Greulich said he did hear a couple of good hits
but did not know who was hitting who. Greulich said after that drill his leg (groin) started
bothering him again so he went into the trainer's room. Greulich was asked ifhe had seen
anyone else in the trainer's room and he said no, he had to wait on the trainer to get back from
getting dinner.
Greulich said he did come back out to the field and they were doing team defense. Greulich was
asked ifhe had seen anything during team defense, involving Greulich stated he had
seen lying down with his hands out like he was trying to breathe. He said the coaches
went to him and they had someone walk him in. Greulich was asked if he had heard
complain of being sick or injured that day and he said no, he really did not talk to him at all that
day. Greulich said from what he saw he thought had heat exhaustion or something like
that.
Greulich said he saw and tending to He said he saw
someone walk into the locker room. He thought it was brother, but really could
not remember, just that there was someone who did walk with him. Greulich was asked if they
used the cart and he said he could not remember. Greulich said he did not see again on
that day.
Greulich was asked if he saw deliver or receive any good hits and he said no. Greulich
said he did not see any significant injuries as a result of practice that day, but did hear people say
they were sore.

On September 30, 2013, Special Agent Matthew Collins conducted an interview with Logan
Fonseca at Woodmore High School in Elmore, Ohio. Fonseca is a junior on the Woodmore High
School football team. Fonseca plays the positions of free safety and wide receiver. Fonseca
stated he was present at practice, but not the entire time, due to a previous injury. Fonseca said
he was in the trainer's room for most of it and went out to the field around 5:45p.m.- 6:00p.m.
Fonseca said he witnessed the team getting water breaks and he believed they got two to three
breaks during the last hour of practice (which he was present).
Fonseca was asked to describe a typical practice day. Fonseca said they would typically dress in
full gear. They would do Team takeaway and Team tackle, walk through defense, team defense,
Noose, Pat & Go, individual offense and team offense. Fonseca described team takeaway as
working on fundamentals of stripping the ball, picking up fumbles and a tip drill. Then they
would do team tackle and he described how they would work on shedding a blocker and making
a tackle. A drill they call "bowling balls," where the player would get down on one knee and
tackle the person onto a pad, which he said was done from one foot away. Then they would use
a pad and wrap up on the pad, starting out five feet away. Fonseca said there were no drills
where they would be more than five feet away and tackle someone.

Interview with Logan Fonseca
Fonseca was asked to describe what happened on Tuesday, September 10. Fonseca said he came
out during Noose and Pat & Go, and said he missed the defensive part. Fonseca said they
skipped Indy "0" and went right into Team Offense. Fonseca said they ran plays for most of the
time he was out there. He described this as first team offense vs. scout team defense. Fonseca
was asked if there were any good plays made by anyone on the scout defense and he stated no.
He was asked ifthere were any good hits and he said yes. Fonseca further described that those
hits were done by the interior lineman. Fonseca was asked if anything stuck out to him during
that practice. Fonseca said one of the linemen hit someone pretty good. He described that Eric
Greulich hit either Thomas Snicker or Dennis Ratz.
Fonseca was asked if he ever saw and he said yes, when they were doing one of
the offensive things. He said went to the back and they had him lay down. Fonseca said
it was around 6:15 p.m. - 6:20 p.m. Fonseca said they had just finished a running play and
ran back and they had him lay down. Fonseca said he did not make the tackle at that
time. Fonseca said Tristan Roth said he thought was dehydrated. Fonseca said he
believed Zach Sandwisch had taken helmet off. He said both and
were both over by Fonseca said as was coming offthe field he
walked past He could not hear what they said to each other, but was
lying down and came over. Fonseca said was lying down with his hands
over his head. He said motioned for the cart (he was pretty sure). They took to
see the Fonseca said he did not see after that.
Fonseca said he had seen prior to practice that day and he seemed normal. Fonseca was
asked if he had seen any other players become injured or sick as a result of practice that day.
Fonseca was asked if he ever heard "New Woodmore or Old Woodmore." He said he only heard
it after the fact and was not present during that time.

On October 1, 2013, Special Agent Matthew Collins conducted an interview with Jon Orman at
Penta Career Center in Perrysburg, Ohio. Also present during the interview was Penta
Administrator, Jerry Gladieux. Orman is a senior on the Woodmore High School football team.
Orman plays the positions of offensive guard and defensive tackle. During the interview, Orman
was asked ifhe had been present and participated in practice on Tuesday, September 10, 2013.
He was asked about normal practice drills and drills conducted on September 10. Orman was
asked ifhe had witnessed anyone become injured or sick that day. He was also asked if there
were any hits that day which stuck out in his mind. Orman stated there were no hits that stuck
out in his mind. He did see lying on the ground, but did not know what caused it.
Orman saw after practice in the locker room and said facial expressions
indicated he was "out-of-it." Orman helped to his brother's car along with trainer.

asked if he recalled how many or how often water breaks were that day and he stated no, he did
not recall.
Orman said they usually go full gear on Tuesdays. He said lineman do ladders and things like
that. Then they will do team tackle or team takeaway and then go into team defense. Orman
described team tackle having one drill where they would start off about five yards away with a
big pad behind one of the players. Orman said they were split up into groups based on positions.
He said another drill they did was called "bowling balls." Orman described how they would pick
the player up and drive them back and then let them back down on their feet.

Interview with Jon Orman
Orman was asked about practice on Tuesday, September 10. Orman said they were not going
hard enough so the coaches made them go back inside and put full gear on and they had eight
minutes to do so. Orman said they were in just helmet, shoulder pads and shorts prior to this.
Whoever came out faster was on one side and whoever came out late was on the other and they
would tackle each other. He clarified that "New Woodmore" came out first and "Old
Woodmore" came out last. New Woodmore was instructed to hit/tackle Old Woodmore. Old
Woodmore just had to stand there. Orman was asked if there were any other instructions given
to Old W oodmore, as far as their hand positions or how they were supposed to stand. Orman
stated he was a part of Old Woodmore and he never heard any instructions. Orman was asked if
there were any instructions given to New Woodmore on the way they were supposed to tackle
and he said no, just tackle them. Orman was asked who was giving those instructions and he
said, He was asked what other coaches were present during the instructions.
Orman said and were also
present. Orman was asked if there were instructions from any of the other coaches and he said
no.
Orman was asked ifthere were any hits that stuck out in his mind and he said no. Orman was
asked what they did after being taken to the ground. Orman said they went into team tackle.
Orman said they did their stations based on positions. Orman was asked if, in this rotation he
had seen being involved in any hits and he said no. After that they went into
team defense. Orman said would be a sub for the scout team offense. Orman said he did
not see any hits but did see standing up and then the next thing he saw was lying
down. Orman did not know how long was lying there. Orman did state that he saw
go over and check on When asked if there were any other coaches who
went over, Orman said pretty much all of them checked on Orman said he knew there
were other players around at that time, but could not remember who. Orman was asked
if he had seen leave the field. Orman said he believed went off on the golf cart,
but did not pay attention because he was on defense.
Orman was asked if he had seen after practice that day and he stated yes. Orman said he
went to the trainer's room to get ice and saw standing in there. He said did not
look good. Orman said did not appear to know where he was. He said his facial
expression indicated he was "out of it." Orman said he had to help to his brother's car.
Orman said the trainer helped too. Orman could not remember any other kids helping to
the car.
Prior to practice that day, Orman did not see in school or in the locker room. Orman also
does not recall any other player being sick or injured as a result of practice that day. Orman was
asked if had told him he was sick or complained ofhis head or neck injury and he stated
no.

Weishuhn stated he was in the trainer's room getting his ankle taped when the other players had
come in to get changed into full pads. Weishuhn stated practice usually starts around 3:15p.m.,
and lasts until 6:30p.m. Weishuhn stated practice lasted a little bit longer on Tuesday,
September 10. Weishuhn said they always got plenty of water and believed they got water breaks
every 15-20 minutes.
W eishuhn was asked to describe what occurs during a typical practice. Weishuhn said they start
off with catching drills called Noose, then go to Pat & Go and then to "quick game." Weishuhn
said each ofthose have little to no contact. Weishuhn said they will then go into defensive
practice where they have tackling drills and then go into team defense vs. scout team offense.
They do team takeaway (interceptions and fumbles). He said they really do not hit in team
takeaway. Weishuhn said team tackle consists of drills for form. He described a drill where they

Interview with Ian W eishuhn
get down on one knee and another person is lined up a foot away with their arms out and they
work on their form. In another drill they use a large pad, almost like a mattress, one player
would put his heels on the pad and the other person would approach from five yards away and
tackle him onto the pad.
Weishuhn was asked to describe what occurred on Tuesday, September 10. Weishuhn said while
he was in the trainer's room getting taped, he saw everyone coming into the locker room to get
changed. Weishuhn said they had started practice in what they call "Thud" (helmets, shoulder
pads and shorts). Weishuhn said he was limited in his participation in practice that day so when
he went out to the field he only wore Thud. Weishuhn said he was walking out and could see
people lined up, but did not see any hits. Weishuhn said he did see people running to the com.
When Weishuhn made it to the practice field the team was doing "quick game" and that is when
he joined in. W eishuhn said the coaches seemed upset that day but he knows sometimes they
just act upset in order to get them (the team) going or motivate them. When they were doing
"team" W eishuhn did not participate. He said he stood off to the side with the coaches.
Weishuhn was asked ifhe had seen anyone who appeared to be sick or injured. Weishuhn said it
had been a while into practice and he saw one of the coaches talking to He said
was lying on the ground with his knees bent. Weishuhn knew there were other people standing
by but could not remember who specifically, however he did name off a couple
sophomores and freshman who were usually standing there. He said he was standing about 10-
15 yards away from during this time. W eishuhn could not remember for sure which
coach was talking to but he thinks it was or He did
recall, whoever it was, telling one of the kids to go get the trainer.
Weishuhn said he heard one of the coaches tell that was overheated and
that he did not feel good. He said the trainer came out on the golf cart and took into the
locker room on the cart. Weishuhn said he saw later, after practice when was in
the trainer's room. He said was lying down with a cold towel over his face. Weishuhn
said and the trainer were outside calling and parents. W eishuhn said he asked
how he was doing, but did not respond.
Weishuhn was asked if he had seen any big hits that day, whether it was delivering or receiving,
and he stated no. Weishuhn said usually the coaches blow the whistle before they get the chance
to see any really big hits.

Interview with Dustin Haar
Haar described having three people involved, one being a ball carrier another person is butt and
pressing, shedding and getting the tackler. Haar does not know what that drill is called. Haar
said they used to do "Bowling Balls". He described them being between three and four feet
away and tackling a person onto a mat or pad. Haar said they did it when was
there. Haar said another drill was called pursuit. In pursuit, they would chase a ball carrier, but
would not tackle them. Haar said they would do first team offense vs. scout team defense and
first team defense vs. scout team offense.
Haar was asked to describe what happened during the practice on September 10. Haar said they
started out in "Thud" (helmet, shoulder pads, shorts). Haar said the coaches said they were not
going hard enough so they had to go into the locker room and change. Haar could not recall who
had told them to go inside. Haar said they had eight minutes to get back out and he made it on
time. Haar said the people who made it on time got to stand in a group. Haar said, "The people
who did not make it out on time had to get into a line and stand there with their hands .. .just
stand there. The one coach just told them to stand there and have us tackle them. They couldn't
do anything about it, they just got tackled." Haar said the coach just told them to stand there he
did not tell them exactly how to stand (whether straight up or crouched). Haar was asked what
he was told to do. He said he was told to go tackle them as hard as he could. Haar said
was the one who was giving these instructions. Haar was asked what other coaches
were there and he said they were all there. Haar said he did not think was there
because he has school. Haar was asked if he recalled who he hit. Haar said he hit Henry Flores,
Dillion Williams and Tony Rozzi. Haar said the hit between him and Rozzi was a pretty hard
hit. Haar said five to eight people made it out on time. He said "Old Woodmore" was only
getting hit once. Haar was asked if any of these hits stuck out in his mind and he said yes. Haar
said he felt like a "Dick" because they were his friends. Haar felt like he had delivered a couple
good hits himself but no others stuck out to him. Haar said after the hit, the players who had
been hit had to run to t ~ corn and back. After this drill, they went into a normal practice.
Haar was asked if there were any other hits during practice at all that day and he stated no. Haar
said he could not remember if they did team tackle drills after that or not. Haar was asked if he
had seen either sick or injured that day. Haar said when they were doing team
offense he saw lying on the ground. Haar said he went and got water because he
thought was overheated because it was really hot that day. Haar was asked if there was
anyone else by at that time, specifically coaches. Haar believed was there.
Haar did not hear any conversation between and Haar was asked if he had talked
to and he said no, that he was just being nice and got him water, then went back to
practice. Haar was asked if he recalled seeing leave the field and he said yes. Haar said
he believed was with Tristan Roth. Haar could not recall if left the field walking
or on a cart.
Haar was asked if he saw after practice that day. Haar said he saw in the trainer's
room with an ice pack on his head. Haar said he did not hear anything from while he
was in there. Haar was asked who else, if anyone was in the trainer's room at that time. Haar
said the trainer, and Colin Sandrock were all in the trainer's room.
Haar did not see any other players who were hurt or injured as a result of practice that day.

Linke plays the positions of offensive guard. and defensive tackle. Linke stated he was at
practice on Tuesday, September 10. Linke said practice started at 3:15p.m. and ended at 5:00
p.m. or 6:00 p.m. Linke said practice used to start at 3:00 p.m., but since a few of the players go
to Penta, they moved it to 3:15p.m. Linke said sometimes practice will end at 7:00p.m.
Linke was asked to describe a typical practice involving drills. Linke said most Mondays,
Wednesdays and Thursdays they are in what is called "Thud" (helmets, shoulder pads and thigh
pads). He said the lineman will separate from the backs and receivers and lineman will go
through ladders, boards and walk through some plays. He said they will do this while backs and
receivers will do Noose and Pat & Go. Then they will come together and go through pass game
and run game. Then they will break off into defense and go through team tackle and team
takeaway. Linke said they would do one or the other, for example team tackle on Tuesday and
team takeaway on Wednesday. Linke described how team takeaway works on stripping the ball,
tip drill and knuckles, where they have to scoop up the ball.

Interview with Spencer Linke
Linke then described the drills for team tackle. Linke said they were doing a drill called
"bowling balls." He described the drill as one person starting off on one knee and throwing what
they call bowling balls by bringing their arms up and behind the ball carrier and working on
wrapping up (about a foot apart). Another drill is a tackle and a defender and a ball carrier. The
tackle has to start off with contact, look to see where the ball carrier is and make the tackle.
Linke said the team got a large black mat last year used for another tackling drill. They would
have a player stand in front of the mat and they were supposed to hit the numbered area with
their shoulder pads (term bowling balls is again used in this drill) and described the drill to be
five yards apart. Because they had just started using the mat last year, Linke was asked if they
had done the same drill before the mat and he stated no. Linke said all of this was a new concept
starting just last year.
Linke was asked to describe what occurred Tuesday, September 10. Linke said they (Penta kids)
were told "thud" gear when they got there. Linke said they started a normal practice until
told everyone to go in and change into full pads and that they had eight minutes to do it.
Linke described how he was approximately five yards away from the field when he heard
say time was up. Linke was the first person separated into the late group. Linke said he
thought there were only eight people who had made it to the field on time. Linke said the people
who made it out on time were called "New Woodmore" and the people who did not make it out
on time were called "Old Woodmore" and they were lined up across from each other. Linke
stated the coaches said that you cannot defend yourself and the New Woodmore is going to hit
the Old Woodmore. After Linke got hit (once) he waited for three to four other people to get hit
and then a group of them (players who were hit) ran to the corn. Linke said he was one of the
slower guys so by the time he returned, there were only two to three people left to get hit. Linke
was asked if he knew exactly who was giving the instructions and he said it was
Linke further stated all of the other coaches were there as well. He said was the one who
told them to take off running after getting hit. Linke said
and were all present during the whole thing. Linke was asked if they were
told to get into a football stance and he said no. Linke said they were five to ten yards apart prior

down. Linke said he was the first person who was hit and he was hit by Eric Greulich. Linke

does not recall playing during that time. Linke said he was about five feet away from

sick or injured. Linke was asked if he saw any good hits during practice. He thought the hardest
hits in practice that day were during the time they came out of the locker room from changing
into their full gear. When asked who was involved in the hard hits, Linke could not remember
specifically but just thought they were hard hits. Linke did not know of any other person who
was sick or injured as a result of practice that day. Linke did not see before practice due
to him arriving from Penta.

On 9/20/2013, at approximately 1430 hours, Special Agent Najmulski interviewed
( for Woodmore High School. The reason for the interview was
to ascertain what his trainer responsibilities are and his assessment of condition the day
of the alleged incident. The interview took place at the high school in an administration office.
was advised he was not under arrest and could leave at any time. He was told he could
quit answering questions at any time. He was also told the door was shut for privacy but he could
open it at any time. The interview was audibly recorded (exhibit 5). The following is a summary
of the interview.
DETAILS:
advised he came to work at Woodmore High School around 1500 hours on September 10,
2013. He had been in contact with the athletic director about the players getting water breaks
every 5-l 0 minutes or so because of the heat. It was understanding that the athletic
director was going to talk to the coaches about this.
said he is typically in the athletic training room every day from approximately 1445-1600
hours. He said he worked the girls volleyball game on this day and thought came to the
training room to be seen around 1800 hours. This is when the junior varisty volleyball game
started. said the game was being played in the gym connected to the training room. He said
this was the first time he knew of being injured.
came into the athletic training room with a couple of players. One of the players came to
the gym and told that had a concussion. could only remember one kid that
came in with Mason Beachler. said he went to the athletic training room and
was already changed out of his shoulder pads. said that is when he began his
evaluation process. said was covering his eyes. He was lying down when first
got there but then sat up.

Interview with
Najmulski asked what his assessment consisted of. said he has a set of questions to ask such
as symptoms or signs to check, like his pupils and balance. said he checked
bilateral grip strength by having him squeeze hand. He said pupils were fine, as
well as his balance. said had typical signs of a concussion; dizziness, light headed
and eyes sensitive to the light. said he felt sick. said he tested with a one leg
stand and he "wasn't much outside the norm for that."
He said was "pretty much walking" when he saw him. said when he walked with
to the car was walking on his "own power." said had no instability.
added, "It was just a little bit slower."
Najmulski asked what primary role would be in a situation like this, since he is not a
doctor. He said he is the primary first care responder. He said he determines if the player needs
further medical treatment. said typically for a concussion he has to decide if the player is
getting worse. said at the time, was walking and talking and fully aware of
everything. was able to give a narrative of what happened and knew that he had been hit
in the head. He told he went "head to head" with another player. said never
told him about the drills.
said he knew enough with the tests he performed on that he would not being going
back into practice, however practice was over by the time was done with
assessment. said he called mother and told her was showing signs of a
concussion. He told her to make sure someone keeps an eye on him all evening and all night. He
said the first 24 hours are most important to determine if things are getting worse, in which case
would need to seek emergency medical care. He told her what he was seeing and what
was telling him. said he did not make the recommendation for medical care on the
first phone call, however he did suggest it on the second call.
said there were 3-4 minutes between each of the phone calls to mother. said
the first phone call lasted at least 10 minutes. After the first call, was talking with
outside when brother, came out and told was not getting up
and not listening to him. said he went back into the training room and instructed to
get up so he could leave with his brother. He said got up slowly and walked out
with him. reiterated he did not have to support walking. said he was walking a
little slower than before, which is why he made the recommendation for him to go to the
emergency room (ER).
After got into the car, called s mother a second time. He told her to take
to the ER and updated her on what he was seeing. was not clear when he may have
spoken to the family for an update on his treatment. At first he said it was a day or two after, and
then he said it was that night by phone.
Contrary to what two other players told Najmulski, said he never went out to the field to
check on said he was at the volleyball game. He was first notified of
injury when he came to the trainer's room.
Najmulski asked about education.
He is certified by the National Athletic Trainer Association. He

said he did six clinical rotations with different sports. said he is hired for five days a week
for after school coverage. He said he is also hired per event such as the volleyball game. He said
he is employed by Fremont Memorial Hospital who contracts with Woodmore High School.
said he also works at Gibsonburg High School.
said none of the coaches talked to him about how was injured.
Najmulski asked how came to the conclusion to not call an ambulance. said, "The kid
was walking and talking." He said he was perfectly conscious. said from what he heard,
was not in that good of condition when he arrived at the ER. He said initially he was just
going to send him home, but was walking and moving pretty slow so he just wanted to be
safe and recommended medical care.

DETAILS:
Dennis advised he plays football for the Woodmore High School football team. He said he was
at practice on September 10, 2013. Dennis said practice started around 1515 hours and lasted
until 1830 hours. He remembered it being really hot that day, but felt the team was given ample
water breaks.
Dennis said when said to pad up he took it as meaning they were going to "amp"
up practice because they were playing a tough team on Friday. He said there had been a
discussion on how good the other team was. After the players entered the field with full pads,
Dennis said the slower players were told by coaches they were getting tackled as punishment for
being late. Dennis said he could see the coaches were getting mad. He did not feel that the drill
was to hurt anyone, but a way to make the players know they had to run and not walk out to the
field.
Dennis was told to tackle the other players since he was one of the players to get to the field on
time. He said he and some of the "New Woodmore" players had gotten together and decided not
to "hard core tackle" the "Old Woodmore" players. He said they felt like "dicks" for having to
tackle them. Dennis said the "Old Woodmore" players were told to line up and not fight back.
He never heard them being told to put their hands behind their back, however some players did
so on their own.

Interview Dennis Ratz
Najmulski asked why the "New Woodmore" players felt like "dicks" for tackling the other
players. Dennis said he thought the drill was too "grand" of a punishment. He thought the
coaches could have made a different punishment. He said he did not feel they should have been
tackled to the ground. N ajmulski pointed out the guys tackle each other all the time and asked
what made this different? Dennis was hesitant when answering, but said he was worried about
the other players mentally.
Dennis thought the distance between the two lines was seven yards, but it may have been off a
few feet for each player. He did not see anyone get injured with this particular drill. Dennis saw
come out during the scout drill and saw him holding his head and lay down. Dennis said
he saw the trainer come over and get He did not hear the conversation. Dennis said he
did not pay attention to what happened to after that. He did not know if stayed
out on the field or went inside.

Williams to ascertain what he witnessed at Woodmore High School's football practice on
September 10, 2013 when was injured. The interview was audibly recorded
(Exhibit 5). The following is a summary of the interview.
DETAILS:
Dillon advised he plays football for the Woodmore High School football team. He said he was at
practice on September 10, 2013. Dillon said practice started around 1515 hours and lasted until
1830 hours. Dillon said the uniform of the day was shorts, helmet, and girdle. He said half way
through practice the coaches did not seem too happy and they were told to change into practice
pants with pads. Dillon said he was with the linemen, but from what he understood the receivers
were not practicing hard so everyone had to change into pads.
Dillon said he was one of the slower kids to get dressed and back to the field.
and told him to stand at the line considered "Old Woodmore" and the kids out on
time were "New Woodmore." New Woodmore was instructed to run and tackle Old Woodmore.
Dillon thought the two lines were approximately 10 yards apart. They then had to run to the
cornfield. Dillon did not hear or see anyone that was injured.
After they took a water break they continued to practice. said they were not
physical enough so they were told to do hitting drills. Dillon described the hitting drill as
everyone is in a circle and "anything goes." He said one player has the ball and other players
take it from him. He said it is not the rules of football and it is known as "the pit." Dillon did not
see anyone get injured during this drill.
Dillon saw come out of practice half way through a drill called "scout team." was
playing wide receiver and exited the field to lay down. Dillon thought was dehydrated

Dillon said after the Old Woodmore/New Woodmore drill he had a conversation with
told him he thought that particular drill was "rough." Dillon said he did not remember
saying he was sick or his head hurt, but not long after their conversation Dillon thought

Parker advised he plays football for the Woodmore High School football team. He said he was

going through the motions. He said the coaches had enough and told them to go

getting dressed. When he got back out to the field he said he saw two lines. No one was saying
anything and he got into the long line. He said he did not know what was going on. He said
started yelling and pushing kids into each other and trying to get them to hit the
other kids. Parker said the kids tackling were "New Woodmore" and the kids getting tackled
were "Old Woodmore." Parker said he thought the lines started out about 15 yards between each
other but towards the end of the line the tacklers got slower and the distance became closer.
Parker did not see anyone get hurt from the drill. The players ran to the cornfield, got water and
then did another drill. He described the drill as a circle. There was someone running the ball and
another blocking. He said "anything goes." He said they usually do it for fun.

Parker came out did not go in. Parker said had said earlier he did not feel well at
school so Parker dismissed him not wanting to go in because of that or dehydration. Parker said

and his legal counsel, Attorney Nathaniel Spitler. The purpose
of meeting with was to discuss allegations of physical abuse that occurred on
September 10, 2013, at Woodmore High School. The meeting took place in Attorney Spitler's
law office, located in Bowling Green, Ohio.
DETAILS:
On Thursday, October 3, 2013, SA Ryan Emahiser met with Woodmore High School
and his legal counsel, Attorney Nathaniel Spitler. The purpose of
meeting with was to discuss allegations of physical abuse that occurred on September
10, 2013, at Woodmore High School. The meeting took place in Attorney Spitler's law office,
located in Bowling Green, Ohio. SA Emahiser advised as to why BCI was
investigating the allegations and he advised that he understood. While speaking with
SA Emahiser advised he was free to leave at any time. The meeting was in the
presence of 's attorney and took place in a closed meeting room at the direction of
Attorney Spitler.
SA Emahiser advised and his attorney that the interview would be digitally recorded,
and that the recording would be added to the report file. (See Exhibit #1) SA Emahiser told
that at this point in the investigation agents have spoken with the majority of all
parties involved, and asked that he be honest about the events that occurred on
September 10,2013. After SA Emahiser advised ofthis recording, the interview
commenced.
SA Emahiser asked ifhe was present at the Woodmore High School football
practice on September 10,2013, and advised that he was. stated that he
arrived after work at the practice at approximately 4:15 p.m. advised that he

arrived around 4:10 p.m. and he noticed that there were no players out on the practice field,
which he said he found unusual. stated that he then spoke to
who told him that practice was not going well and that players were not giving a
lot of effort. said that made the players go into the locker room and change
from just shoulder pads, helmet, and girdle and put on their full pads. said that
asked him to change the pace of practice and set the tone for practice. said that he
asked what he wanted him to do and said something physical. said that
he told that they would do bowling balls, a tackling drill consisting of one player
wrapping up another player and tackling them to the ground. said that said that
they were way behind on practice and indicated that they did not have a lot of time.
said the team normally conducts drills in a circuit and he normally conducts the
bowling ball drill. stated that to save time they decided to conduct the bowling ball
drill with all of the players in one instance, place the players across from each other and do the
drill all at once. said that they did not indicate to the players right away what the drill
was, but he later told them it was bowling balls. said as the players came out he was
hustling them to hurry and placed them into two lines. said that once the players were
in their lines he told those that were on time that they were the "new Woodmore." He told the
other players that came out late that they were not prepared for practice, being selfish and they
were acting like "old Woodmore." said that he distinguished the two lines as "new
Woodmore," the players that were on time, and "old Woodmore," the players that were late.
said that the "new Woodmore" players he designated as the tacklers, and the "old
Woodmore" players as the tacklees. stated that he was talking to all the kids and he
was not separating the kids into new and old Woodmore.
SA Emahiser asked specifically if the players that were late were placed into the "old
W oodmore" tacklee line, and the players that were on time placed into the "new Woodmore"
tackler line. said that he did not recall separating the players into two groups.
stated that he did not recall exactly what his instructions were but that he said, "Ready,
hit bowling balls." said that intentions were to go two or three times for each line of
players, allowing each line to be tacklers. stated that this did not happen the way it
was intended because indicated to move on to the next practice segment.
said it was not his intention to only allow one set of players to tackle, because that is
not how that drill is performed by design. said that his intention when he started the
drill was for players to run two to three times, and for players to tackle two to three times.
advised that everything happened very fast and that he told players to run to the
cornfield for being late. said that he recalled when called for the next
drill, only one line of players tackled, while the other line only ran to the cornfield after being
tackled.
SA Emahiser asked if the line that only received the tackles was the line that arrived
back from changing late, responded, "I don't know, I can't say for sure." SA Emahiser
asked if the players that were doing the hitting were the players that arrived from
changing on time, and he stated that he was also unsure of that as well, but probably. When
asked how many players were in each group, late and on time, said that he was unsure
due to the players being grouped all together. said that the drill only lasted for about a
minute, and that there were players mismatched by size so he was moving players around
matching them up with another player of similar stature. stated that he had just started

putting players with other players of similar size and the drill was over. SA Emahiser asked
again if he gave any specific instructions prior to the exercise and he stated that he
only told the players, "bowling balls." said that he probably said that the line closest
to him consisted of the tacklees and the line farthest away consisted of the tacklers.
said there were no specific instructions other than telling the players, "bowling balls."
advised SA Emahiser that his intentions were for each line to tackle several times,
however he believed that some players tackled several times and others only once.
said he was unsure if all of the players were present from changing. advised that at no
time were any players instructed to put their hands behind their back, nor to just take a tackle.
estimated the two lines were only three yards apart during the drill. said
that the drill he ran was not a very organized drill, so it was difficult for him to see how many
players were in each group. SA Emahiser asked if he ever put his hands on any
players while running the drill and said that the only time he touched players was to
match them up with players of similar size. said he wanted to ensure good technique,
as well as prevent larger players from tackling smaller players.
advised SA Emahiser that the purpose of the drill was to change the tone of practice
and that they used a common drill to do that, but he recalls thinking after the drill that it was not
very physical. said that he did not observe anyone taking any serious hits during the
drill he ran, and again stated that he remembered thinking that the drill was not very physical.
also said that he never saw anyone appearing ill or injured afterwards. SA Emahiser
asked what other coaches were in the general area when he was running the drill and
he advised and
stated that the other coaches were within viewing area of the drill he was running.
said that the drill he ran was the same bowling balls drill they normally do, except for
the players were doing the drill as one large group instead of a small group at a tackling station.
When SA Emahiser asked if he saw any players hitting harder, or outside the normal
lines of the tackling drill, he advised that he did not, but that he cannot watch them all.
said that normally the drill is conducted within a small group as a circuit drill and a
large pad is used behind the player being tackled. said that the players normally line
up in single file and tackle each other, and the tacklee is tackled into the pad on the ground.
SA Emahiser asked what drill the players moved onto next and he advised the "bull in
the ring" drill. advised that another coach ran the bull in the ring drill, and that while
that was drill was being run he was off to the side where the other coaches were.
advised that he was unsure who ran the bull in the ring drill. said that he did not
witness any players that appeared to be ill or injured after the bull in the ring drill.
said that he does not normally run that drill.
SA Emahiser asked if he at any point noticed at practice, and
said that he noticed later in practice when they were running a team
offense/defense drill where they run normal plays. said that he did observe
playing a defensive position when he made a good tackle on another player. said that
the only other time he saw was when was sitting down on the ground behind
and that was talking to said that he did not
recall had his helmet on or off, but he was sure that it was said that

was leaning down talking to and that was the last time that he noticed
during practice. said that the only other time he saw was after practice when
he saw lying on a table in the trainer's room without his helmet. said he
tapped on the leg and asked how he was doing, and responded with a facial
expression and they made eye contact. said that had an ice pack next to his
head, but nothing on his head. said that after seeing someone in the coaches'
room told him that was not feeling well.
SA Emahiser asked if there was ever any discussion in regard to cancelling practice
that day due to the excessive heat, and he advised that he did not hear any discussions on that
topic.
SA Emahiser asked if he had any questions prior to concluding the interview and
advised that he did not. At that time the interview was ended.

advised Agents that on September 10, 2013, she received a phone call from the
Woodmore athletic trainer at 6:32p.m. advising her that her son was showing signs of a
concussion. said that the trainer told his wife what signs to look for in regard
to the possible concussion. The trainer advised that was on his way home at that
time with his brother, advised that at 6:49 p.m.,
received a second call from the trainer and that the trainer advised her that and
would be arriving home and that they needed to take to the emergency room right away.
advised that brought home and that upon arriving at their home
came into the house. said he asked where was and
told him that was in the car, not moving or talking. Mr. said that he and his
wife then went out to the car, and supported head as they started to leave for the
emergency room. Mr. said that as they started to pull out of the driveway,
pulled into their driveway. Mr. said that told them that
"got hit a couple times hard today in practice." Mr. said that they left their residence for

the St. Charles Emergency Room. Mr. said that during the drive to St. Charles,
was foaming at the mouth, his eyes were rolling back in his head and he was unresponsive. Mr.
said on the drive appeared to "come to" a little but when he would speak it did
not make much sense, and at one point said that he could not move his arms and legs.
Mr. said that upon arriving at the St. Charles Emergency Room, the hospital's staff
assistance was required to remove from their vehicle and place him into a wheel chair.
The advised agents that they were unsure what to tell the hospital staff about what had
happened to because they did not know. They only had been told that he could have
possibly suffered a concussion. SA Emahiser advised and that he had
received the medical records from St. Charles and St. Vincent's Hospitals from Betsy Gordon of
Ottawa County Job and Family Services, which were being added to this report. The medical
records are attached to the administrative portion of this report, attached to the report from Social
Worker Betsy Gordon.
SA Emahiser asked the if had been ill prior to practice on September 10, and
they advised that he was diagnosed with a sinus infection. The said that had
been prescribed a Z-Pac for his infection and that he had taken the second dose on the morning
of September 10.
advised that he had spoken with one friends who plays football
as well, Tyler Mulligan. Mr. said that Tyler told them that the athletic director came out
to the football practice and told the coaches to cancel practice because of the high heat index that
day. Mr. said that he spoke with and Athletic Director Steve Barr, and
they told him that practice ended at 5:00p.m., which Mr. said was inconsistent with the
time arrived home.
The said that they had concerns of a particular drill during football practice. Mr.
advised that he had been told by his son, and friends of his sons, that the
players were told to go in and change their uniforms into full uniform and pads. Mr.
said that some of the players who were late were punished with a tackling drill and that he had
heard that was tackled defenseless. Mr. went on to say that neither his son,
nor any of his sons' friends saw actually get tackled defenselessly. Mr.
stated that they had seen helmet and that there were scratches and a piece now
missing out of the faceguard that were not on the helmet prior to practice on September 10. Mr.
said that they retrieved helmet from the school at the direction of Social
Worker Betsy Gordon, and the helmet was later taken by the Elmore Police Department. Mr.
said that he was unaware as to when could have received his injuries, and that
even after talking to other players that were at practice they could not determine when
was injured.
SA Emahiser advised the that agents would be speaking with all players and coaches of
the football team to get their individual account of the events that occurred on September 10,
2013, during the football practice. SA Emahiser told the if they had any further
information related to September 10, or any concerns to contact him at any time.

TITLE: INTERVIEW WITH ZACH SANDWISCH
SUMMARY:
On September 30, 2013, Special Agent Matthew Collins conducted an interview with Zach
Sandwisch at Woodmore High School in Elmore, Ohio. Present during the interview was Zach's
father, Mark Sandwisch. Zach Sandwisch is a sophomore on the Woodmore High School
football team. Sandwisch plays the position oflinebacker and wide receiver. During the
interview Sandwisch was asked if he had been present and participated in practice on Tuesday,
September 10, 2013. He was asked about normal practice drills and drills conducted on
September 10,2013. Sandwich was asked ifhe had witnessed anyone become injured or sick
that day. He was also asked ifthere were any hits that day which stuck out in his mind.
Sandwisch was present at practice, however was not participating due to a prior injury.

On September 30, 2013, Special Agent Matthew Collins conducted an interview with Zach

does remember them getting one after every drill.
Sandwisch was asked to describe the drills on a typical day. First he stated they begin in
NOOSE. This is where they go at a fast walk and work on planting their feet in order to change
direction. Then they would go to Pat & Go. He described Pat & Go by the receiver working on
getting off the ball and getting past a defender. After Pat & Go they would do "Quick Game."
Sandwisch described this as short quick routes receiving the ball from the quarterback without a
defender. After Quick Game, they would go to "Run Game." Run Game is just going through
running plays, again with no defenders. From there they would go to a water break. Then they
would go to Team Defense. He said sometimes they would go to Team Offense and walk
through offensive plays at about 75 percent speed before going to Team Defense.

Interview with Zach Sandwisch
Sandwisch then described the defensive drills. He described how they would break off into
groups and rotate to different stations where these drills were conducted. One drill he described
there being a large black pad. Players were to stand five yards apart while one player stood like
a scarecrow and the other player tackled him onto the pad. The other drill consisted of a player
on one knee, spaced about one yard apart. The player on one knee would stand up and wrap up
the other player and they would both fall onto the pad. After Team Tackle they would go to
Team Takeaways. Team Takeaways consisted of tip drill and strip drill. Sandwisch says other
than stripping, there is no other contact during Team Takeaway. From there they would go to
Team Defense.
SA Collins asked Sandwisch to describe the practice and events on Tuesday, September 10.
Sandwisch stated the practice started with players wearing what they called "Thud" gear
(helmets, shoulder pads, thigh pads with shorts over top). Sandwich stated they got through most
of the drills and were getting ready for team. Coaches were not happy with the receiver's
performances and said they were running routes at about half speed. said
everyone had eight minutes to get changed into full gear. Sandwisch followed them up to the
locker room in the golf cart. Sandwisch said he told them when they had six minutes left. Some
of the players said they did not care because they did not think the coaches were really timing
them. Sandwisch said he drove back out to the field and waited for all of them. Sandwisch said
there were people showing up in just less than eight minutes. He described there was a gap
between the people who made it and those who did not. told "You
have this." Sandwisch said walked over to the field where the seventh and eighth
graders practice and the other coaches were huddled in a circle by the water talking.
had "New Woodmore" in one line (kids that did make it on time) and "Old Woodmore" (kids
who did not make it on time) in another.
Old Woodmore was told they were "gonna stand there and get their asses whooped and they
were not gonna do anything about it." Sandwisch said New Woodmore was told to, "Just plow
them and hit them as hard as you can." Sandwisch described that they were about eight yards
away from each other. Sandwisch described that Old Woodmore would only get hit once but
due to there being a smaller number ofNew Woodmore, the "hitters" had to hit multiple times.
So once they hit someone they had to get up and hit another and just keep going on down the
line.
As for the Old Woodmore kids, once they got hit they were to run to the cornfield and back.
Sandwisch described that not everyone was able to hear instructions due to not being out on
time. Some of the players who were really late and did not hear instructions, fought back when
they got hit because they did not know they were supposed to just stand there. For instance,
Tony Rozzi, Todd Williams and one other kid got hit by New Woodmore and fought back and
came out and got in Tony's face and said, "Why you fighting back all you are is a
pussy and you can't do anything about it, you're supposed to just stand there now go to the
field!" Sandwisch said always yells on Tuesdays like that.
Sandwisch said, "After that so-called drill we went to Oklahoma drill." Sandwisch describes
Oklahoma drill as being in a big circle and having a down lineman, a linebacker and a running
back. Sandwisch said he remembers being in that drill three times, once at running back

and twice at linebacker. Sandwisch said there were no big hits during this drill. Sandwisch said
he remembers moving around pretty good and did not realize he was hurt at that point.
After that they went to Team Offense. was covering Maliki Brown (WR).
started puking and Henry Flores asked him ifhe was alright and said he did not feel so
good. came out and sat on the sidelines. Sandwisch went over to who was on
the ground and asked him if he was ok and he said he was light headed. Sandwisch said
had mumbled this. Sandwisch said he helped take off his helmet and shoulder pads.
Sandwisch said was moving his head and arms around at this time so he did not think he
got hurt, he just thought got hit. Sandwisch said he did not see get hit at any time
during "Team" (SA Collins took this as referring to Team Offense, the drill currently being
discussed). Sandwisch said he did hit someone but he did not get hit. Sandwisch said
was on the field when he was puking. He said Maliki and Flores were right there as well.
Sandwisch was asked if anyone had told the coaches and Sandwisch said yes. Sandwisch said
came over and said, " , are you alright?" Sandwisch stated said, "Yeah,
I'm just light headed, I'm fine." Then said, "Hey why don't we take you back
to the locker room, let you cool down and let look at you." Sandwisch said he and Chase
Kuchenriter helped up but pretty much got up on his own (Kuchenriter just held
out a hand to assist and pulled himself up). Sandwisch said was walking like he
was dizzy but he was fine. Sandwisch said once they got to the locker room, Kuchenriter walked
with the rest of the way and Sandwisch went back to the field. Sandwisch said
did not complain of his neck or head hurting but just said he was dizzy. Sandwisch demonstrated
how was moving his head around, which appeared to SA Collins like he was trying to
stretch out his neck but Sandwisch felt it was more like he was trying to see where people were
at.
Sandwisch was asked if he had seen all the hits during New Woodmore/Old Woodmore and he
stated yes. He was then asked if any of the hits from New Woodmore/Old Woodmore stood out
in his mind. Sandwisch immediately said yes, hit Parker Lowe and Parker Lowe is
5' 3" and he got hit pretty hard. Dustin Haar had two to three good hits but he did not see who
Haar had hit. While we were talking about any hard hits from that day Sandwisch said
was on defense during Team Offense and he hits really hard. He said you would not think it
because is only 135 pounds, but he throws his whole body into it when he hits.
Sandwisch said hit one of the receivers on a play but does not know who it was.
Sandwisch listed the players who he believed it could have been; Maliki Brown,
Evan Ulinski or Brad Bringman.
Sandwisch was asked if he had seen earlier that day and he said yes, because he has
classes with him. Sandwisch said seemed to be fine until he saw him lying down
towards the end of practice during Team "0." Sandwisch also stated he did not see the trainer
( there because he was probably at a volleyball game.

On September 30, 2013, Special Agent Matthew Collins conducted an interview with Brandon
Cowell at Woodmore High School in Elmore, Ohio. Cowell was asked ifhe had been present at
practice on Tuesday, September 10, 2013, which he stated yes. Cowell was asked how long
practice was on that day. He stated it started around 3:15p.m. and lasted until6:30 p.m. When
asked how many water breaks they had that day Cowell stated, plenty. Cowell stated they would
get water breaks after every drill. He further stated the drills would last approximately 5-10
minutes.
Cowell was asked to describe the type of drills and sessions at a normal practice. Cowell stated
after stretching they would start off with "NOOSE." Cowell described Noose (for his position
WR) where they would work on footwork, agility and releases. After Noose they would do PAT
& GO. Cowell described Pat & Go as receivers trying to get off the ball when closely guarded
by a defender. He further described this as swatting the defenders hands/arms away in order to
get open. After Pat & Go, they would do Individual Offense (staying divided by positions). This
is where the receivers would again work on footwork in addition to catching and route running.
Next they would do 7 on 7 (QB, WR and RB vs. LB, CB and S). Then they would go into Team
Offense (if in full pads it was full-go with first team Offense vs. Scout team Defense). After

Interview with Brandon Cowell
Team Offense they would break up and do Individual Defense (breaking up into positions).
After Individual Defense they would go into 7 on 7 again then into Team Defense (first team
Defense vs. Scout Offense). Cowell was asked ifthere were tackling drills done and he stated,
yes. Cowell said they would also do Team Tackle and Team Takeaways depending on the day.
Cowell was asked how the day went on Tuesday, September 10,2013. Cowell said they started
off with NOOSE, the normal offense procedure. They got up to 7 on 7 and receivers were
having a bad practice. Cowell said it was a bad practice all the way around at this point. He
stated it was hot out and everyone was grumpy and they still had to practice. Cowell said
sent them in to get full pads on and they had eight minutes to do it. Cowell was asked
how he did on the time. Cowell said he was late. SA Collins asked him if there was a term used
for those who were late. Cowell stated it was "Old Woodmore." Cowell was also asked ifthere
was a meaning behind it, other than being late. Cowell said we have developed into a better
team and the "New Woodmore" would not be late, being late is not acceptable.
Cowell was asked what happened once he was out on the practice field. Cowell said there was a
group in a line who made it out on time (approximately six or seven of them). The late ones got
into a line across from them about five to seven yards away. SA Collins asked Cowell to
demonstrate how far away. SA Collins asked what the instructions were at that point. Cowell
said gave all the instructions while watched. Cowell said they
were told to take the hit, "but I mean not as in hands behind your back." Cowell continued by
stating you were allowed to take a step back or put your hands up to defend yourself. SA Collins
asked if this is what told them and Cowell said no, but it was a "given" with anything.
Cowell was asked if they had done this drill before and he did not say yes or no, but instead
described how they would do something similar, but they would have a large pad behind the
person being tackled (called form tackling). told "Old Woodmore" to stand
there, take the hit and run to the com. Cowell was asked what instructions were given to the
"New Woodmore," or people delivering the hit. Cowell said they were just told, "hit em."
Cowell was asked if he remembered who hit him and he said yes, he remembered it was Evan
Sandwisch because he was glad it was one of the smaller guys. Cowell went on to describe that
the people doing the hitting were not going all out. He said it was not soft either. Cowell said
you would get hit harder in Team Defense when you catch a hitch pass (as a scout offense
receiver). Cowell said no hits stuck out in his mind. He said nobody had been taken up off their
feet and blown way back or out of proportion.
Cowell said after they ran to the com and back they went into 7 on 7 and continued practice as
normal. Cowell said they did not do any team tackling drills that day. Cowell was asked if there
were any hits that stuck out in his mind during Team Offense or Team Defense that day. Cowell
said, "Nothing more than normal."
Cowell was asked if he saw any player who appeared to be sick or injured that day. Cowell
said, "Besides no." Cowell said something about passing out so SA Collins
clarified with him by asking if he saw pass out and he said no. Cowell said he did not
see anything out of the ordinary until fell down. SA Collins again clarified with Cowell
and asked if he actually saw fall down and he stated no, because he (Cowell) was playing
offense at the time. Cowell said he looked over and was already lying on the ground.
Cowell said he heard Zach Sandwisch call for to come over and look at
Cowell said called for the trainer to come over immediately and he basically got taken

care of. Cowell said he could not remember if the trainer was already there or not, but said he
does remember the trainer being there at some point. Cowell said the trainer is always there
unless there is a volleyball game away or something. Cowell said the trainer put him on the golf
cart and took him inside.
Cowell said he later saw after practice lying on the trainer's table. Cowell said he had a
cloth over his head and he was not talking or anything. Cowell said he was not looking very
good. Cowell said he never heard him talk because he was not in there that long. Cowell said
prior to practice he heard was not feeling good that day but he did not notice anything out
of the usual and he seemed normal to him. Cowell said he never heard tell any of the
coaches he was not feeling good before practice.
