Background Information
Elsa lives at home with her mom and dad and dog. She is an only child but does have some
cousins who she is close with of similar age. She has been attending childcare for about a year and a
half, 5 days a week.
Mom reports that Elsa was born one week late after a non-significant pregnancy. She reported
no problems with the pregnancy or birth. Elsa was born naturally and weighed 8lbs, 9 ounces at birth,
she was 20 inches long. Mom reports that Elsa has had a typical development, she sat up around 7
months and was walking by the time she was 1 years old. Mom reports that at home there are no
developmental concerns, however mom states that Elsa is very headstrong and can have tantrums if she
is forced to do something she does not want to do. Mom reports that since Elsa has started school she
has been calmer and more willing to do things. Her language is advanced and mom and dad enjoy
having conversations with her. Elsas favorite things to do at home is play with her dog, Bobo and her
kitchen set, making dinner for mom, dad and Bobo when they get home. She also eats very well at
home and does watch about 30 minutes of TV a day, mostly while mom is getting dinner ready. Bath
time is an easy part of the day and bedtime is a well versed routine of 3 books and a song, before being
left alone to fall asleep. Elsa is most nights asleep by 8:30pm. Elsa has had a fairly healthy childhood
with no major illness or injuries.


Behavioral Observations
Elsa was observed in her classroom setting at Youth and Family Outreach. She appears to be a
happy, social little girl who enjoys being around her friends. Among her favorite activities at school are
puzzles, books and block area. She also loves to run around outside on the playground Her teachers
describe her as a busy body. Elsa is familiar with me as a teacher in a room she was in before moving
up to the preschool room; therefore she is very familiar and cooperative with me.

The assessment was done in the therapy room at Youth and Family Outreach over the course of 3 days.
The BDI-2 is a standardized, individually administered test of a childs development. It is administered in
structured, observational and interview formats. It is comprised of 5 major domains and several
subdomains within those domains.
The Developmental Quotient is used for each domain and has an average score of 100 with the average
range of scores being 85-115.
The Percentile rank shows how the childs score compared to other children of the same age. Scaled
scores are used for the subdomains, with an average score being 10 and the average range being 7-13.
Discussion of results
Adaptive: The adaptive domain assesses the childs self-care and personal responsibility skills.
In the subdomain of self-care Elsa scored in the 95
th
percentile with an age equivalent of 49 months and
a scaled score of 15, well above average.
In the subdomain of personal responsibility Elsa scored in the 95
th
percentile with and age equivalent of
51 months and a scaled score of 15, again above the average of 10 for subdomain score.
Her total developmental quotient for the domain was 122, again above the average score of 100. Elsa
ranked in the 93
rd
percentile overall for the adaptive domain.
Personal-Social: This domain assesses how the child interacts with adults and peers as well as
developing their own self-concept.
In the subdomain of adult interaction Elsa scored in the95th percentile with an age equivalent of 55
months and a scaled score of 15.
In the subdomain of peer interaction Elsa scored in the 75
th
percentile with an age equivalent of 42
months and a scaled score of 12.
In the subdomain of Self-concept and social role Elsa scored in the 63

percentile with an age equivalent

Her total developmental quotient for the domain was 110 placing her in the 75

Communication: This domain assesses the receptive and expressive language the child has.

In the subdomain of reasoning and academic skills Elsa scored in the 16

percentile with an age
equivalent of 28 months and a scaled score of 7 which is below the average score of 10 and on the
bottom of the 7-13 range.
In the subdomain of perception and concepts Elsa scored in the 37

36 months and a scaled score of 9, again below average however within the average range for the
subdomain.
Her total developmental quotient for this domain was 96 placing her in the 39

percentile for the
cognitive domain.

Total:
Her BDI-2 total developmental quotient is 112 placing her in the 79
th
percentile overall.
Summary and Recommendations:
Elsa is a lovable, social, busy little girl, who was assessed for the benefit of giving me familiarity to the
BDI-2. She was very excited to be able to go into the special blue room. (The therapy room at school is
called the blue room). In the blue room Elsa was very cooperative, however towards the end of the
assessment Elsa wanted to play with the toys in the room and focused less on the structured activities,
this is what I believe is the reason for the lower scores in the cognitive domain. I would recommend re
testing in the cognitive domain in a quite setting with little to no distractions.
Elsa scored very well in all the domains excluding the cognitive domain her scores ranged from 75
th

percentile to 93
rd
percentile. Overall she scored higher than average with a final developmental
quotient score of 112, where average is 100. Although Elsa tested lower in the 2 subdomains of
reasoning and academic skills and perception and concepts she still scored within average range on
both.
No other recommendations would be made at this time.
