PROMOTION RULES & REGULATIONS

Promotion Dates: The Wisconsin Dental Association Tongue n Cheek patient e-newsletter
subscription promotion (the Promotion) is sponsored by the Wisconsin Dental Association Inc.
(the WDA). The promotion begins at 12:01 a.m. Central Time (CT) on August 7, 2014
(Promotion Start Time) and ends at 11:59 p.m., Central Time (CT) on August 14, 2014
(Promotion Cut-off). The Packers preseason football game takes place in Green Bay, Wis. on
August 22, 2014. The Promotion is subject to these Rules as well as to all applicable federal,
state and local laws and regulations.

Eligibility: Promotion is open to legal U.S. residents who are 18 years of age or older as of the
date of entry who possess a valid account, except employees and immediate family members
(spouse, parent, child, sibling and their respective spouses, regardless of where they reside) of
WDA employees, affiliated companies or subsidiaries or employees of participating promotion
sponsors, organizations, companies or businesses are not eligible to win. This Promotion does
not involve gambling of any kind and no entry fee, purchase or other consideration is required to
enter. The offer to participate in this Promotion is void where prohibited by law.

How to Enter: Visit http://www.wda.org/enewsletter-sign-up and subscribe to the Tongue n
Cheek and Teeth, too! e-newsletter. You will need to provide the promotion with permission
to your name and email address. By subscribing, you accept these official rules. One
sweepstakes entry is allowed per person throughout the duration of the promotion.

Prize: One (1) winner will be selected in a random drawing (the Drawing) for a chance to win
the prize (the Prize), which consists of one (1) set of four (4) tickets to the August 22, 2014
Packers preseason football game. Each Prize package is valued at approximately $320.

All decisions on all matters pertaining to the Drawing are final. WINNERS ARE
RESPONSIBLE FOR TAX AND OTHER FEES ASSOCIATED WITH PROMOTION. WDA
will not cover transportation or any other costs related to this Promotion.

Winner Selection: A random selection of the winner will occur on or about August 15, 2014
from among all eligible entries received (Those who subscribe to the Tongue n Cheek and
Teeth, too! patient e-newsletter). Winners must be subscribed to the Tongue n Cheek e-
newsletter by 11:59 p.m. CT on August 14, 2014. Winners will be announced on the WDA
Facebook page Wall. The potential winner will be contacted by the WDA and have by 8 a.m.
CT by August 19, 2014 to provide their full name, a daytime phone number and current address
to claim their prize. The Sponsor will notify the winners how to claim their Prize.

No Prize transfer, assignment or substitution by winners permitted. If any potential winner
declines the Prize, fails to claim the Prize, is unavailable for Prize fulfillment, fails to abide by
the Official Rules, or is ineligible, Sponsor may select an alternate winner from all remaining
eligible entries.

Caution: ANY ATTEMPT BY AN INDIVIDUAL TO DELIBERATELY DAMAGE ANY
WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS PROMOTION
MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS. SPONSOR RESERVES THE
RIGHT TO DISQUALIFY SUCH INDIVIDUALS MAKING SUCH AN ATTEMPT, VOID
HIS/HER ENTRIES, AND SEEK DAMAGES FOR ANY SUCH INDIVIDUALS TO THE
FULLEST EXTENT PERMITTED BY LAW. The Wisconsin Dental Association, Inc. is not
responsible for (i) electronic transmissions that are inaccurate or were not successfully received;
(ii) any problems or technical malfunctions, errors, interruptions, deletions, defects, delays in
operation or transmission, communication failures or human error that may occur in the
transmission, receipt or processing of entries, or for destruction or unauthorized access to, or
alteration of, entries; (iii) failed or unavailable hardware, network, software or telephone
transmissions, damage to entrants or any persons computer and/or its contents, or causes
beyond the Promotion Parties reasonable control that jeopardize the administration, security,
fairness, integrity or proper conduct of this Promotion; or (iv) any other errors or problems of
any kind relating to or in connection with the Promotion, whether computer, network, technical,
mechanical or otherwise, including, without limitation, errors or problems which may occur in
connection with the administration of the Promotion, the processing of entries, the announcement
of the winner or in any Promotion-related materials. Sponsor reserves the right to cancel, modify
or terminate the Promotion if fraud, misconduct or technical failures destroy or threaten the
integrity of the Promotion, or if a computer virus, bug or other technical problem or unauthorized
human intervention corrupts the administration, security or proper conduct of the Promotion as
determined by the Promotion Parties, in their sole discretion. In the event of early termination,
Sponsor may, at its sole discretion, select the potential winner randomly from all eligible, non-
suspect entries received as of the time/date of such termination.

Additional Rules & Restrictions: By participating in this Promotion, entrants agree to abide by
and be bound by these Rules and by all decisions of the WDA, which decisions shall be final and
binding in all matters relating to this Promotion. In the event the winner is found to be in
violation of any of these rules, he/she will be required to forfeit the Prize. Acceptance of the
Prize constitutes permission for the WDA and its designees to use winners name, city and state
of residence and/or likeness for purposes of advertising and trade in any and all media (now or
hereafter known) worldwide in perpetuity, without limitation or further compensation,
notification or permission, unless prohibited by law. By participating, entrants assume all
liability in connection therewith and further agree to release and hold the WDA and all other
parties involved with the Promotion harmless from and against any and all claims, damages and
liability of any kind arising from or in connection with, in whole or in part, directly or indirectly,
the acceptance, possession and/or use or misuse of the Prize and/or participation in the
Promotion.
