Dear Sirs,

Fuel Oil Splash Trouble of Auxiliary Boiler

Recently, a container vessel under MOL operation had suffered Oil Splash trouble of Aux. Boiler F.O. Pipe during berthing. Hereinafter, we would like you to inform the outline and the cause of this accident, and instruct countermeasure to prevent similar accident.

Outlines of Accident
The vessel berthed at Singapore at 17:00LT, and cargo work was continued through the night. The next morning at 06:00AM (One (1) hour before departure), ship staff found oil splash near Aux. Boiler. (Please see attached picture below) Fortunately, there is no fire and no one injured by oil leakage. Ship staff immediately stopped Aux. Boiler, but it took four (4) hours to clean up splashed fuel oil. (The estimated F.O. leakage volume was above 100 Ltrs) Besides, additional Tug Boat was needed for departure, due to insufficient electric power for using Bow Thruster without Turbo Generator.

Cause of Accident
According to the investigation after cleaning spilled oil, a copper pipe of F.O. Line pressure gauge was found broken and disconnected. In addition, ship staff had not been aware of the oil leakage until next morning, therefore a large amount of F.O. above 100 Ltrs were splashed off.

Copper pipe of F.O. Line Pressure Gauge were broken at root connection.

Countermeasure
1. The pipe material of FO Line for gauge shall be exchanged with steel if existing pipe material is copper. 2. Anti Splash Tape (equivalent to FN Tape) is so effective to prevent oil splash and fire accident. Please check again that Anti Splash Tape are applied to the connections of high pressure oil line. 3. Especially, when you carry out routine maintenance of Aux. Boiler Burner, please check the condition of the connection of F.O. pressure line, including above mentioned pressure gauge connection. 4. E/R ITV Monitor is very helpful to watch the important machineries which are possible to cause serious fire problem. During E/R unmanned operation time (M-zero), duty officers are requested to check the monitor periodically.
