GORDONS ASSESSMENT

BEFORE ADMISSION ON ADMISSION


Health Maintenance Management: - Mrs. CA was admitted last January 27, 2014 around 5:40 pm, ambulatory, due to hypogastric pain. She understands that the purpose of her treatment is to know what is the problem why she had hypogastric pain. She thinks that the treatment is of great help to her condition. According to her, na ospital lang ako nung nanganak ako, puro sila cesarean section as verbalized. She expects that this hospitalization will help her condition. Nutrition/Metabolic: Mrs. CAs appetite during admission changed, as verbalized hindi na ako kumakain, pakunti-kunti lang, naka liquid diet lang din kasi ako, pati ang pag-inom ko nga tubig kunti lang din.

Nutrition/Metabolic: Mrs. CAs appetite before was ok, according to her yung mga usual nating kinakain, isda, manok, gulay, prutas, at kanin yan lang naman ang halos kinakain ko as verbalized. She admits that they like to eat on restaurants. Had no difficulty in swallowing or eating. She doesnt have any dental problems as verbalized napabunot ko na lahat ng mga sira kung ipin, at wala din akong postiso. She drinks 7 glasses of water in a day. Elimination: She had a normal bowel elimination a year before she was diagnosed by her illness. dalawang beses sa isang araw ako mag bawas, isa sa umaga at isa naman sa gabi as verbalized. She also admit that she had a normal urinary elimination, & had not experience any difficulty. She had experience having skin rashes (bungang araw) which makes her sweat due to the hot weather.

Elimination: She had difficulty on her bowel elimination a year after she was diagnosed on her illness. As verbalized kung dati sa isang araw dalawang beses akong magbawas, ngayon yung pinaka matagal 10days bago ako makapagbawas. She admits that she experienced constipation and diarrhea most of the time. But, her urinary elimination was not affected, she can still urinate normally. She often

experience having skin rashes because of her condition which makes her stay most of the time in bed. Activity/Exercise: According to Mrs. CA, she spent most of her time watching Television, hanging out with her siblings and have their bonding together. She doesnt have a regular exercise as verbalized minsan lang ako nag-eexercise. She sometimes experienced chest pain, difficulty of breathing, and back pain dati nung mataba pa ako, sumasakit yung dibdib ko, yung likod ko, nahihirapan din ako minsan huminga as verbalized. She doesnt have any vices. Activity/Exercise: She spent most of her time in bed due to her condition. She just watch television and sleep. Does not experience any chest pain or difficulty of breathing, but she still experienced back pain because she always stays in bed. ang laki nang pagpayat ko ngayon, dati 60 plus yung kilo ko, ngayon 50 or 40 plus nalang as verbalized by her. (DAI, WALA BAYA SYA NATIMBANG NAMU ATONG 1ST DAY. PERO SA ON ADMISSION NIYA NGA TIMBANG KAY 51 SIYA.) Sleep/Rest: She had difficulty on sleeping when she was admitted for she felt uncomfortable due to her condition.

Sleep/Rest: Mrs. CA sleeps at around 10pm or 11pm and wakes up every day as a routine at 4:30am, and feels rested and ready for her daily activities. She had an insomnia, yan lage yung problema ko, di ako makatulog agad tuwing gabias (WA NAKOY MASUMPAY TINAY) verbalized.Had not done any interventions to help aid her problem. She admits that she take naps for 1 hour after lunch break.

Cognitive/Perceptual: As verbalized nagsusuot ako nang glasses yun bang nabibili lang, yung mga reading glasses lang. She uses her glasses when she reads for she is far sighted. She had no problems with regards to her hearing, had no discharges, pain or tinnitus felt. She admits that she feels back pain due to her cesarean section according to her, and states that it does not affect her daily activities. She also feels pain when defecating.

Cognitive/Perceptual: She doesnt feel any pain when defecating because she had watery stool with no particles on it. (WALA NAKOY MA LARLAR )

Self-perception/Self-esteem: Mrs. CA feels fine most of the time. She does not want to change anything from herself or her appearance for it is against the law of their belief.

Self-perception/Self-esteem: She feels weak but she can still manage to stand up and go to the bathroom to do her necessities. According to her if there would be anything she wants to change about herself it would be her illness. Her illness affected her a lot, for most of the time she felt pain and cannot do anything about it. She is anxious about her condition and fears that she might not live long enough to be with her family. She alleviates her feelings by her husband and siblings way of encouraging her. Role/Relationship: Role/Relationship: Mrs. CA lives with her husband. She Mrs. CAs family managed her illness by seeks help to her husband and nearby way of being with her and encouraging siblings when problems arise. Her her most of the time. family members doesnt depend on her. According to Mrs. CA, her status does not affect her relationship with her family, but instead it had established a stronger relationship between them. Sexuality: Sexuality: Mrs. CA had her first menstrual period at the age of 15 years old and does not (DAI? WHAT AKO IBUTANG HERE?) have any menstrual problems. She had 2 sons: eldest is 16 y/o and the youngest was 12 y/o. Mrs. CA admits that she performs BSE (breast selfexamination) a week after her menses and she had her last Pap test last 2013. She had no known reproductive infections. She doesnt take any contraceptives. Coping/Stress: Coping/Stress: Mrs. CA can cope with her stresses with The present condition of Mrs. CA the help of her husband and her causes her stress. It affects her relatives. There are no known stressors emotionally, physically. But with the that affected her before. She solves her presence of her husband together with problems most of the time with her her siblings and other relatives we have husband, together with whatever observed that she tries to overcome it. personal crisis that she will be having and admits that she can handle problems successfully. Values/Belief: Values/Belief: The important things to Mrs. CA is her Her condition doesnt affect her belief
