DECLARATION

______________________, location ____________ do hereby declare and undertake that:

1. I have applied for enrolling to Corporate Post Graduate Diploma in Business Administration (C-
PGDBA) being offered and conducted by Symbiosis Centre for Distance Learning.

2. I know that acceptance of my application is subject to the standard enrolment procedure of SCDL
which involves fulfillment of necessary eligibility criteria outlined by SCDL and submission of
necessary documents and payment of full non refundable registration fees of Rs.1,000/-.

3. I have also been informed that the program will commence in the month of August 2014.

4. I have also been informed that SCDL shall not commence the program until minimum 30 numbers of
students are enrolled per location. I have applied for enrollment and paid the fees with complete
knowledge of this provision. I have been also informed that in case minimum number is not achieved
the fees paid, other than registration fees, will be refunded by SCDL.

5. I have perused the eligibility criteria for admission to the said program as informed by my employer as
per Annexure -1 to this declaration.

6. I also undertake to pay the program fees of Rs./-51000 upon confirmation of admission, which I have
been informed that is to be paid in following manner:

7. I know that if I fail to pay the fees as agreed in clause 6 above, SCDL shall have the right to cancel
my admission.

8. I also undertake to attend all the classes and in case my attendance falls below 80 % then in that
case I am not eligible to appear in the examination.

9. I am also informed that minimum 10 students per specialization is required to conduct the classes for
the 3rd Semester, failing which SCDL shall only organize counseling sessions.

10. In case of my transfer to some other location within the Company, I will be able to avail the classroom
lecture and other facilities at such transferred location only if SCDL has tie up / agreement for such
location & that the same program is being conducted by SCDL at such location and the same
schedule/timetable/commencement date. In absence of any tie up / agreement, I will have to attend
lectures / sessions at present location only.

11. I also know that in case I leave the present employer for any reason whatsoever or my employment is
terminated, within 3 months of commencement of the program, then I have option to transfer my
admission to regular PGDBA program of SCDL for running academic session on payment of deficient
fees and charges (if any) and in such case all the rules / regulations applicable to PGDBA program
shall be acceptable to me.

12. I understand that refund of fees paid will be done by SCDL on submission of refund application by
me within 90 days from the date of enrollment. SCDL shall deduct 50% as administration charges,
provided he/she is in the service of Cognizant. If Any Cognizant nominated student enrolled for this
program resigns or is terminated from the services of Cognizant, his/her admission will be cancelled
and in such cases Cognizant will not be required to pay any remainder fees(if any) for such students
to SCDL & SCDL shall not refund any Agreement of fees paid against such students to Cognizant


13. I also know that in case I leave the present employer for any reason whatsoever or my employment is
terminated, after 4 months of commencement of the program, then my admission to this program
shall stand canceled and I will not be eligible for any refund of program fees paid by me to SCDL

14. The evaluation methodology of SCDL has explained to me by my employer and I am satisfied with
the same as mentioned in Annexure -2 to this declaration.

15. In case I want to cancel my admission, I am required to make an application in writing to SCDL. I also
know that I am eligible to refund only prior last date of payment of third installment as mentioned in
clause 6 above.

16. I have been informed and I agree that SCDL shall deduct 50% of the fees paid as administration
charges and refund the remaining amount.

Annexure-2

Evaluation Methodology, Assessment & Standard of Passing:

(i) Online assignment: For each subject there will be an online assignment of 30 marks. Only two
free attempts of assignment for each subject (course) is allowed. Additional attempt of assignment
will be provided in event of failure.
There will be total 4 attempts to attempt any assignments. (2 Free + 2 Paid). The students are
required to pay Rs. 300/- for 3rd and 4th attempt in the event of failure. No further attempts will be
provided to the students

(ii) Minimum number of marks for passing: 50% in each subject.

(iii) Method: To be attempted through SCDL web site.

(iv) Online examination: For each subject there will be an online examination of 60 marks objective
and 10 marks subjective. Student can only re-attempt an online examination in the event of failure.

Online Examination within India: SCDL shall make necessary arrangement for students to appear for
online examination at the corporate premises once a month as per the mutually agreed schedule. The
corporate shall provide requisite infrastructure, PC, Internet connection, administration & resources
arrangements. The online exam at corporate premises will be monitored & managed by the respective
program coordinator from that corporate. Students who fail to appear for monthly online examination at
corporate premises or have backlog exam, for such students online exam will be conducted through
designated test centre along with regular SCDL students through On Demand Examination procedure
and policies.
Online Examination outside India: -- in an administered mode from an authenticated location such as
company, university, embassy, Consulate or school etc. The administered mode implies scheduling the
said examination with SCDL and being monitored by a respectable person from above enumerated
institutions/organizations. The administering authority and the scheduled examination shall require prior
approval of SCDL. The on line examinations will at all times be conducted as per the rules and policies
set forth by SCDL from time to time.
Examination Fee: Rs.300/- or US $ 20 per exam per attempt (non refundable) as applicable time to
time.
Fees should be paid in the form of DD drawn in favor of The Director, SCDL, payable at Pune.

On receipt of the exam fees & schedule SCDL shall schedule examination at corporate premises

Award of Diploma: Students are required to pass in online assignment and online examination
separately in each subject. The diploma will be awarded after passing in all online assignments & online
examinations & project.

Eligibility Criteria:

A. Each COGNIZANT TECHNOLOGIES SOLUTIONS LTD nominated student must be a graduate
from a UGC recognized / accredited university/institute.

B. A student who has appeared for final year exam may apply for the said program but, will be
admitted only on a provisional basis. Such a student will be confirmed provided he/she passes
in the final year exam and provides the final year mark sheet /graduation certificate within 5
months from the date of enrolment.

C. If such a student fails to provide the aforesaid documents regarding graduation in the said
timeline his/her admission to the said program will be cancelled and SCDL will not be liable to
refund any fees paid by a student in such an event.
