Res. Sir,
May I be kindly permitted to bring the above stated subject matter to you kind eye and file for
sort of required favor from your good office on receiving this letter from me.
Till date the Guntur Metropolis sample collection point is on name, but in fact it had been started
by Mr. Malladi Vasu S/o. Malladi Edukondalu who is an inhabitant of Guntur city itself. He had started
the point but looking to my economically situation he decided to help me by just putting my name in the
Agreement papers, and all these years I am managing this point.
But to day I am facing some problems with the collection boy Mr. Laxman. He is not hand overing
me the money properly. So I have decided to give the point to Mr. Malladi Vasu back since originally he
is the owner. In this decision I do not have any sort of objection or even pressure . This transferring is
purely on my wish and there is no completion.
Hence I only request you people to kindly co-operate to me in this issue and further do kindly treat
Mr. Malladi Vasu as the owner of the Sample Collection point Guntur, for which I shall be highly thankful
to you Sir.
Also deal with him only in all aspects.

This is to say that I Mrs. Polisetty Srilakshmi W/o. Polisetty Srikanth inhabitant of 13-07-9/a
Gunturivarithota, Guntur Ward No:9, here agrees with my full concern to transfer the owner ship of
Metropolis Sample Collection point, belongs to M/s. Mertropolis Health Servies (India) Ltd., which is
located at Guntur from me to Mr. Malladi Vasu S/o. Malladi Edukondalu inhabitant of Old Guntur Guntur
city who is residing at door No:16-15-234, Yadavula Bazar, Old Guntur, Guntur, since I am facing some
financial problems like Collection boy Mr. Lakshman is not remitting properly to me the collection in time
and sometimes misandling the cash.
At the same time Mr. Malladi Vasu is the real owner of this collation point right from the beginning.
Hew only wants to help me by putting the ownership in my name. He is so kind enough to give me the
ownership all these days for which I am so thankful to him.
Hence I am here transferring the ownership title to him back with full consent of mine.
