Cashless Hospitalization

a. Cataract, Benign Prostatic Hypertrophy, Hysterectomy f or Menorrhagia, Fibromyoma, Hernia, Hydrocele, Congenital
Internal Diseases, Fistula in Anus, Piles, Sinusitis are not payable f or all Insured Persons.
b. Any heart, kidney and circulatory disorders are not payable f or all Insured Persons suf f ering f rom
hypertension/diabetes. If these diseases are pre-existing at the time of proposal they shall not be covered even during
subsequent period of renewal.

Citibank will charge an annual transaction f ee of Rs.195 per individual per policy. However if you book the policy online, this
f ee of Rs.195 will not be charged. The premium mentioned above will be reduced by 10% if you cover 3 or more members of
your f amily.For example, if you buy Health Forever f or yourself , your spouse and 2 children, the net premium payable will be
10% less than the total cost as per the table above

Claims
You can make a claim under your Health Forever coverage in two ways:
1. Get admitted to a hospital that is part of the Health Forever network, call the 24-Hour toll-f ree helpline (1800 345 88 99) and
take an authorization at least 72 hours in advance. Thus you avail of Health Forever's Cashless hospitalization benef it. Royal
Sundaram will settle the bills directly with the hospital.
In case of an emergency hospitalization, call Medicare within 48 hours af ter being admitted to take the authorization.
2. If admitted into any other hospital, all bills along with supporting documentation are to be sent to Royal Sundaram f or a
reimbursement.

Enrollment
Once you enroll f or the plan, the applicable premium depending upon the plan chosen by you will be debited to your Citibank
Credit Card or Bank Account. Coverage begins f rom the date Citibank pays premium to Royal Sundaram towards your Health
Forever Plan. Royal Sundaram will issue the policy certif icate along with your Cashless Health Card and send it to the Indian
mailing address which you would be providing in the f orm.
Exiting the plan
Should you wish to cancel your Health Forever coverage, you just need to write to Citibank N.A, P.O. Box 4380, Anna Salai
P.O, Chennai 600 002. In case you cancel your coverage bef ore the end of a f ull policy year, Royal Sundaram will ref und
part of the premium paid as per applicable short period rates.

Policy Exclusions
The Company shall not be liable to make any payment under this Policy in respect of any expenses whatsoever incurred by
any Insured Person in connection with or in respect of :
1. Pre-existing Conditions: Any medical condition or injury, which existed bef ore the commencement of the policy period
(even if unknown to the insured) or f or which care, treatment or advise were sought, recommended by or received f rom a
Doctor. Complications arising f rom a pre-existing medical condition or injury will be considered part of such pre-existing
condition or injury. Pre-existing conditions will be covered af ter f ive years of continuous insurance with us.
2. 30 Days Waiting Period: Any disease contracted by the Insured Person during the f irst 30 days f rom the commencement
date of the cover. This exclusion shall however, not apply if in the opinion of Panel of Medical Practitioners constituted by the
Company f or the purpose, the Insured Person could not have known of the existence of the Disease or any symptoms or
complaints thereof at the time of making the proposal f or insurance to the Company.
3. First Year Exclusions: During the f irst year of the operation of the Policy the expenses on treatment of :

waived f or Insured Persons holding an existing Health Insurance Policy, provided the Policy is renewed bef ore the expiry
date of the existing Health Insurance Policy.
4. Treatment arising f rom or traceable to pregnancy/childbirth.
5. Circumcision unless necessary f or treatment of a disease not excluded hereunder or as may be necessitated due to an
accident.

a. Ionising radiation or contamination by nuclear f uel or f rom any nuclear waste f rom burning nuclear f uel or
b. Radioactive, Toxic, explosive or other dangerous properties of any explosive nuclear machinery or part of it.
6. Any treatment relating to change of lif e.
7. The cost of spectacles, contact lenses and hearing aids.
8. Dental treatment or surgery of any kind unless requiring hospitalization.
9. Convalescence, general debility, a `run-down' condition or rest cure, external congenital disease, def ects or anomalies,
sterility, venereal disease or intentionalself injury.
10. All expenses arising out of any condition directly or indirectly caused by or associated with Human T-Cell Lymphotropic
Virus Type III (HTLB-III) or Lymphadenopathy Associated Virus (LAV) or the Mutants Derivative or variations of Def iciency
Syndrome or any syndrome or condition of a similar kind commonly ref erred to as AIDS. (Inf ormation available in the Policy
Summary booklet).
11. Charges incurred at Hospital or Nursing Home primarily f or diagnostic, X-ray or laboratory examinations or other
diagnostic studies not consistent with or incidental to the diagnosis and treatment of the positive existence or presence of
any ailment, sickness or injury, f or which conf inement is required at a Hospital/Nursing Home as def ined.
12. Expenses on vitamins & tonics unless f orming part of treatment f or injury or disease as certif ied by the attending
Physician.
13. Voluntary medical termination of pregnancy during the f irst 12 weeks f rom the date of conception.
14. Injury or disease directly or indirectly caused by or contributed to by nuclear weapons/materials or radioactive
contamination.
15. Injury or disease directly or indirectly caused by or arising f rom or attributable to War, Invasion, Act of Foreign Enemy,
Warlike Operations (whether War be declared or not).
16. Any claim or expense of any kind caused directly or indirectly by:

Any routine or preventative examinations, f ascination, inoculations or screening.
18. Outpatient treatment charges.
19. Sex change or any treatment which results f rom, or is in any way related to, sex change.
20. Hormone replacement therapy.
21. Renal dialysis, except where this is in connection with acute secondary f ailure and is part of the intensive treatment.
22. The treatment of psychiatric, mental or nervous conditions and insanity.
23. Any cosmetic, plastic surgery, aesthetic or related treatment of any description, whether or not f or phychological
reasons, unless medically necessary as a result of an accident.
24. Use of intoxicating drugs/alcohol and the treatment of alcoholism, solvent abuse, drug abuse or any addiction and medical
conditions resulting f rom, or related to, such abuse or addiction.
25. Any treatment received in convalescent homes, convalescent hospitals, health-hydros, Nature Cure clinics or similar
establishments.
26. Any stay in hospital f or any domestic reason or where there is no active, regular treatment by a specialist.
27. Any treatment received outside India.
28. Industrial disaster.
29. Any Ayurvedic, Homeopathic, Naturopathy or any other f orm of local medication.
30. Complication of surgery, therapy or treatment administered on the Insured Person which is not prescribed or required by
a Registered Medical Practitioner/Registered Medical Institution in their prof essional capacity.
31. Taking of drugs unless it is taken on proper medical advice and is not f or the treatment of drug addiction.
32. Any f ertility, sub-f ertility or assisted conception operation.
33. Any person whilst engaging in speed contest or racing of any kind (other than on f oot), bungee jumping, parasailing,
ballooning, parachuting, skydiving, paragliding, hang gliding, mountain or rock climbing necessitating the use of guides and
ropes, pot holing, abseiling, deep sea diving using hard helmet and breathing apparatus, polo, snow and ice sports and
activities and similar hazards.
34. Any person whilst engaging in aviation, whilst mounting into or demounting f rom or traveling in any aircraf t other than as
a passenger (f are paying or otherwise) in any duly licensed standard type of aircraf t anywhere in the world. ["Standard
type of Aircraf t" means any aircraf t duly licensed to carry passengers f or hire or otherwise by appropriate authority
irrespective of whether such aircraf t is privately owned or chartered or operated by a regular airline or whether such an
aircraf t has a single engine or multiple engine"].
