process renewal of your membership for the year 2014-15 (on/before 30

1. Authorised Signatory Form and Nomination Form to be filled in with photograph and signed
by authorised signatory with office seal.
2. Two photographs of each of your two nominees for the FHRAI Discount Cards. One
photograph should be pasted on the Nomination Form.
Please send the Authorised Signatory Form duly completed alongwith the nomination
form, failing which the discount cards will not be issued.
3. Annual Membership Subscription amount in favour of F H R A I by Demand Draft
payable at New Delhi.
4. Listing Details of your establishment (sample enclosed). Please update the listing details to
be published in Electronic format (CD/Website) of Hotel & Restaurant Guide India 2015.
5. Hotel Fact Sheet (HFS) duly completed (for hotel members only).
