How can an individualized
planning process lead to
careers in the real world?
Each student is di!erent. Each pathway to
adult life is unique. To meet the unique
needs of each student, each transition plan
must be as di!erent as individual
snowakes. Transition plans are informed
by:

!the community;
!e!ective and proven practices.

From 2008 to 2011, Cornell Universitys Employment
and Disability Institute evaluated the transition and career
development activities of 16,000 students with 60
school-community collaborations across the State of
New York.
We compared that student data with the National
Longitudinal Transition Study 2 to understand the
contributions of certain leading transition practices on
the lives of youth with disabilities.

What happens when we add up these percentages?
We found that there was no one model; e"ective
practices are indigenous and can be mixed and
matched in di"erent ways. Which of these involve
extensive individualized planning?

another to plan for life outside of school...living, learning

When considering your transition budget, you
probably think, I could do more if I had more.
However, to make the case for more resources,
it is essential that a potential funder
understands how this additional funding will
improve the lives of your students -- in a
sustainable manner.

of each of your students -- helping them meet
their long term goals?
