Dear Sir:
Christian greetings!
I seek the benevolence of your office in my persistence and desire to apply for the scholarship
grant offered by a private company. This year, I came to Cebu Doctors' University in good faith that
they can help me to study Medicine. I am indeed grateful to be given the Entrance Scholarship which
was only limited to a free tuition fee for the whole academic year. This semester, I paid the
Miscellaneous Fees using the Cash Prize I received from my Alma Mater and from my Review
Center. However, I am determined to seek for another scholarship that can cover the Miscellaneous
Fees, since my parents can hardly afford to finance my studies. The news about the scholarship
offered by the private company, Maxicare Health Corporation and Equitable Foundation,
Incorporated, for a deserving student of the university brought me a new hope. This scholarship grant
is the answer to all my prayers to fully support my studies. Herein listed are my credentials for this
application.
I graduated in elementary at Port Junction Adventist Elementary School in 2004 and in high
school at Naga View Adventist College in 2008 as a consistent honor student and VALEDICTORIAN.
I have also been the recipient of EXCELLENCE IN MATH and EXCELLENCE IN SCIENCE awards.
Furthermore, I received a LEADERSHIP AWARD for the outstanding co-curricular and leadership
performance serving as an exemplar of service to the school, the Church, and in the community.
When I entered college, I was offered scholarships from various universities, but I sought to pursue
my degree in Manila Adventist Medical Center and Colleges, Inc., where I was priveleged to enjoy a
full scholarship. All through those years, I was consistently on top of my class in the pursuit of
maintaining my scholarship. Last April 2012, I graduated with the degree of Bachelor of Science in
Nursing where I received the highest award SUMMA CUM LAUDE with a general average grade of
1.203 in all subjects in my curriculum with 1.50 as the lowest grade. In addition, I was also honoured
to receive the FLORENCE NIGHTINGALE AWARD from the College of Nursing, and recognized for
having achieved an OUTSTANDING PERFORMANCE in the CLINICAL INTERSHIP PROGRAM with
the average grade of 94.25%. To continually enhance my skills and knowledge in the field of health, I
enrolled in the Basic Life Support and Advanced Cardiac Life Support Course last April 2012 where I
attained the HIGHEST GRADE in the written examination and received a Certificate of Proficiency
and License in BLS-CPR and ACLS from the Philippine Heart Association Philippine College of
Cardiology. Recently, last January 2013, I was very privileged to be recognized as TOP 7 with a
rating of 84.4% in the Nursing Licensure Examination held on December 2012.
Herewith attached are my credentials for your perusal.
In my intent to pursue my studies in the field of Medicine as a SCHOLAR, I pray that I will
receive a positive response on my humble request. If granted the scholarship, I will be very grateful
and will maintain it with excellence to the best of my abilities.
