One of the most common
questions I receive from
seniors wanting to remain
at home is What is
the difference between
homecare and home
healthcare? Sure, they
sound a lot alike, but the
two are very different in
many ways. They are both
designed to help the sick,
elderly, or disabled but
their level of delivery and
their outcomes can differ.
Knowing the difference
between the two can help
you and your loved ones
determine what resources
are available to you, how the
services will be paid for, and
what you can expect from
each service.
The goal of homecare is
to make it possible for
people to remain at home
rather than use long-term
or institutional-based
nursing care. Homecare
agencies offer in home
assistance such as in
home aides, nursing care,
and sitter services for
extended periods of time.
Most patients that need
homecare services cannot
manage on their own and
need additional help in
the home that their family
cannot provide. Homecare
agencies can assist with
daily tasks such as bathing,
dressing, meal preparation,
medication reminders,
running errands, and light
house-keeping. This help
may be for long or short
periods and is completely
up to the patient to decide.
Homecare agencies charge
for their services by the hour
or accept long-term care
insurance, Medicaid, or VA
benets for payment. The
Veterans Administration
has an Aid and Attendance
benet plan for qualifying
homebound veterans.
Individuals with Medicaid
may qualify for home care
programs such as PCS
(Personal Care Services)
and CAP (Community
Alternative Program).
In contrast, Home
Healthcare agencies provide
nursing, physical therapy,
occupational therapy,
speech therapy, and social
work services through
Medicare and other types
of health insurance plans.
When a patient qualies for
nursing or therapy in their
home, he or she may also
receive the help of a home
health aide for non-medical
assistance like bathing
and dressing. However
these non-medical services
alone are not covered by
Medicare. Home Healthcare
is medically-focused care
which must be prescribed by
a physician. To qualify under
Medicare, a person must be
homebound, as dened very
specically by Medicare.
Medicare coverage is
triggered only by the need
for nursing or therapy.
The visits are intermittent
(meaning usually a few
times a week) and usually
last for one hour or less.
The duration is limited by
Medicare with most patients
achieving their home health
goals within a few weeks.
Regardless, whether you
need homecare or home
health care, the goal for the
two are the same - to help
you or your loved one live
more comfortably in the

WHAT IS THE DIFFERENCE BETWEEN
HOMECARE AND HOME HEALTHCARE?

Weight loss has
become a huge topic
with vast marketing
campaigns selling
many different ideas
and selling points.
This blog is an
attempt to simplify
the hype, clarify the
misinformation, and
return to common
sense. I am not a
medical professional
and do not claim to
be; the information
presented here is
intended to educate
and help us make
our own choices in
regards to our health.
The information
presented below is
what I have come
to understand after
15 years of learning
about health and my
own journey with
health, exercise and
weight.
1. Basic Attitude:
Eating to live or
living to eat? If
we are eating to live,
we eat the amount of
food that is needed
to sustain the body.
If we are living to eat,
we can eat and eat,
getting full at every
meal, and generally
consume much
more food than our
bodies need. Which
attitude do you live
by? This concept
shines light on trying
to lose weight by
following a diet,
versus establishing
a healthier lifestyle
pattern. Diets are
temporary and lead to
temporary weight loss
and gain. Lifestyle
changes shift our
patterns of eating and
living.
2. Food as Fuel:
Our amount of
activity in a day
determines how
much our bodies
need. In a car, the
engine uses as much
fuel as you allow into
it with the gas pedal.
If we are driving
on the interstate all
day, we will need
to refuel our cars a
couple times. If we
just drive across
town and back, we
likely wont need
to refuel that day at
all. Are our bodies
much different? If we
are very active one
day and burn 3,000
calories, then eating
3,000 calories is no
big deal. However,
if we are not very
active and eat 2,500
calories, but only
burn 500 that day,
guess where the extra
2,000 calories go?
Into our fuel tank!
This becomes added
weight when it is not
burned as fuel or
energy.
3. Our bodies
handle different
types of fuel in
different ways.
The three fuel types
are fats, carbs and
proteins. We will
also look at these
three types of fuel in
regards to their effect
on our blood sugar.
When the body is
active it uses more
sugar in the blood
to fuel the cells and
muscles of the body.
Fats: First off, fats
are not bad; in fact,
they can be good.
They have been given
a bad reputation
over and over again
with a belief that
they cause health
problems. Now,
there is a difference
between a healthy
fat (avocados and
sh oil, for example)
and unhealthy fats
(hydrogenated oils
like those in store-
bought donuts and
pastries) in regards
to our health. Fats
provide the steady
fuel source needed
to sustain the body;
when consumed, they
do not interfere with
the blood sugar level.
The body naturally
breaks down fat to
keep blood sugar at
healthy levels. This is
a slow process, so if
one is expending a lot
of energy in shorter

periods of time, carbs will
be needed to sustain blood
sugar levels.
Carbs: Our food has
become saturated with
carbohydrates. It is my
belief that this is a large part
of the weight gain problem
in our country. Carbs are
made out of sugar, or get
converted to sugar very
easily in our stomachs.
It doesnt matter if it is a
sugary drink or whole wheat
pasta; they go directly into
our blood sugar. If we eat
too many carbs without
enough activity, our blood
sugar goes up, and our
body has to produce more
insulin to bring blood sugar
levels back to a safe level
(Yes, blood sugar that is
too high is very dangerous
to our bodies). The excess
sugar gets turned into fat
that is held in our bodies
as future fuel. This cycle
of eating too many carbs,
blood sugar going up,
and production of excess
insulin is a large part of
eventual insulin resistance;
pre-diabetes and Type 2
diabetes that are plaguing
our country at large; as well
as weight gain.
Proteins: Proteins are
the building blocks of the
connective tissue in the
body, like the muscles. They
help regulate metabolism
and cell function. Proteins
have usually been the
shining star of diets; no
one usually nds anything
wrong with them. Like fats,
they do not increase blood
sugar immediately when
consumed, and they can
be broken down into sugar
when the body needs.
Some basic guidelines I
have come to follow as the
result of my understanding
of how our bodies work are:
1. Reduce excess
carbohydrate intake.
a) Drink water or
un-sweetened tea. Avoid
drinking sugary beverages;
our bodies do not need
them.
b) Skip the dessert; it is
full of excess carbs. Eating
dessert is about having a
pleasurable end to a meal.
Another pleasurable option
is to enjoy the meal in itself,
and enjoy the pleasure of a
healthier body.
c) Reducing carbs
may make you cranky and
crabby at rst. The body
is adjusting to burning a
new energy source, and
blood sugar levels may be
adjusting to this as well.
d) If you need a snack,
nd one that has a balance
of fats, carbs and protein.
For example, eating potato
chips or ice cream provides
limited nutrients but a high
amount of carbs, whereas
eating a healthy food bar
or a handful of almonds will
give you a nice balance of
energy to utilize.
2. Maintain
appropriate activity
levels that balance
exercise with your
food intake.
a) Great ways to start
exercising are walking for
one mile or more and going
to a yoga class.
3. Set a common-
sense goal.
a) Setting a goal for
weight loss may be what is
needed, but remember we
can also set goals for living
healthier and exercising
more, too. If setting a goal
just to lose weight doesnt
cut it, or it adds too much
pressure of failure, try a
different goal that may lead
you to a healthier you, and
maybe you will lose a few
pounds and feel great while
you are at it.

Learning that you or a loved
one has been diagnosed
with an advanced illness
is a daunting prospect.
Its also an opportunity to
gather critical facts, tap into
community resources, and
take advantage of a team of
professional care providers.
The Power of
Teamwork
Hospice, for example, is
a set of services that can
be provided at home, in a
community facility, or in a
hospice house wherever
the patient prefers to reside.
Hospice care takes a team
approach, offering patients
the expertise of doctors,
nurses, social workers,
chaplains, and nursing
assistants. This team of
medical professionals
coordinates care on an
individual basis, promoting
a patients quality of life at a
time when its needed most.
Home Is Where the
Heart Is
Most commonly, hospice
care takes place in a
patients home. The
comforts of a familiar
environment help with the
challenges of advanced
illness and make it easier
to coordinate caregiving
when its provided by
family and friends. Should
those circumstances
prove difcult to maintain,
volunteers can be assigned
to help with personal
support and household
tasks. Along with assisting,
volunteers also uplift
families with a loving
presence.
Everyone Needs a
Break
In the case of extended or
acute illness, the strains
associated with caring
for a patient can become
overwhelming. When that
happens, hospice can
arrange for intermittent
breaks for caregivers.
Respite care, as its called,
allows a patient to stay in a
community facility for a set
amount of time while the
caregiver rests and takes
care of necessary tasks. At
the end of the period, the
patient returns home again,
and everyone is better
equipped to continue on.

THE CONFUSION
OF PALLIATIVE CARE
If you think you know what
palliative care is about, plan
to be surprised:
What exactly does
palliative care mean?
The word palliative comes
from Latin and means to
soothe or to relieve. The
idea behind palliative care
is to give patients better
quality of life by relieving
them of pain and suffering.
Is palliative care the
same as hospice?
Palliative care has
traditionally been
associated with end-of-life
care and a six-month life
expectancy. But, in recent
years, its use and scope
have widened dramatically.
Nowadays, palliative care
serves patients with curable
illnesses assisting them
on the journey to recovery
and helps patients with
chronic illnesses as well,
allowing them to enjoy life
even in the face of long-
term health conditions.
How is palliative
care different from
palliative medicine?
Palliative care typically
involves a patients nal
months of illness while
palliative medicine is more
closely tied to treatments
for both short-term
recuperation and ongoing
illness. Palliative medicine,
in effect, is the mature
version of palliative care,
serving a more diverse
cross-section of patients
and assisting them with all
the nuances of symptom
management and complex

During the winter months, many
people experience symptoms of
upper respiratory infections such as
sore throats, fever, sinus pressure,
runny nose and cough. While
bacterial infections can cause these
symptoms, they are usually caused
by viruses.
Antibiotics do not treat infections
caused by viruses. Because of
this, antibiotics for sore throats and
bronchitis are appropriate only about
10 percent of the time.
The Centers for Disease Control
and Prevention (CDC) lists antibiotic
resistance as one of the most critical
public health threats. Overuse
of antibiotics can cause serious
infections such as Methicillin-
Resistant Staph Aureus (MRSA) and
Clostridium difcile (C.dif).
Bronchitis symptoms include cough,
low-grade fever, chest discomfort
and fatigue. The cough may start
out non-productive but later it will
be productive of mucus. Treatment
for viral upper respiratory infections
focuses on symptoms relief.
Frequent hand washing and coughing
into your sleeve will help prevent
viral upper respiratory infections from
spreading to others.

Bronchitis is inammation of
the airway mucus membranes.
About 90 percent of bronchitis is
viral and only about 10 percent
is bacterial. The CDC does not
recommend antibiotic treatment for
uncomplicated acute bronchitis.
A Harvard study shows that even
though it is proven that bronchitis
and sore throats are typically
caused by viruses, providers are
still prescribing antibiotics at a
rate of 73 percent and 60 percent
respectively.
Patients often demand antibiotics
for their upper respiratory
symptoms, said Bechtol. We try to
educate our patients but sometimes
the expectation for antibiotics is still
there.
Some insurance companies,
including Blue Cross/Blue Shield,
have gotten involved and have
started penalizing providers for
treating upper respiratory infections
such as bronchitis with antibiotics.
While I do not agree that insurance
companies should be making
medical decisions, I do understand
the push to restrict the overuse of
antibiotics, said Brian Bechtol,
physician assistant and CEO of
Urgent Care of Mountain View.
If you develop symptoms of
bronchitis or sore throat, you
should see your provider. Only he
or she will know if you truly need
antibiotics. He or she can talk to
you about treatment options. It
will take efforts from providers and
patients to stop inappropriate use of
antibiotics in the U.S.

Skin Health is determined
by a variety of factors that
include genetics, as well
as internal and external
factors that contribute to
the overall condition of our
skin. External factors like
pollution, weather, skin
care habits (or lack of them)
and misuse of products
can result in imbalances
in the oil to water ratio
that keeps skin at its
healthiest. Internal factors
like hormones; the foods,
chemicals or medications
we ingest; and factors like
stress and sleep are also
manifesting evidence in
our bodys largest organ,
the skin. Genetic factors
like oil production must be
managed and controlled,
while many skin conditions
can be corrected or
minimized with proper
skin care.
Many individuals struggle
with skin disorders
and conditions without
understanding how to best
care for their skin and the
changes it undergoes as
we age, as the seasons
change or as a result
of hormones, illness or
stress. Their perception
of their own skin may be
based solely on its current
appearance rather than on
a thorough skin analysis
to determine the skin type,
skin conditions, disorders,

rating. It can be confusing

I was working with
a number of different
manufacturers and retailers
in the specialty sock
market, among others,
and I noticed a real gap in
the options being made
available to the public,
Higgins says. On one
side, there were all manner
of compression socks and
stockings that doctors
could prescribe to help
people with circulation
issues, and on the other
side there were plenty
of big, bulky braces and
splints that could be used
to provide orthopedic
support. But many of the
same people needed both,
and they simply werent
willing or able to function
with the options available to
them.
In response to this
observation, Higgins went
to work developing a
concept that has evolved

It All Started With
Sore Feet
With over two million
Americans being treated
annually for plantar fasciitis,
this painful and debilitating
condition is one of the
leading causes of podiatric
consultation. While often
associated with runners, it
actually can affect anyone
who walks or stands on
their feet, which explains
why its so common.
The plantar fascia is a tough
band of tissue that connects
the heel bone to the toes,
stretching across the sole of
the foot and providing some
spring in each step. When
this ligament becomes
inamed due to overuse
or strain, it results in sharp
pain at the heel, along
the arch of the foot, and
sometimes into the lower
Achilles tendon.
My feet had become so
stiff and sore from plantar
fasciitis that I could barely
walk, recalls Rev. Lonny
McSherley It was very

There is a variety of medical
equipment specically
designed to help you
reduce your risk of falling.
Bathrooms tend to be a
problematic area because
they are generally small or
even cramped, frequently
have a hard surface oor
covering and often have
water or liquid spills placing
you at risk for falling.
Economical solutions for
decreasing your risks are
the installation of grab bars
in bathtub and showers or
near toilets. For assistance
with bathing, you could
choose a shower chair or
transfer bench which allows
for safer transfers in and
out of the bathtub. When
choosing one of these
options, you could also
add a hand-held shower
to allow you to adjust your
water direction to best meet
your needs. Another simple
tactic for reducing your risk
would be to add a non-skid
surface to the bottom of
your bathtub or shower.
This can be accomplished
through the use of non-skid
peel and stick tape and
pads, or the use of a non-
skid bath mat.
Another option for reducing
your chances for falling
when bathing would be to
consider the installation of
an accessible shower or
bathtub. The primary benet

Sometimes patients
succumb to a disease, in
spite of the best treatments
and support. When the
time comes for a family
to confront a loved ones
declining health, hospice
care can be a dignied
way to make that persons
nal days as comforting as
possible.
Hospice is a type of care
designed to make the nal
moments of a persons
life as pain-free and
manageable as possible.
It can take place in a
medical facility, but very
often hospice care occurs
at home or wherever
the ill person feels most
comfortable.
Hospice care is usually
the last step when all
other options have been
exhausted. Making the
decision to move a loved
one to hospice care can be
an emotionally wrenching
time. Decisions such as
these may be better made
at a time in a persons life
when he or she is not sick.
That is why living wills that
spell out details for end-
of-life care can be quite
valuable. Decisions are
made with a level head and
not wrought with emotions.

Hospice is a type of care and a
philosophy that focuses on the
palliative care of terminally ill patients.
Rather than providing medication
to try to treat the illness, medicine
is offered to make a person more
comfortable and remove any pain.
Psychological therapy may also be
offered to help the person come to
terms with the end of life. Although
hospice is a concept that has
gradually evolved since the 11th
century, the principles of modern
hospice care can trace their roots to
the 1950s and Dame Cicely Saunders,
a nurse and social worker who is
known as the creator of the hospice
movement.
If hospice care is outlined in a dying
persons living will or expressed
wishes, there are some guidelines that
can be followed by the sick individual.
Visit various medical care centers
to see if away-from-home options
provide the care and environment you
desire. Be sure to establish what type
of hospice care you prefer. This may
include care within a nursing home or
hospital, or in the comfort of your own
home.
Sign a medical durable power
of attorney. This signed document
gives authority to an adult age 18 or
older, who then has the right to make
necessary medical and healthcare
decisions for you in the event you
become incapacitated.
Clearly indicate your wishes
regarding resuscitation. A signed
document may alert emergency
healthcare personnel or others of your
wishes not to be resuscitated. This
document must be signed by you and
a doctor and witnessed by others.
Decide on the duration of care.
Hospice care generally lasts six
months. This enables a long stretch
of palliative care. Should you live
beyond the time period, hospice care
can continue. Studies have shown
that patients who receive hospice care
for at least 30 to 60 days gain greater
benet than those who are placed in
hospice only in their nal days.
Investigate nancial options for
hospice. Medicare and Medicaid
provide hospice coverage in 44 states.
Many private insurance plans have a
hospice benet as well. Understand
how payment is made to gure out if
private funding will be necessary.
Not all hospice programs are the
same. Certain core models of care are
followed by many hospice facilities,
but options vary. Hospice can provide
comfort in a persons nal days and
help sufferers better enjoy life and
time spent with loved ones in these
last days.

virtually all of that hard
work, from administering
medication to hanging
IVs to moving patients
and equipment, is done
standing up on hard
surfaces. So its no
surprise that many nurses
experience foot and back
pain after a long day -- or
night -- on the job.
Millions of people in
physically demanding
occupations experience
chronic back, hip and foot
pain. Repeated strain on
the feet, hips and back
not only causes pain but
also gradually reduces
the ability of muscles
to recuperate quickly.
As a result, healthcare
professionals such as
doctors, chiropractors and
massage therapists spend
countless hours treating
patients whose pain and
discomfort stem from job-
related injuries.
For nurses, these ailments
and injuries are difcult
to avoid. In fact, research
shows that nurses suffer
from lower back pain more
than people in any other
profession. The constant
physical demands of the
profession also prevent
nurses from getting enough
rest between shifts, which
can exacerbate both the
problem and the pain.
While treatments for foot,
hip and back pain can
be effective, nurses can
also take steps to prevent
strain in the rst place. The
following are a few proven
strategies for staying
healthy and injury-free on
the job.
Wear the right shoes.
Nurses need shoes
that t well and provide
proper support. Since it is
important to give shoes a
day to air out before being
worn again, its best to have
more than one pair of work
shoes. Full-time nurses
should replace their shoes
every six months.
Tend to your feet. Foot
problems, such as corns,
bunions and ingrown
toenails, are painful and can
sap ones energy, so treat
them early and often.
Stand up straight. Good
posture is one of the keys
to preventing both foot and
back pain. When the body
is in the correct position,
the vertebrae that form
the spine are aligned and
muscles and ligaments can
hold them in place without
strain. If necessary, various
types of posture supports
can help ensure that the
body is properly aligned
and supported properly.
Bend right. Always be
aware of how you move
your body. When lifting
equipment or moving
patients, give your back a
break. Instead, bend at your
knees and let your legs do
the work.
Think teamwork. If your
facility has a lift team, dont
hesitate to use it. If not,
work in pairs when lifting,
shifting or transferring
patients.
Turn, dont twist. Lifting
and twisting can strain the
lower back. When picking
up heavy equipment or
even a patient, plant your
feet rmly on the oor for
support. As you turn, be
sure to move both your feet
and your body.
Make a move. Holding
equipment or patients in
one place for a prolonged
period can put enormous
strain on the neck,
shoulders and back.
Whenever possible, ask
for help or relief. At the
very least, stretch out once
youre done.

Consider your total income
as well as your nancial
obligations (i.e., mortgage,
car payments, etc.) and
then shop around for the
cost of childcare. If it makes
more sense to become a
single-income household,
even if its just for a few
years, then you will have
another important decision
to make.
Determine who is staying
home. Couples who have
decided that its in their
best interests to become a
single-income household
once their child is born
must decide who will be
staying home and who will
continue working. Its easy
to say the parent who is
earning the most should
continue working, but that
decision is not so black
and white. Consider the
cost of healthcare offered
by each of your employers.
Some healthcare plans
are signicantly more
affordable than others, and
this might weigh heavily on
your decision, especially if
both parents are earning
relatively similar incomes.
Another thing to consider is
each of your opportunities
for advancement at your
current company and within
your eld. Earning potential
should factor heavily
into the decision as to
which parent will continue
working, especially if
you plan to have another
child down the road. The
conversation as to who will
continue working should be
treated delicately.
Discuss your housing
needs. Couples who are
expecting a child often
feel this is a great time to
abandon apartment living
and buy a home of their
own. But chances are you
can comfortably manage to
live in your apartment for a
few more years after your
child is born. Those extra
years of apartment living
can give you the chance to
save more money so you
can afford a nicer home
in a better neighborhood
that boasts better schools.
Or apartment living for a
few more years may give
you more time to save
and place a larger down
payment on your home
down the road. The larger
the initial down payment,
the lower your monthly
mortgage payment, so
it might be in your best
interest to squirrel away a
few more dollars before you
go house hunting.
Establish a nancial
safety net. Many nancial
advisors suggest both
singles and couples have
between three to six
months income saved in
case of emergency, such as
a layoff or an accident that
prevents you from working.
This is especially important
for expecting couples, who
will soon have an additional
mouth to feed. This safety
net should be big enough
so you can comfortably
pay all of your bills,
including those for housing,
transportation and food,
for three to six months.
If saving that amount of
money is not realistic, save
as much as possible and
continue to do so once your
child is born.
Purchase life insurance.
Life insurance is perhaps
the most important
purchase expectant couples
can make once they learn
a baby is on the way. Its
understandable if young
couples without children
have no life insurance, but
that child on the way will be
relying on his or her parents
for nancial support for at
least the next 18 years. Life
insurance ensures you can
provide that support even if
something happens to both
you and your spouse.

and doctors. Patients are
more informed than ever
before and are interested in
taking a more active role in

Thanks to the Internet,
people are able to access
information that, in the past,
was not easily available. A
few decades ago, medical
data may have been elusive
and lled with confusing
jargon the layperson might
not have understood
without a crash course in
biology. However, today
there are different Web
sites that clearly spell out
information about certain
illnesses and diseases. This
means that patients may no
longer be walking blindly
into appointments with
doctors.
Facing an illness is never
easy, and the decision-
making abilities could be
hampered by emotions and
the desire to improve as
quickly as possible. Patients
who do not have all of
the facts may have to rely
solely on the expertise of
caregivers and physicians
when making important
healthcare decisions. By
knowing the avenues of
information, a patient can
nd assistance with careful
decision-making.
While learning about
an ailment is important,
there are some things to
consider.

The American Physical
Therapy Association says
physical therapists diagnose
and treat individuals of all
ages who have conditions
that limit their abilities to
perform functional activities.
Limitations in mobility may
result from injury or illness
or be present at birth.
Many physical therapists
develop a plan to reduce
pain and restore function
through various treatment
techniques with the ultimate
goal of restoring a patients
functional independence.
Physical therapists are
licensed healthcare
professionals who must
receive a degree from
an accredited physical
therapist program before
taking national licensure
exams that enable them to
open a practice or work in a
facility. The Bureau of Labor
Statistics predicts a 30
percent growth in physical
therapy careers through
2018, which is a rate better
than all other occupations.
Unemployment rates for
physical therapists are very
low, and they are in high
demand.
Physical therapists employ
various physical modalities
to help with certain
conditions. For example,
a patient may be asked to
perform various range-of-
motion exercises to restore
function to an injured
part of the body. Physical
therapists also may use
heat, cold and electrical

Physical therapy sessions
frequently include some
form of massage as well.
While athletes frequently
rely on physical therapy as
they recover from injuries,
others can benet from
physical therapy as well. For
example, physical therapy
may work in conjunction
with other treatments for
cardiopulmonary disease.
The cardiopulmonary
system delivers oxygen
to active tissues, which
plays an important part
in movement. When the
cardiopulmonary system
is compromised, muscles
and other tissues may not
function as they should,
and certain exercises and
mobility therapies may be
needed. Physical therapists
also aid in improving
physical ailments related
to neurological diseases,
such as cerebral palsy,
multiple sclerosis, spinal
cord injuries, Parkinsons
disease, and Alzheimers
disease. Physical therapists
may also help treat poor
vision, poor balance and
paralysis.
Children who have learning
disabilities related to a
neurological or physical
condition may benet from
certain forms of physical
therapy. For example, vision
problems can compromise
academic performance, so
physical therapy that aims
to enhance visual tracking
skills and strengthen the
eyes in conjunction with
corrective lenses may help
youngsters, or even adult
students, perform better in
the classroom.
Very often physical therapy
will be recommended by
a general doctor or an
orthopedic surgeon as part
of recovery from a surgery
or a condition. The therapist
may work in conjunction
with another doctor to
provide a program

and safe recovery. Physical

gradually and resistance is
slowly built up as the body

COMMUNITIES
For people who reside
in rural communities,
access to healthcare
services is often limited.
Some rural facilities
may not be equipped to
handle transitional care or
specialized follow-up care
after a life-changing illness
or event. Allevant Solutions,

of Mayo Clinic and Select
Medical, is a resource
available to make improved
healthcare a reality.
According to the U.S.
Department of Health &
Human Services, one-
quarter of Americas
population lives in rural
areas, where hospital
closures have had a
profound impact on
access to healthcare.
Compared to people who
live closer to cities, rural
residents generally have
fewer doctors and fewer
resources. In addition,
ventilator or transitional care
services may not be readily
available locally.
Other difculties include
delivering services that
target special health
needs to rural areas.
Low population density
can make it difcult
to advertise important

distribute vaccines, such
as inuenza or shingles,
and newborn and young

be happy to know that
those physical benets are

cognitive benets as well.
As the American
Psychological Association
acknowledges, the
connection between
exercise and mental health
is hard to ignore, and the
APA notes that the following
are just a few of the mental
benets men and women
might reap from regular
exercise.
Improved mood
Many people feel great after
exercising, especially if that
exercise comes at the end
of a particularly stressful
day. However, those extra
laps on the track or those
hours spent on the treadmill
dont just pay short-term
dividends. In a controlled
trial overseen by Duke
University researcher
and clinical psychologist
James Blumenthal,
sedentary adults with
major depressive disorder
were assigned into one of
four groups: supervised
exercise, home-based
exercise, antidepressant
therapy, or a placebo pill.
Those in the exercise and
antidepressant groups had
higher rates of remission
than those in the placebo
group, and Blumenthal
concluded that exercise
was generally comparable
to antidepressants for
men and women with
major depressive disorder.
In addition, in following
up with patients a year
later, Blumenthal found
that those who continued
to exercise had lower
depression scores than
those participants who were
less active.
Blumenthals study was not
the only one to conclude
that exercise can have a
positive impact on mood.
In a review of 11 studies
that examined the effects of
exercise on mental health,
Boston University professor
of psychology Michael Otto
and his colleagues found
that exercise could be a
powerful tool when
treating clinical depression,
and even recommended
clinicians include exercise
as part of their treatment
plans for depressed
patients.
Antidote to
Anxiety
Some researchers, Otto
included, have begun to
examine the effects of
exercise on treating and
possibly preventing anxiety.
The bodys nervous system
responds quickly when
people feel frightened or
threatened, often causing
the bodys heart rate to
increase and sweating and
dizziness to occur. Those
people who are especially
sensitive to anxiety respond
to these feelings with fear,
and that makes them
more likely to develop
panic disorders. But Otto
and fellow researcher
Jasper Smits of the Anxiety
Research and Treatment
Program at Southern
Methodist University studied
the effects that regular
workouts might have on
people prone to anxiety.
Since exercise produces
many of the same physical
reactions, such as sweating
and an elevated heart
rate, the body produces
when responding to fear
or threats, Otto and Smits
wanted to determine if
exercise might help people
prone to anxiety become
less likely to panic when

with heightened sensitivity
to anxiety, Otto and Smits
found that the subjects who
participated in a two-week
exercise program exhibited
marked improvements in
anxiety sensitivity compared
to those participants who
did not take part in the

this improvement was a
result of the exercise group
participants learning to
associate the symptoms
common to both fear
and exercise, such as
sweating and an elevated
heart rate, with something
positive (exercise) instead
of something negative
(anxiety).

The demand for fresh
produce has increased
in recent years as more
people are turning to fresh
fruits and vegetables for
their nutritional value. That
increase in demand has
forced many suppliers to
import more produce from
other countries, which could
be putting consumers
health at risk.
Although the United
States and Canada may
have stringent standards
for produce, many other
countries do not. Less
stringent regulations
overseas can result in
irrigation water carrying
sewage, pollutants and
parasites to crops, and
herbicides and pesticides
may be used in abundance
in foreign countries where
such usage is subject to
little, if any, oversight. Fewer
regulations means some
farms pay more attention to
prot than to the purity and
safety of crops. The Pure
Food Growers of America
states that the average
American consumes
more than 10 pounds of
insecticides and herbicides
every year from produce.
Many of these substances
are proven carcinogens.
Thoroughly washing and
soaking fresh produce is the
key to removing potential
hazards from foods. Organic
fruits and vegetables may be
less risky, but even organic
foods are susceptible to
contamination because of
potentially unsafe handling
practices.
All produce should be
washed before eaten. Before
cleaning produce, stock up
on a few supplies. You will
need a large plastic bowl,
some apple cider vinegar or
baking soda and a produce
brush. Add enough cool
water to cover the produce
you will be washing. Add
either three tablespoons per
gallon of water of the vinegar
to the bowl or sprinkle about
three tablespoons of the
baking soda into the water.
Its best not to mix both
the vinegar and the baking
soda, or you may end up
with a foaming, overowing
concoction thanks to the
chemical reaction that
occurs when vinegar mixes
with baking soda.
Add the vegetables or fruit
to the treated water and
allow it to soak for around
10 minutes. Use a vegetable
brush to thoroughly scrub
the produce. Some foods,
like celery and lettuce, have
dirt or bugs trapped in their
ribs and folds. Soaking and
scrubbing can dislodge any
bugs. Instead of washing the
entire head at once, wash
lettuce leaves as they are
used to retain the vitamins
and minerals.
After rinsing the produce,
allow to dry before eating. A

Routine health screenings
are an integral part of
maintaining personal
health. Although some
individuals make frequent
visits to their physicians,
many others limit their
doctor visits to their annual
physicals.
Those who have never had
a physical examination may
be unsure of what to expect
of such a procedure. That
uncertainty can induce
anxiety. However, a
physical examination is a
simple procedure for many
people. Depending on a
patients age, doctors may
consider a more extensive
examination, but the
following procedures are
what constitute a physical
for younger patients
without preexisting medical
conditions.
Vital Signs
During a physical, doctors
will check patients
vital signs. Patients can
expect to have their
blood pressure taken and
pulse rates measured.
Respiration rates may be
taken to determine if there
is a lung or heart problem.
Doctors also will examine
a patients ears, nose and
sinuses. The neck and
possibly under the arms
also will be examined to
check the feeling of the
lymph nodes and the
carotid arteries. Some
doctors hook patients up to
a machine that measures
heart rhythm through
sensors on various areas of
the body.

patients medical histories,
updating their charts with
any new information when
necessary. Inquiries about
particular health issues
that run in the family will be
discussed, as well as any
illnesses or hospitalizations
patients may have had
in the past. Doctors will
ask patients if they are
taking any medications or
supplements, and also will
ask if patients have been
dealing with any nagging
medical issues.
Physical
Examination
During the physical
examination, doctors will
look at and feel patients
abdomens to detect the
size of their livers and to
determine if there is any
abdominal uid present.
The stethoscope may be
placed on the abdomen
to listen for bowel sounds.
The physician also may
examine other areas of the
body, including the back and
spine to ensure there are no
abnormalities. A weight and
height check will be given
to measure for body mass
index, or the percentage
of body fat patients have,
which will then be measured
against the norm for patients
of similar age, weight and
height.

check that the uterus and

the breast tissue. A sample
may be taken from the
cervix so a Pap smear can
be conducted to determine
if cervical cancer is present.
An external exam will look
for sores or other indications
of disease.
Men can expect a visual
examination of the penis and
testicles. The physician will
check the external structure
to look for abnormalities like
tumors or hernia. To check
for hernia, the physician may
examine visually or feel the
scrotum and ask patients to
cough as part of the exam.
Dermatological
Exam
Some doctors will examine
the skin for the appearance
of unusual moles or growths
that could be indicative of
skin cancer or refer patients
to a dermatologist for such
an examination.
Additional Tests
Nerves and neurological
responses, including
strength and balance tests,
may be tested during a
physical. Doctors also may
look for sensory changes in
the extremities of the body.
Doctors may order urine
and blood analyses to verify
that the levels are within
range. A blood test usually
will include a cholesterol
test, and may include an
examination of blood-sugar
levels to rule out diabetes.
Men and women over the
age of 40 may be referred
to specialists who can
conduct a mammography,
colonoscopy or prostate
examinations. These tests
will rule out the presence of
cancer or other diseases.
Doctors also may suggest
patients have their vision and
hearing checked.
Annual physicals can
reassure men and women
that they are in good health.
Such examinations also
can serve as early detection
systems that can catch
potential health problems
before they become more
serious.
