A. Activity: Feeding Program: Chicken Macaroni Soup with Hard Boiled Eggs B. What to submit: Parental Consent i. Deadline: 10 January 2014 ii. Note: NO Parental consent reply slip, NO trip. Transportation fee: Prepare Php150.00 (will be collected on the day of the trip) If applicable: Drop-off letter indicating reasons for drop-off, specific drop- off location, parents signature and parents photo copy of valid I.D. with signature. Possible route to the fieldwork venue is via Espaa-Q.Ave.-Commonwealth-UPC.P.Garcia-Katipunan-Marcos Hwy-Masinag-Cogeo-Venue o Note: You will not be allowed to alight the bus without the Drop-off letter with parents photo copy of valid I.D. with signature. C. What to bring: Food (packed snacks/lunch) and water for yourself. 3 Cooking knives and 3 chopping boards per group 1 dozen COOKED hard boiled eggs (per group) Camera for documentation D. What to wear: CWTS shirt (Please buy your NSTP shirt at TYK Rm. 202 on 28 Aug. PhP 90.00 only) Maong/jogging pants UST I.D. Working shoes/rubber shoes/sneakers X No sandals, flip-flops, shorts, slippers and too many accessories E. Reminders: Eat your breakfast. Be punctual. Come at least 15 minutes before the assembly time. Be in your best behavior. Behave in accordance with the code of conduct and discipline as contained in the Students Handbook. Grumbling, complaining or any improper attitude will not be tolerated. If you have medical conditions that needs consideration that please inform your facilitator supported by a medical certificate before the trip.

Always stay with your group. If theres a need for you to go somewhere, inform your Group Leader and Facilitator. Never leave the area without your Facilitators permission. Call the attention of your Facilitator or Group Leader regarding any difficulty encountered or untoward incident. Always be ready with your documentation. Each activity and visit is important. Do not be absent on the day of your trip. There will be NO special trip for the absentees. No smoking. All activities should be styro-free. Avoid displaying valuable items or wearing unnecessary jewelries. Enjoy and have fun. Be friendly and polite to everybody in the area and always smile.
