Causes
Itcanbeduetodysregulationofcertainhormonesorlocalcausessuchasexcessivenipplestimulation.
Hormonalcausesmostfrequentlyassociatedwithgalactorrheaarehyperprolactinemiaandthyroidconditions
withelevatedlevelsofTSHorTRHhormones.Noobviouscauseisfoundinabout50%ofcases.

Lactationrequiresthepresenceofestrogen,progesteroneandprolactin,andtheevaluationofgalactorrhea
includeselicitingahistoryforvariousmedicationsorfoods(methyldopa,opiates,antipsychotics,serotonin
reuptakeinhibitors.)andforbehavioralcauses(stress,andbreastandchestwallstimulation),aswellas
evaluationforpregnancy,pituitaryadenomas(withoverproductionofprolactinorcompressionofthepituitary
stalk),andhypothyroidism.Adenomasoftheanteriorpituitaryaremostoftenprolactinomas.Overproductionof
prolactinleadstocessationofmenstrualperiodsandinfertility,whichmaybeadiagnosticclue.Galactorrhea
mayalsobecausedbyhormonalimbalancesowingtobirthcontrolpills.

GalactorrheaisalsoasideeffectassociatedwiththeuseofthesecondgenerationH2receptorantagonist
Cimetidine(tradename:Tagamet).Galactorrheacanalsobecausedbyantipsychoticsthatcause
hyperprolactinemiabyblockingdopaminereceptorsresponsibleforcontrolofprolactinrelease.Ofthese,
risperidoneisthemostnotoriousforcausingthiscomplication.Casereportssuggestprotonpumpinhibitors
havebeenshowntocauseGalactorrhea.
Neonatalmilk
Neonatalmilkorwitch'smilkismilksecretedfromthebreastsofmanynewborninfants.Itisconsidereda
normalvariationandnotreatmentortestingisnecessary.Infolklore,witch'smilkwasbelievedtobeasourceof
nourishmentforwitches'familiarspirits.
