STANDARD OPERATING PROCEDURE (SOP) RESEARCH FUND APPLICATION (FOR STUDENT) ADVANCED MEDICAL & DENTAL INSTITUTE UNIVERSITI

SAINS MALAYSIA

1. Applicants are required to fill in the Research Fund Application Form: a. Research grant application form: i. ii. AMDI Student Research Fund with checklist form. Graduate Research Fund (ex: USM-RU-PRGS, USM-PERDANA,IPS Graduate Fund) with checklist form. b. If the application involve subjects and/ or samples which requires approval from Human Ethical Committee (JEPeM) and/ or Animal Ethical Committee (JEPeH), the application should be submitted together with: . i. Human and / or Animal Ethical Approval Application form. ii. Patient Information and Consent Form. *** Important note :
Patient Information and Consent Form should be submitted in 2 version, Malay and English, using the template which has been provided.

Please make sure every application sent was signed by Supervisor in application form and checklist.

Additional Info For research conducted outside USM, applicants should use an ethical form provided by the institution. (ex: If subject and location of research at Hospital Pulau Pinang, applicant must fill the ethical form from Hospital Pulau Pinang/ Ministry of Health). 2. Application form from part (1a) dan (1b) mentioned above must be send to AMDI Research Secretariat (SKP) in both soft and hard copies for prosedure checking. 3. Application forms will be screened uncompleted forms will be returned to the applicants for correction, while completed forms will be included into the agenda of AMDI Research Committee (JKP) meeting thas has been scheduled. 4. The application form will be recorded in (IRIiS) IPPT Research & Innovation Information System. 5. Applicants will be asked to present their research proposal in AMDI JKP meeting. The AMDI JKP will evaluate the research proposal.

6. Applicants will be informed regarding the status of her/his application; either rejected or approved with condition (have to modify the application and re-submit to AMDI SKP together with the editted notes). The corrected application will be included into the next AMDI JKP meeting. 7. For successful application, the application form will be endorsed by AMDI JKP chairman. At this stage, decision will be made whether the research can be initiated using AMDIs fund (under AMDI Student Research Fund scheme) if no ethical issues involved. 8. For those applications that involve ethical issues, their application will be

forwarded to the Human Ethical Committee (JEPeM) USM or Animal Ethical Committee (JEPeH) USM for approval. 9. While the research conducted outside USM, the ethical approval (Human/ Animals) is under institution/agencies. (ex: If subject and location of research at Hospital Pulau Pinang, applicant must obtain the approval from Clinical Research Centre from Hospital Pulau Pinang/ Ministry of Health) *** Important note:
Applicants should be inform AMDI Research Secretariat (SKP) the status of the ethical application.

10. For succesful application, AMDI SKP will issue a Letter of Approval for Research endorsed by AMDI Director to applicants. A copy of Approval Letter for Research will be given to the AMDI Deputy Director (Finance). 11. Research will be conducted in full extent.
