Presenting Problem: Difficulties with math concepts in the areas of mental math and other area of curriculum. Learners mother passed away from cancer in January.
Recommendations: 1. Follow-up with xxxxxx, Homeroom Teacher Grade Five for history of last years academics to review previous difficulties and strategies used. Team Follow-up date June 2, 2014, 2014.

Presenting Problem: Learner has many assignments not passed in and is missing a lot of school lately.
Recommendations: 1. Contact parents regarding attendance 2. Student will stay on Tuesday and Thursdays after school for enhancement period to catch up and receive support at that time.
