1,2,3 I spend the day checking the schedule to make sure we have a certain amount of treatment we need each day and
then making a list of what I needed to schedule on what day. Then when the patients came up front I scheduled
them and gave them certain days we could get them in for treatment. I then entered in some new checks that were
mailed to us and double check the information entered in previously. I then stamped the checks and put them in a
pile and in the drawer for our office manager to take to the bank. I called on existing patients to make sure they
were still on the insurance plan and got breakdown and frequencies on new patients.

1,2,3 I went into the office in the morning but Dr. Linford was out of town. I answered phones and was aware of how to
schedule for the week and where to place patients. I also called ahead on New Patients insurance to make sure that I
was able to get a breakdown before they came in for the following week. I entered in a few checks Jennifer had not
been able to enter in.

1,2,3 Dr. Linford was out of the office again this day. I went into the office still and answered phones and called on any
new insurance to get the breakdown of new patients that called. I also scheduled patients who needed to get in for
treatment or moved patients appointments around to a time that worked best with them. I was also aware of what
was best for Dr. Linfords schedule too. I went through the checks that had been done for the week and stamped
them and double-checked to make sure that they were entered in correctly.

1,2,3 Today I worked on all 3 objectives working on my better understanding to scheduling appointments, entering in
checks and calling on insurance information.

1,2,3 I have many other duties to my job as well but am very focused on learning my objective thoroughly, I worked on
insurance companies and making sure patients were up to date on the correct insurance and that it was in the
computer correctly. I also scheduled patients who needed to come back for treatment and answered phones. I also
was able to double check the checks entered to make sure the amount was the same.

1,2,3 Since this dental office has only been opened for a year I am able to call every day on new patients insurances. We
usually have about 3-4 new patients a day. I called on more breakdowns and frequencies and stamped checks and
looked at the amount to check to see if they were the same. I scheduled treatment for patients if they needed to get
back.

1,2,3 I can already tell I am getting a better understanding about how the schedule should fill up and look and we are
booked all next week. I have been working hard eery day at work to get the schedule better. I also help Jennifer, who
works up front the days Im not here with entering checks. And I also got a breakdown on all the patients for the day.

1,2,3 I scheduled treatment for almost all of the patients today that need to come back or made notes to call them next
week to get them in for future treatment once they look at their schedule. I entered in checks and also called on the
new patients insurance that came in today.

1,2,3 I called on new patients for the day and got full breakdowns from insurance companies. Some of the insurance
companies are a pain to get ahold of to a live person. Sometimes I can only receive a fax sheet but I am in the
process to find out how to contact someone voice to voice. I am still entering in checks and double checking the
amount entered in. I also am working on scheduling patients when they are in need of treatment or calling on the
phone accordingly to the time Dr. Linford would like.

1. I will have a better understanding with scheduling patients. I will schedule patients with the amount of time needed for each
procedure. In the end, the appointments will show the proper amount of time Dr. Linford will have to finish the treatment with
each appointment. I will be finished with this objective by August 4

2. I would like a better understanding of entering checks from insurance companies. I will enter in the amount on the checks into the
computer to show the payments were made. Then I will double check to make sure the amount are both correct and stamp the
checks to be taken to the bank to cash. This objective will be completed by August 4

3. I will gain a better understanding about calling insurance companies on patients to get a full breakdown of what their specific
insurance plan covers for each patient. I will ask about benefits and frequencies for each individual patient and the plan. I will
write all of the information down and enter the information into the computer. This part is crucial because in the end this will be
how I determine a close estimate of what the patient may owe for any future treatment. I will complete this objective by August

1,2,3, We have new patients every workday so in the mornings I call on the insurance. And take notes on all of the
information I gather from the insurance companies. I entered in checks that we received and also scheduled patients
for treatment.

1,2,3 We had a staff meeting to make sure I am scheduling the right way Dr. Linford would like. We went over the
accurate timing on what treatment needed longer times than other treatment. But so far the schedule is improving a
lot and we are full for two weeks. We are excited we are getting so busy. I also was able to call on the new patients
insurance when they brought their information to the appointment. I entered checks in and verified the amount was
correct on the check and in the computer.

1, 2, 3 I have gradually been getting better at all three of my objectives. Ive been working hard on all three everyday and
feel like its getting better. Some insurance companies are hard to get ahold of to a live person but I have gained
experience how to get ahold of people even though it takes a lot of time.

patients at a timely manner. Everyday I can feel an improvement.

1,2,3 Today was a really busy day because we were off on Friday due to July 4

. A lot of people called to schedule and
surprisingly enough we had a lot of new patients call. I got all of their insurance figured out and a breakdown done
before they come into the office that way we know what the insurance will cover if they need treatment. I worked
on all 3 objectives today.

started just a few weeks ago.

1,2,3 I called insurance companies for breakdowns on new patients. I also scheduled patients for treatment for next week
and made sure everyone left with an appointment made. I also double-checked checks that were entered in by
Jennifer the day before.

1,2,3 Summer has been a little slower than we would like, patient keep canceling appointments but I have still scheduled
every patient that needs to come back for treatment or cleanings. The scheduled is slowly starting to fill up like
normal again. I called on a list of patients who need treatment and cleanings. I was able to schedule a lot of people. I
also double-checked checks and call on a few different or new insurance plans.

1,2,3 Patients, who I called on Wednesday, started calling today to schedule appointments. I made sure timing was
valuable for Dr. Linford. I entered in checks from the mail and insurance companies. I also called insurance
companies.

1,2,3 We have had quite a few new patients. Everyday we have at least 2 so I am constantly calling to talk to the insurance
companies and get the right information we need at our office. I also scheduled treatment and hygiene for patients
who needed to come back and make appointments. I checked checks entered in and made sure the amounts were
correct.

1,2,3 I got a breakdown for a large family coming in and each one of their own personal breakdowns of the insurance
policy. I also scheduled people who need to come back for treatment. I check the checked entered in and entered a
couple new ones we had.

1,2,3 I have worked hard on all 3 objectives and have come a long way since the beginning even though its been a few
short weeks I have definitely seen an improvement in myself and in the work place.

1,2,3 I have completed my hours but continue to strive to continue by objectives as well as my other work. I schedule time
but better and every patient that walks through the door Ive been able to schedule them for cleanings or treatment.
I also have been able to talk to live people on insurance companies instead of a voice recording, which is so relieving
that was really hard in the beginning.

1,2,3 I have worked really hard over this semester to work on all 3 of my objectives. Today I realized I have improved a lot
and it feels like an accomplishment. I have come really far from when I first started. Its a great feeling.
