pit$h well in e"$ess of full astern' (he pit$h indi$ation needle rea$hed the e"treme of its s$ale and
appeared to be stu$k at 100@ astern' (he ) $ontrol o%erride button $ould ha%e resol%ed the
situation if a$ti%ated, but the master was unfamiliar with its use' In an attempt to +$at$h the needle
and pull it ba$k+, he put the pit$h demand handle to full astern' (his resulted in the pit$h
in$reasing to nearly 110@ astern and the ship pi$ked up speed' 8y the time he used the

te$hni$ian for the ) eAuipment had made an error in adjusting the astern pit$h $alibration
settings' #stern mo%ements were not tested e"$ept during a $rash stop test' When the %essel
$ommen$ed normal ser%i$e, the problem with the astern pit$h appeared a few times' (he root
$ause of the problem remained unknown' It was only when an astern mo%ement in e"$ess of -0@
was demanded that the defe$t manifested itself, resulting in the a$$ident'
LESSONS
? #lways $arry out a full set of ) tests, both ahead and astern' #stern mo%ements are not used
as often as ahead mo%ements' *owe%er, they are eAually important' If the ) system
malfun$tions in restri$ted waters, it is highly likely that it will result in an a$$ident' 8efore e%ery
operation in restri$ted waters, $arry out a $omplete set of engine tests in both ahead and stern
dire$tions'
? :amiliarise yourself with the eAuipment you operate ? in parti$ular, understand what to do when it
fails' In an emergen$y situation, you will ha%e %ery little time to think and reason' If you are not
fully $on%ersant with the failure modes of $riti$al eAuipment, you may not be able to rea$t Aui$kly
