Hello Fellow Parents, The Prom committee is so excited that the big event is just around the corner.

There are a few items that we need your help with so every student has a wonderful time. First, the Liability Form attached must be signed by both the student and parent/guardian and returned prior to the day of the prom. If the students date does not attend PCA, or is not a Junior or Senior at PCA, you will need to get them a liability form and have them complete the form and return it prior to the day of the prom also. Unfortunately, there can be no exceptions to this. Second, please complete the Payment/Information voucher and return with payment. Included on this voucher is some necessary information that will help us better match students together for the bus ride down to the event. Please return completed Liability Waiver, Information Voucher and payment to Jen Sherman, 6421 Willowdale Drive, Plano, TX 75093. We will also be at Main Street during lunch at main street on 3/28, 3/31, 4/1, 4/4, 4/7, 4/8, 4/11. Destination Unknown Comfort Color T-Shirts will be available for sale beginning March 28th. Deadline to order is April 18th and cost is $15.

LIABILITY RELEASE AND HOLD HARMLESS AGREEMENT FOR 2014 JUNIOR-SENIOR FORMAL - DESTINATION UNKNOWN

Assumption of Risk The undersigned student and the undersigned parent and/or guardian of the above-named student (collectively the undersigned) hereby assume any and all risk, known or unknown, that may exist as a result of the above-named students participation in the Event, including, but not limited to traveling to the Event, attendance and participation at the Event, and traveling from the Event. Release of Organizers The undersigned do hereby voluntarily and expressly, on their behalf and on behalf of the above-named student, release and forever discharge the Organizers from any and all liability, claims, damages, demands, or personal injuries which arise from or are in any way connected with the above-named students participation in the Event, including those allegedly attributable to negligent, reckless, or grossly negligent acts or omissions of the Organizers. Indemnification of Organizers The undersigned do hereby voluntarily and expressly, on their behalf and on behalf of the above-named student, agree to indemnify and hold harmless the Organizers from any and all liability, claims, damages, demands, or personal injuries which arise from or are in any way connected with the above-named students participation in the Event, including those allegedly attributable to negligent, reckless, or grossly negligent acts or omissions of the Organizers. Obligation to Obey Organizers The undersigned student shall abide by and obey all instructions given by the Organizers with respect to the students participation in the Event. The undersigned further understand that in the event that the undersigned student fails to comply with instructions given by the Organizers, the Organizers have the right, in their sole discretion, to remove the undersigned student from the Event, and to be released to the students parent or guardian. The undersigned acknowledge that they have had sufficient opportunity to read this entire Release and Hold Harmless Agreement, they fully understand its content, they execute it voluntarily, and they agree to it by its terms.
